  • Cellular senescence is a process by which cells enter a state of permanent cell cycle arrest. It is commonly believed to underlie organismal aging and age-associated diseases. However, the mechanism by which cellular senescence contributes to aging and age-associated pathologies remains unclear. Recent studies showed that senescent cells exert detrimental effects on the tissue microenvironment, generating pathological facilitators or aggravators. The most significant environmental effector resulting from senescent cells is the senescence-associated secretory phenotype (SASP), which is constituted by a strikingly increased expression and secretion of diverse pro-inflammatory cytokines. Careful investigation into the components of SASPs and their mechanism of action, may improve our understanding of the pathological backgrounds of age-associated diseases. In this review, we focus on the differential expression of SASP-related genes, in addition to SASP components, during the progress of senescence. We also provide a perspective on the possible action mechanisms of SASP components, and potential contributions of SASP-expressing senescent cells, to age-associated pathologies.

  • Mitochondria have long been recognized as ATP engines for the cell and recently as a dynamic and mobile organelles that control cell death and life. This exquisite organelle is the site of reactive oxygen species production and is highly vulnerable to exogenous stresses, resulting in catastrophic damage to the cell. Mitochondrial dysfunction is linked to a wide range of age-associated degenerative diseases, such as metabolic syndrome, cardiovascular disease, and neurodegenerative diseases. Understanding the molecular mechanisms of mitochondria-dependent pathogenesis may provide important strategies to treat these diseases. Indeed, mitochondria are emerging therapeutic targets for the mitochondria-related diseases. In this paper, we review the recent concepts of mitochondrial biology and how mitochondria are involved in age-associated degenerative diseases. Furthermore, we summarize the therapeutics which target to improve mitochondrial functions.

Association between age at menarche, oral health behavior and oral health status (초경연령과 구강건강행태 및 구강건강상태와의 관련성)

  • Objectives: This study aimed to examine the association between age at menarche and two major oral diseases. Methods: Data from the 7th Korean National Health and Nutrition Examination Survey (2016-2018) were used. The participants included girls and women aged 12 years and above. Thedataofa total of 7,420 participants were analyzed. Statistical analyses were performed using complex samples cross-tabulation analysis and complex samples general linear model. Results: The average age at menarche of those aged 12 to 80 years was 13.81 years. The age at menarche was found to be associated with decayed-missing-filled teeth (DMFT) (p<0.05). When the age at menarche was nine years, DMFT was 4.20 when it was 19 years, DMFT was 10.80. The age at menarche was found to be associated with periodontal tissue state (p<0.05). The group with early menarche age had healthier periodontal tissues (17.1%), whereas the group with late menarche age had more unhealthy periodontal tissues (17.9%) (p<0.001). Conclusions: The age at menarche was associated with two major oral diseases. Therefore, it is necessary to provide women with oral hygiene care and oral health education to prevent major oral diseases before their secondary sex characteristics appear.

Analysing Risk Factors of 5-Year Survival Colorectal Cancer Using the Network Model

  • The purpose of this study is to identify the factors that may affect the 5-year survival of colon cancer through network model and to use it as a clinical decision supporting system for colorectal cancer patients. This study was conducted using data from 2,540 patients who underwent colorectal cancer surgery from 1996 to 2018. Eleven factors related to survival of colorectal cancer were selected by consulting medical experts and previous studies. Analysis was proceeded from the data sorted out into 1,839 patients excluding missing values and outliers. Logistic regression analysis showed that age, BMI, and heart disease were statistically significant in order to identify factors affecting 5-year survival of colorectal cancer. Additionally, a correlation analysis was carried out age, BMI, heart disease, diabetes, and other diseases were correlated with 5-year survival of colorectal cancer. Sex was related with BMI, lung disease, and liver disease. Age was associated with heart disease, heart disease, hypertension, diabetes, and other diseases, and BMI with hypertension, diabetes, and other diseases. Heart disease was associated with hypertension, diabetes, hypertension, diabetes, and other diseases. In addition, diabetes and kidney disease were associated. In the correlation analysis, the network model was constructed with the Network Correlation Coefficient less than p <0.001 as the weight. The network model showed that factors directly affecting survival were age, BMI levels, heart disease, and indirectly influencing factors were diabetes, high blood pressure, liver disease and other diseases. If the network model is used as an assistant indicator for the treatment of colorectal cancer, it could contribute to increasing the survival rate of patients.

Risk Factors of Readmission to Hospital for Pneumonia in Children (소아 폐렴의 재입원에 대한 위험인자)

  • Purpose: We analyzed the risk factors affecting readmission of children with pneumonia. Methods: We retrospectively analyzed the medical records of pediatric patients admitted to the Department of Pediatrics at the Jeonju Presbyterian Medical Center from January 2007 to August 2016. We classified patients who were readmitted with pneumonia within 30 days of discharge as the readmission group and patients who were admitted with pneumonia for the first time as the first admission group. Results: Among 158 patients, the study (readmission) group included 82 patients and the control (first admission) group included 76 patients. Age, the percentage of segmented neutrophils and lymphocytes, the number of admissions in the last 12 months, the associated diseases (respiratory diseases such as asthma), and the affection of the right upper lung were analyzed as risk factors for readmission. However, based on a regression analysis, only age and associated diseases were found to be significant risk factors. The rate of readmission increased with younger age. When there were associated diseases, the rate of readmission also increased. Conclusions: Young age and associated diseases were significant risk factors for readmission for patients with pediatric pneumonia. When pediatric patients are admitted with pneumonia, if they are young and/or have associated diseases, a comprehensive approach is needed to reduce the rate of readmission with careful consideration of precise examination, treatment, timing of discharge, and follow-up.

Factors Affecting Regular Medical Services Utilization of Chronic Disease Patients - Focusing on the Hypertension, Diabetes Mellitus, Hyperlipidemia - (만성질환자의 정기적 의료이용에 영향을 미치는 요인 - 고혈압, 당뇨병, 고지혈증을 중심으로 -)

  • Objectives: This study aims to identify the factors associated with regular medical services utilization of chronic disease patients. Methods: The research selected 4,489 adults aged over 30, diagnosed with hypertension, diabetes, hyperlipidemia, hypercholesterolemia, from the Korea health panel. We analyzed states of regular medical service utilization using descriptive statistics. Multiple regression analysis was used to examine the main factors associated with regular medical services utilization in chronic disease patients. Results: In terms of socio-demographic factors, gender, age, marital status, education level, employment, household income and disability were significantly different between hypertension, diabetes, hyperlipidemia and hypercholesterolemia. Among health status and behavioral factors, number of chronic diseases, subjective health status, smoking, high risk drinking, regular meals, physical activity, obesity were significantly different. From the multiple logistic regression analysis, age, number of chronic diseases, obesity, type of chronic diseases were associated with regular medical services utilization. Conclusions: It is necessary to develop effective health education programs and individualized approach to improve continuous management in chronic diseases patients.

Age and Gender Differences in the Relation of Chronic Diseases to Activity of Daily Living (ADL) Disability for Elderly South Koreans: Based on Representative Data

  • Objectives: This study investigated the gender and age differential effect of major chronic diseases on activity of daily living (ADL) disability. Methods: Surveyfreq and Surveylogistic regression analyses were employed on the 2005 Korean National Health and Nutrition Examination Survey (KNHANES) with a sample of 3,609 persons aged 65 - 89. Results: After adjusting for potential covariates, stroke, among elderly men more so than women, had a 2-3 times greater odds of engendering ADL disability in the 65-69 (p < 0.05) and 70-79 age groups (p < 0.01). In comparison to elderly women, cancer, diabetes, and incontinence in elderly men was associated with a higher risk of ADL disability in the 70 - 79 age group (p < 0.05), and this association was also observed for pulmonary disease in the 80-89 age group. Among elderly women, however, a significant association between incontinence and ADL disability was identified in all three age groups. In addition, this association was found in pulmonary disease and diabetes in elderly women aged 70 - 79 years. Significant gender differences were observed in the association between stroke in the 60 - 79 age group and cancer in the 70 - 79 age group. Conclusions: Age and gender differences were observed in the effect of chronic diseases on ADL disability.

Factors associated with access to dental care among Korean adults (성인의 구강의료접근성의 관련요인 - 구강질환의 의사진단 및 처치율을 중심으로-)

  • Objectives: The aim of this analysis was to investigate factors associated with dental visits in terms of getting diagnosis and receiving care for diagnosed dental diseases among Korean adults. Methods: Data used in this analysis were from the 2005 Korean National Health and Nutrition Examination Survey which involved a representative sample of 25,487 adults nineteen years of age and older who resided in Korea. This analysis used data of adults who had completed health interview survey (n = 25,215). Uni- and bi-variate analysis, Chi-square test, and logistic regression analyses. were conducted using SAS. Results: The percentages of people diagnosed (treated) dental canes, periodontal disease. and temporomandibular joint disorder were 70.4(77.1), 16.1 (55.7), and 0.6(37.4), respectively. The reception of treatment for diagnosed dental caries and periodontal disease was significantly associated with older age, high monthly household income, and high education attainment. Logistic regression model indicated that age, monthly household income, education attainment, type of health insurance, and chewing problem were significantly associated with getting diagnosis and treatments of dental diseases. Most frequently answered reason for non - or delayed treatments of diagnosed dental diseases during the last one year was cost of treatments. Conclusions: The findings indicated that socio-economic-status still significantly affected reception of needed dental treatments among Korean adults despite the national health insurance system. Oral health policy and programs should be augmented to provide further support to adults of low socio-economic-status who are more prone to dental diseases yet lack resources for needed dental treatments.

Health Risks in relation to Dietary Changes in Korean Americans (재미 한인의 식생활 변화와 관련된 건강 상태 연구)

  • Differences in dietary intake of Korean Americans (KA) by the length of residence in the U.S. were studied in relation to health and disease patterns in Michigan. Of 1860 questionnaires mailed, 637 (34.3%) responded with demographics, anthropometrics, self-reported disease patterns, lifestyle, and dietary patterns (93-item FFQ developed for KA). Complete responses were available for 498 people (263 men,253 women, aged 30-87 yr), who were then divided into three groups based on the number of years lived in the U.S.: 15y, 16-25y, 26y. Age-adjusted weight, height, BMI, and waist to hip ratio were within the normal ranges and did not differ among the three groups. Chronic diseases frequently reported in men and women (age-adjusted) included hypertension (14.4%, 14.1%), digestive diseases (5.4%, 8.5%), diabetes (3.6%,4.9%), and arthritis(3.3%, 12.5%), respectively. The length of residence in the U.S. was inversely associated with the age-adjusted prevalence of digestive diseases (12.4%, 6.5%, 0.4% in men, 13.0%, 11.7%, 0% in women). The length of residence was positively associated with health consciousness, receiving regular health care, taking supplements regularly, and exercising, while inversely related to smoking. The majority favored American foods for breakfast and Korean foods for dinner. Foods frequently consumed included cooked rice (w/other grains). kimchi, coffee (w/cream and sugar),bread (white and dark), citrus juice, milk (low fat and skim), lettuce and cucumber, apple, seaweed, and soda. Intake frequency of the Korean starch food group was inversely associated with the length of residence for both men and women (p<0.001), but not with the American starch food group. Fat intake did not differ by the length of residence in the U.S. The dietary changes were associated with the length of residence and chronic disease patterns for the first generation of KA, which should be further examined 'for the subsequent generation of KA.
