항암화학요법으로 인한 말초신경병증 환자의 한방치험 1례

Abstract

The purpose of this study is to report a clinical case of a patient with CIPN (Chemotherapy Induced Peripheral Neuropathy) successfully treated with Korean medicine interventions including acupuncture, herbal medicine and moxibustion. The patient is a female with CIPN who was diagnosed with ascending colon cancer and suffering from paresthesia of extremities after chemotherapy of platinum compounds. The patient was treated mainly with electroacupuncture (Ex-LE 10), indirect moxibustion and herbal medicine (Ucha-Shinki-hwan granule). The clinical outcomes were measured by NCI CTCAE grade, EORTC QLQ-C30 (CIPN20) scale and patient's assessment of symptoms. In this case, the NCI CTCAE grade was improved from II to I. The scores of global health status and functional scale in QLQ-C30 were increased and symptom scale was decreased. Especially, this case shows a noticeable decrease in sensory scale in QLQ-CIPN20. In patient's general assessment, the scale was changed from 10 to 5. Korean medical interventions including acupuncture, herbal medicine and moxibustion could potentially be an effective treatment for CIPN if further researches are conducted.

서 론

항암화학요법은 암의 주된 치료방법 중의 하나이나, 이러한 치료는 암세포뿐만 아니라 정상세포에도 영향을 주어 환자에게 전신 부작용을 유발할 수 있다. 그 중 말초신경병증은 항암화학 요법을 받는 환자의 30-40%에서 발생하며, 영향 인자는 항암제의 누적 용량, 강도, 치료 기간, 다른 신경독성 약제와의 동시 투여, 환자의 연령, 당뇨, 알코올 남용과 같은 이전 병력 등이다1). 증상은 말초신경계의 손상으로 인한 신경병증성 통증, 감각저하 등이 주를 이루며, 이는 항암 환자의 삶의 질을 저하시키는 심각한 요인이 되고 있다. 항경련제, 마약성 진통제 등의 약물요법과 수기치료, 운동요법 등의 비약물요법이 치료법으로 사용되고 있지만 그 효과에 대한 근거는 부족한 실정이다2,3).

항암화학요법 후 발생한 말초신경병증의 한의학적 치료에 대한 보고 중 침치료에 대해서는 증례 및 임상연구가 있으나 치료 방법과 평가 방법 등이 다양하여 아직까지 그 효과에 대한 근거는 미흡하다4-8).

한약치료에 대한 연구로는 동물모델에서 석창포, 감국, 백과, 패모가 증상 개선 효과를 보였으며9-12), 임상연구에서 보양환오탕, 시호가용골모려탕, 우차신기환, 계지가출부탕의 통증 개선 혹은 신경 보호 효과가 보고된 바 있다13-16). 기타 치료로 약침 및 봉독크림을 통해 증상이 호전된 연구결과들이 있었다17,18).

실제 임상에서는 복합치료가 적용되고 있는 만큼 본 논문에서는 침, 뜸, 한약 치료를 병행하여 항암화학요법 시행 후 발생한 말초신경병증이 호전된 사례를 보고하고자 한다. 환자의 상태 및 항암제의 종류에 따라 증상과 예후가 다양한 만큼 향후여러 치료 방법이 제시되고 이에 대한 근거가 확보된다면 암환자의 삶의 질을 높이는 데에 많은 도움이 될 것이다.

 

증 례

1. 환자정보: 47세, 여자, 163cm, 57.9kg

2 진단명

Ascending colon cancer, CIPN (Chemotherapy-induced peripheral neuropathy)

3. 주소: Paresthesia (hand and foot), Fatigue

4. 현병력: 2012년 10월 16일 ascending colon cancer(pT4aN1aM0) 진단 하에 2012년 10월 23일 근치적대장절제술(Laparoscopic extended right hemicolorectomy)을 시행하였으며, 2012년 11월 21일부터 2013년 5월 10일까지 12차에 걸쳐 FOLFOX (Oxaliplatin + Leucovorin + 5-FU) 보조항암화학요법을 받았다. 이후 수족부의 저림 심화되고 피로감 지속되어 2013년 6월 7일 OO대학교 한방병원에 입원하여 6월 21에 퇴원하였다. 진단 및 항암화학요법과 관련한 자세한 환자 정보는 Table 1에 기재하였다.

7. 초진 시 검사소견

1) 四診 소견

2) 주소증

(1) 수족 저림 및 이상감각: 본 환자는 항암화학요법 10회 이후부터 양측 수족부에 저림 및 냉감이 발생하였다. 이후 점차 증상의 정도가 심해져 11회와 12회 항암화학요법 이후에는 각각 5일간 항경련제인 neurontin을 매일 300 mg 이상 복용하였으나 증상의 호전은 없었다. 본원 입원 시 수족부의 증상은 NRS 3~4 정도이고 특히 수지와 족지 말단부에 찌릿한 통전감이 있어 물건을 집기가 불편하며 5분 이상의 보행이 힘든 상태였다.

(2) 피로: 항암치료 이후부터 평소대비 50% 정도 피로감 증가하였으며, 본원 입원 시에는 기력저하로 활동을 기피하는 편이었다.

8. 치료 및 처치

1) 침 치료

입원기간 동안 1일 1회 총15회의 침치료를 시행하였으며 각 혈자리에 0.5~1.0 cm 깊이로 자입(동방침구제작소, 0.20 mm x 0.30 mm. stainless)하여 득기 후, 20분간 유침하였다. 풍지, 중완, 하완, 천추, 외관, 신문, 태계, 족삼리, 태충, 팔풍에 자침하였으며 양측 팔풍에는 전침(slow and fast mode, 20/100Hz)을 적용하였다.

2) 뜸 치료

입원기간 동안 1일 1회 총15회의 간접애주구를 적용하였다. 양지, 외관, 태충, 임읍, 용천, 족근부에 강화미니뜸(이화당, 한국) 중 강도 微(약 50℃)인 것을 사용하여 시술하였다. 1회 시술시 각 혈위에 3장씩 시행하였다.

3) 한약 치료

입원기간 동안 1일 3회 총14일간 보신지 과립(쯔무라제약, 일본) 1포를 식후 30분에 투약하였다. 보신지 과립 1포의 원료약품 분량은 택사 3 mg, 우슬 3 mg, 지황 5 mg, 정제부자 1 mg, 차전자피 3 mg, 계피 1 mg, 목단피 3 mg, 복령 3 mg, 산약 3 mg, 산수유 3 mg 이다.

9. 평가 방법

1) Global Assessment (G/A)

입원 당시 환자가 느꼈던 통증 및 불편감을 ‘10’으로, 통증 및 불편감이 없는 상태를 ‘0’으로 가정했을 때, 치료 이후 환자가 자각하는 증상의 정도를 숫자로 표현하도록 하였다.

2) NCI CTCAE (Ver. 4.03)

National Cancer Institute (NCI) 에서 정의한 Common Toxicity Criteria for Adverse Event (CTCAE) 의 하위항목중 말초신경병증 관련 항목은 Grade I~IV로 구분되어 I: Mild symptoms or asymptomic, II: Moderate ; limiting instrumental ADLs(Activities of Daily Living), III: Severe ; limiting self-care ADLs, IV: Life-threatening consequence로 정의되어 있다. Grade I 은 의학적 처치를 필요로 하지 않는 수준이나, Grade II 이상에서는 의학적 처치를 권장하고 있다19).

3) EORTC QLQ-C30

European Organization for Research and Treatment of Cancer (EORTC) 에서 개발한 Quality of Life Questionnaire-Core 30 (QLQ-C30)은 암환자의 삶의 질을 파악하는 도구로 전반적인 삶의 질(General health status), 기능영역(Functional scale), 증상영역(Symptom scale)의 3가지 하위영역으로 이루어져 있다. 점수는 지침에 따라 100점을 만점으로 환산하여 삶의 질 영역과 기능영역 점수가 높을수록, 증상영역 점수가 낮을수록 삶의 질이 좋은 것을 의미한다.

4) EORTC QLQ-CIPN20

uality of life questionnaire chemotherapy-induced peripheral neuropathy (CIPN20)은 QLQ-C30의 하위 모듈로 Postma 등20에 의해 개발되었고 한국어로 번역한 것을 사용하였다. 이 도구는 총 20개의 문항으로, 운동영역 8문항, 감각영역 9문항, 자율신경영역 3문항으로 구성되어 있으며, 점수는 지침에 따라 100점을 만점으로 환산하여 점수가 높을수록 삶의 질이 낮은 것을 의미한다.

10. 치료 결과

1) 증상 및 G/A

치료 4일째에 수장부 저림이 G/A 10에서 G/A 7로 감소하였으나 수지와 족지 말단부의 찌릿한 느낌과 족부의 냉감은 지속되고 있었다. 치료 7일째에는 안정 시 저림이 많이 감소되었으나, 스마트폰 구동 등의 수지 동작 시에 말단부가 여전히 저리다고 하였다. 치료 9일째에는 손바닥과 발바닥을 만졌을 때 느껴지던 둔하고 먹먹한 느낌이 완화되고 수족부 모두 저림 증상이 G/A 6으로 감소하였다. 치료 12일째에는 오래 보행하거나 기립했을 때 발바닥에 느껴지던 냉감과 부종감이 경감되었다고 하였고, 치료 15일째인 퇴원일에는 수족부의 저림 증상이 G/A 5 정도로 감소하였다. 치료기간에 따른 G/A 변화는 Fig. 1에 그래프로 나타내었다.

2) CTCAE

치료 전 ‘Paresthesia(이상감각)’ 항목에서 Grade II(Moderate symptoms; limiting instrumental ADLs) 정도의 증상을 호소하였으나, 치료 후 Grade I(mild symptoms)으로 호전되었다.

3) EORTC QLQ-C30

전반적인 삶의 질 점수와 기능영역 점수가 증가하고, 증상영역의 점수가 감소하여 환자의 삶의 질이 향상되었음을 보여주었다. 치료 전 후의 항목별 점수는 Table 2에 기재하였다

4) EORTC QLQ-CIPN20

감각영역, 운동영역, 자율신경계 영역의 점수가 모두 감소하여 말초신경병증 증상이 호전되었음을 보여주었다. 치료 전 후의 항목별 점수는 Table 2에 기재하였다.

 

고 찰

항암화학요법으로 인한 말초신경병증은 platinum compounds, taxanes, vinka alkaloids, bortezomib, thalidomide 등의 항암제에 의해 유발되는 것으로 알려져 있으며, 증상의 정도와 예후는 약물의 종류, 용량, 투여기간 등에 따라 다양하다21. 본 연구의 환자는 platinum 계열의 oxaliplatin이 포함된 항암화학요법을 받았는데, oxaliplatin은 약제 투여 중 또는 직후에 발생하여 1주 이내에 증상이 소실되는 급성 말초신경병증과 치료 종료 이후 장기간 지속되는 만성 말초신경병증을 유발할 수 있다. 급성 신경병증의 경우 증상의 정도는 심하지 않으며 화학요법 치료를 받은 환자의 60~90%에서 발생한다. 만성증상은 oxaliplatin의 누적 사용량이 높을수록 나타나기 쉬우며 증상은 기온저하에 의해 악화되는 수족부위 이상감각이나 감각저하가 주를 이루고 발생률은 10~18%이다22).

말초신경 손상의 객관적인 정량 검사로는 Electromyography (EMG), Nerve Conduction Studies (NCS), Quantitative Sensory Tests (QST) 등의 신경생리학적검사가 있으나23) 이러한 검사들은 항암치료 도중 항암제 용량조절 및 중단을 결정하기 위해 필요하다고 판단될 때 주로 사용하는 것으로 본 연구의 증례에서는 항암치료가 거의 종료되는 시점에 증상이 발현되어 별도로 시행되지 않았다.

항암치료로 인한 말초신경병증에 대한 평가는 주로 환자 설문평가로 이루어지는데, 본 증례에서는 일반적으로 사용되고 있는 NCI-CTCAE의 분류와 EORTC QLQ-CIPN20를 적용하였다. Ellen M. 등의 연구24)에서 신경독성이 있는 항암화학요법을 받은 환자의 QLQ-CIPN20 평균 점수는 감각영역 20.17점, 운동영역 14.33점, 자율신경계영역이 3.49점이었으며, 항암화학요법을 받은 대장암 환자를 대상으로 한 국내 연구25)에서는 감각영역 점수가 32.7점, 운동영역 16.12점, 자율신경영역 22.88점이었다. 본 증례에서 치료 전 감각영역 점수는 48.15점으로 기존 연구의 평균점수보다 높은 반면 운동영역의 점수는 4.17점으로 낮았다. 이는 oxaliplatin 에 의해 유발되는 신경병증은 운동신경 증상은 거의 없고, 자율신경계 증상은 간헐적으로 나타나며, 이상감각이나 감각저하 등의 감각신경 증상이 주를 이룬다는 기존 연구 결과25)와 일치한다.

Oxaliplatin에 의한 만성 말초신경병증의 자연경과는 평균적으로 항암화학요법 종료 후 13주가 지나서 서서히 회복되기 시작하며 10%의 환자에서 2년 이상 증상이 지속되기도 한다22). 그러나 본 증례의 환자는 2주간의 치료로 NCI-CTCAE(Gr II → I) 및 EORTC QLQ-CIPN20(감각영역 48.15 → 14.81점)상 조기 회복을 보였다. 경과 판정은 항목별 점수 변화가 5-10점인 경우 약간의 변화, 10-20점은 중간 정도, 20점 초과는 상당한 변화로 분류되는데26), 감각영역에서 30점 이상의 경감을 보여 임상적으로 의미있는 호전이라 할 수 있다. 퇴원 후 외래 진료를 통해 경과를 관찰한 결과, 남아있던 증상이 서서히 경감되어 4개월 뒤에는 거의 소실되고 족부 저림만 간헐적으로 남아있었다.

항암화학요법으로 인한 말초신경병증의 치료는 약물요법으로 칼슘과 마그네슘, 비타민 E, 삼환계 항우울제, 항경련제, 아세틸-L-카르니틴(Acetyl-L-carnitine), 글루타민(Glutamin), 글루타티온(Glutathione), 마약성 진통제 등이 있고, 비약물요법으로 침, 보조기, 운동 요법, Pulsed infrared light therapy(PILT), 경피신경전기자극(Transcutaneous nerve stimulation, TENS), 캡사이신 연고, 수기치료 등이 있지만 실제 암 환자를 대상으로 한 연구가 부족하여 근거가 제한적인 실정이다2,28).

항암화학요법으로 인한 말초신경병증의 침치료에 대한 임상연구는 증례4-6)와 대조군 연구6-8)가 보고된 바 있으며, 주로 곡지, 외관, 합곡, 족삼리, 태충, 팔풍 등의 혈위가 사용되었다. 대체로 침치료가 증상 개선 효과를 보였고, cobamamide나 neurotropin 등의 신경보호 치료를 받은 대조군보다 증상의 개선률이 높게 나타나기도 했으나6,7), vitamin B1/B6치료 및 위약대조군과 유의한 차이가 없다는 결과8)도 있었다. 전침을 적용한 임상연구에서는 족임읍-양릉천, 태충-음릉천, 합곡-곡지, 후계-소해에 전침을 적용한 그룹에서 증상이 호전되었으나 위약 대조군과 유의한 차이는 없었다8)본 증례에서는 통증 발생 인접 부위인 팔풍혈에 전침 자극을 가함으로써 척수의 신경전달인자 조절을 통한 진통 효과를 기대하였다. 특히 저빈도와 고빈도의 교대전류가 내인성 오피오이드 시스템에서 신경전달물질인 endomorphin과 dynorphin의 상보작용을 유도하여 통각억제 효과를 나타낸다는 연구 결과29)에 따라 20Hz와 100Hz의 교대전류를 적용하였다. 그러나 동물 모델 연구에서 paclitaxel 치료 후 발생한 신경병증에 100Hz 보다 10Hz에서 통증 억제 효과가 높았고30), 신경병증성 통증에 고주파보다 저주파의 진통 효과 및 지속시간이 길다는 보고31)가 있어 향후 치료 시 주파수를 변경해볼 필요가 있다.

환자가 이상감각 중 특히 냉감을 호소하여 족저부에 간접애주구를 적용한 것도 치료에 도움을 준 것으로 여겨진다. 기존 연구에서 단독 뜸 치료 효과에 대한 보고는 없으나 관원, 또는 증상 발현 부위 인접 혈위에 뜸 치료가 병행된 경우들은 있다32,33). 또한 본 환자는 수족저림과 피로감 외에도 만성 비염으로 인한 콧물, 전신 부종감 등을 호소하며 脈沈弱, 舌淡하여 腎氣虛寒으로 보고 우차신기환 과립제인 보신지(쯔무라제약, 일본)를 투약하였다. 항암치료로 인한 말초신경병증의 한약치료 중에는 우차신기환에 대한 연구가 가장 활발히 진행되었는데9-16), oxaliplatin 투여 직후 또는 병행하여 우차신기환을 복용한 군이 무처치 또는 Ca2+/Mg+ 처치 대조군보다 신경병증 발생률이 현저히 낮았다14,15). 또한 paclitaxel이나 carboplatin 치료를 받은 암환자에서 vitamin B12단독 투여군보다 우차신기환 병행 투여군에서 6주 후 신경병증 발생률이 유의하게 낮았다16). 한편 우차신기환이 말초 통각수용체의 민감도를 낮추고 혈관확장 등을 통한 말초 순환 개선으로 진통효과를 유도하며, 신경축삭 손상예방과 신경돌기 재생 및 시냅스 형성 보조로 손상된 말초신경의 회복을 돕는다는 작용기전에 대한 연구도 있었다13).

이상 본 증례에서 항암화학요법으로 인한 말초신경병증에 한약치료, 침치료, 팔풍혈 전침, 뜸치료가 수족부의 저림 및 이상감각 증상을 호전시키는 효과가 있음을 확인하였으며 고찰 결과 침치료와 우차신기환 관련 연구에서 그 근거를 확인할 수 있었다. 더불어 안전성만 확보된다면 향후 연구에서 이미 신경손상이 진행된 항암치료 종료 후가 아닌, 항암치료 중에 한의학적 치료를 병행함으로써 더 큰 효과를 기대해 볼 수 있을 것이다.

 

결 론

항암치료로 인한 말초신경병증으로 손발 저림 및 이상감각을 호소하던 환자를 대상으로 팔풍혈 전침 위주의 침치료, 우차신기환을 비롯한 한약치료, 하지 부위 뜸치료 시행 후 증상의 호전을 확인하였기에 이를 보고하는 바이다.
