Anti-metastatic Effect on Cancer cell and Immune System Activation by Orally Administered Boyanghwano-tang (보양환오탕(補陽還五湯) 경구투여 후 면역 활성화에 의한 암 전이 억제 효과)

  • Objectives: This study was designed to investigate intestinal immune system activation and anti-metastatic effect on cancer cells by orally administered extracts of Boyanghwano-tang. Methods: To observe immunomodulating effects of Boyanghwano-tang on Peyer's patch cells, we measured cytokines GM-CSF, IL-4. In addition to observing effects of Boyanghwano-tang on hematopoiesis, we measured proliferation of bone marrow cells mediated by Peyer's patch cells in vitro. IgA induction activated in intestinal content and serum was measured to observe the effect of orally administered Boyanghwano-tang on mucosal immune system. After administering ovalbumin (OVA) with Boyanghwano-tang, Proliferation of Peyer's patch cell was measured to investigate gut immunostimulatory effect. Anti-metastatic experiments were conducted in vivo mouse model by using colon 26-M3.1 carcinoma cell. Results: The amounts of GM-CSF and IL-4 in the culture supernatant of Peyer's patch cells were significantly increased compared to the control group. The proliferation of bone marrow cell was significantly up-regualted with Boyanghwano-tang. These results indicate that oral administration of Boyanghwano-tang enhances the secretion of hematopoietic growth factors such as GM-CSF and IL-4 from Peyer's patch cells, and these cytokines also act on modulator of bone marrow cell proliferation. After orally administering OVA with Boyanghwano-tang, IgA induction and Proliferation of peyer's patch cell was up-regulated with Boyanghwano-tang. These results means orally administered Boyanghwano-tang activates intestinal immune system and has an inhibitory effect on tumor metastasis. In addition, We found that orally administered Boyanghwano-tang significantly inhibited tumor metastasis in vivo. Conclusions: Orally administered Boyanghwano-tang appears to have considerable activity on the anti-metastasis by activation of immune system.

Effects of Boyanghwano-Tang on the Development of Experimentally-induced Endometriosis in the Rats (보양환오탕(補陽還五湯)이 실험적으로 유발된 흰쥐의 자궁내막증에 미치는 영향)

  • Purpose: This study was performed to investigate effects of Boyanghwano-Tang on the development of experimentally-induced endometriosis in rats. Methods: Endometriosis was induced in rats by autotransplanting uterine tissue to the peritoneum and divided them into three groups: (1) sham-operated group(n=8), (2) surgically induced endometriosis and untreated control group (n=8), (3) surgically induced endometriosis and Boyanghwano-Tang treated group. Boyanghwano-Tang was orally administrated for 15 days after operation. Then we measured the body weight, the volume of endometriotic implants, the weight of uterus and ovary, and investigated the concentration of cytokines (MCP-1, TNF-$\alpha$, IL-$1{\beta}$, IL-6) in peritoneal fluids. Histopathology, immunohistochemistry for COX-2 and VEGF, and histochemistry for mast cell in transplanted uterine tissue were performed. Results: - The volume($mm^3$) of endometriotic implants in Boyanghwano-Tang treated group ($122.3{\pm}45.0$) was significantly decreased(p<0.05) compared with control group($222.1{\pm}109.1$). - The concentration(pg/$m{\ell}$) of MCP-1 in peritoneal fluids in Boyanghwano-Tang treated group($1026.3{\pm}196.5$) was significantly decreased(p<0.05) compared with control group($1412.5{\pm}345.7$). - The concentration(pg/$m{\ell}$) of TNF-$\alpha$ in peritoneal fluids in Boyanghwano-Tang treated group($936.5{\pm}94.3$) was significantly decreased(p<0.01) compared with control group($1126.2{\pm}139.9$). - The concentration(pg/$m{\ell}$) of IL-$1{\beta}$ in peritoneal fluids in Boyanghwano-Tang treated group($78.5{\pm}13.3$) was significantly decreased(p<0.01) compared with control group($105.3{\pm}17.6$). - The concentration(pg/$m{\ell}$) of IL-6 in peritoneal fluids in Boyanghwano-Tang treated group($107.4{\pm}15.8$) was decreased compared with control group($122.8{\pm}19.3$). - Histopathologically, proliferation of endometriotic epithelia, infiltration of inflammatory cells and angiogenesis in transplanted uterine tissue of Boyanghwano-Tang treated group were weakly observed than those of control group. - The percentage of positive epithelial layers for COX-2 in Boyanghwano-Tang treated group($51.5{\pm}14.1$) was significantly decreased(p<0.01) compared with control group($75.1{\pm}16.3$). - The VEGF expression of endometriotic epithelia, neovascular endothelia and stromal cells in transplanted uterine tissue of Boyanghwano-Tang treated group were weakly observed than those of control group. - The number of mast cells in adjacent tissue of transplanted uterine tissue in Boyanghwano-Tang treated group($75.9{\pm}13.1$) was decreased compared with control group($91.0{\pm}28.3$). Conclusion: On the basis of these results, we concluded that Boyanghwano-Tang have inhibiting effects on the development of transplanted uterine tissue. And these effects may be related with decreased production of MCP-1, TNF-$\alpha$, IL-$1{\beta}$ and decreased expression of COX-2 and VEGF by administration of Boyanghwano-Tang.

The Effect of Boyanghwano-tang Extracts on Reversible Forebrain Ischemia in Mongolian Gerbil (Mongolian Gerbil의 Reversible Forebrain Ischemia 모델에 미치는 보양환오탕의 효과)

  • Objectives : This study was carried out to evaluate the effect of Boyanghwano-tang Extracts on Reversible Forebrain Ischemia in Mongolian Gerbils. Methods : The water content of cerebral edema and the morphologicalㆍneurocytochemistrical change of neuron in MG were implemented to conduct this study. Results : The change rate of water content by ischemia in the normal group was 78.90% on the third day and 79.12% on the seventh day after an attack of ischemia. The rate in the control group was 80.34% and 81.72%. In the sample group the rate was 79.85% and 80.97%. This is a significant result. According to the result of the optical microscopic examination, the cells seen to be continuous and systematic in the pyramidal cells of hippocampus were changed into discontinuous and unsystematic forms in the control group. But compared with the control group, the cells were less damaged in the sample group. Conclusions : The data shows that the effect of Boyanghwano-tang Extracts on Reversible Forebrain Ischemia in MG is a significant result.

Improvement of unilateral hyperhidrosis in spinal cord injury following treatment with revised Boyanghwano-tang: a case report (보양환오탕가감방으로 호전된 경추 손상 유래 편측성 다한증 1례)

  • Hyperhidrosis is common complication of spinal cord injury, but localized unilateral hyperhidrosis is relatively rare disorder without autonomic disreflexia. A 52-year-old man with a 10-month history of cervical injury induced tetraplegia complained of excessive intermittent left-sided sweating. The sweating occurred by urinary retention or without any autonomic dysreflexia. The patient sweated excessively on the left face and upper body. In the point of Differentiation of Syndrom (辨證), the patient was diagnosed as Gi-Heo-Hyeol-Eo (Pi-Wei-Qi-Xu 氣虛血瘀) and was administered revised Boyanghwano-tang (reserved Bu-Yang-Huan-Wu-tang), and he was almost complaint free during 4 month about none dysreflexial hyperhidrosis.

The Effect of Modified Boyanghwano-tang on the Brain Infarction Through the Anti-apoptosis of Neuronal Cells in Ischemic Rats (가미보양환오탕이 뇌허혈모델에서 신경세포보호를 통해 뇌경색억제에 미치는 효과)

  • Objectives : The purpose of the study is to determine the neuroprotective effect of modified Boyanghwano-tang(mBHT), a traditional Korean medicine, on the transient focal cerebral ischemia in rats. Methods : Focal ischemia and reperfusion were induced by middle cerebral artery occlusion(MCAO) for 90 min, followed by 144 h reperfusion in rats. mBHT(200mg/kg body weight, p.o.) was administrated in rats once a day during reperfusion. At the end of treatment, brain infarction was measured by TTC staining, and histological change was observed by H&E staining. The expressions of Bax, Bcl-2 and cytochrome c in ischemic brains were determined by immunofluorescent analysis. Results : mBHT significantly reduced the cerebral infarct volumes of the MCAO rats. mBHT also attenuated the neuronal cell death and the expressions of pro-apoptotic molecules, bax and cytochrome c in ischemic brains. Further, mBHT significantly increased the survival time of ischemeic rats and the expression of anti-apoptotic molecule, Bcl-2 in ischemic brains. Conclusions : Our results suggest that mBHT is neuroprotective and may prove to be useful adjunct in the treatment of ischemic stroke.

Neuroprotective effect of modified Boyanghwano-Tang and the major medicinal plants, Astragali Radix and Salviae Miltiorrhizae Radix on ischemic stroke in rats (허혈성뇌졸중 흰쥐모델에서 가미보양환오탕(加味補陽還五湯)와 주요 구성약재인 황기(黃芪), 단삼(丹蔘)의 뇌신경보호효과에 대한 연구)

  • Objectives : In this study, the neuroprotective effects of modified Boyanghwano-Tang (mBHT) and the major medicinal plants, Astragali Radix(AR) and Salviae Miltiorrhizae Radix(SMR) were investigated in transient middle cerebral artery occlusion (tMCAO)-induced ischemic stroke of rats. Methods : mBHT(400 mg/kg) and AR(154 mg/kg) or SMR(62 mg/kg) water extract orally injected in rats after 90 min occlusion of MCA and then allow reperfusion to 24 h. Brain infarction was measured by TTC staining and the expressions of NOS isoforms and apoptotic molecules were determined in ischemic brain by Western blot. Results : The results showed that mBHT has stronger neuropreotective property through inhibitions of the PARP cleaved and caspase-3 activation in ischemic rats, and could reduced infarction volumes comparison of those of AR or SMR, respectively. While, AR extract has an angiogenic property through increasing the expressions of eNOS and VEGF, and SMR extract has a strong anti-inflammatory effects through inhibition of iNOS expression in ischemic brains. Conclusions : These results suggest that mBHT has multifactorial therapeutic advantages through anti-apoptosis, anti-inflammation and angiogenesis for ischemic stroke based on a synergistic combination of ingradients rather than monotherapy.

A Case Report of Korean Medicine Treatment on Clinical Symptoms in a Patient with Post-stroke Deep Vein Thrombosis (항응고제 치료에도 임상증상이 호전되지 않은 뇌경색 후 심부정맥혈전증에 대한 한의 치험 1례)

  • This case report shows the effect of Korean medicine treatment in a patient with deep vein thrombosis (DVT) after cerebral infarction who is suspected of having post-thrombotic syndrome (PTS) due to the loss of thrombus after receiving new oral anticoagulant (NOAC) treatment. The patient was treated with Korean medicine (Boyanghwano-tang and Hyulbuchuko-tang) three times a day. Acupuncture, moxibustion, infrared, and extra-physical therapy were given to the patient for 24 days. Clinical assessment - grading of edema and circumference of the left lower extremity were observed twice a week until the end of the treatment. NOAC treatment was continued for 24 days. After treatment, the patient's edema grade and circumference of the extremity improved. On the 24th day, the patient's chief complaints improved, and she requested termination of treatment. The results suggest that Korean medicine treatments, including Boyanghwano-tang, Hyulbuchuko-tang, and acupuncture therapy, can be effective in improving the clinical symptoms of DVT and PTS.

Neuroprotective Effects of Boyanghwano-tang on Intracerebral Hemorrhage-Induced Rats Using Immunohistochemistry (보양환오탕(補陽還五湯)이 흰쥐 뇌출혈 손상에 미치는 영향에 대한 면역조직화학 연구)

  • This study was performed in order to evaluate the neuroprotective effect of Boyanghwano-tang (BYT) water extract on intracerebral hemorrhage (ICH). ICH was induced by the stereotaxic intrastriatal injection of bacterial collagenase type IV in Sprague-Dawley rats. BYT was orally given once a day for 3 days after ICH. Hematoma volume and percentage edema were examined. As imflammatory markers, myeloperoxidase (MPO)-positive neutrophils infiltration and iNOS expression in the peri-ICH regions were examined using immunohistochemistry. As cellular damage markers, c-Fos, Bax, and HSP72 positive cells in the peri-ICH regions were measured also. BYT significantly reduced the hematoma volume and percentage edema of the ICH-induced rat brain. In the peri-hematoma regions, BYT significantly reduced MPO-positive neutrophil infiltration and iNOS expression of the ICH-induced rat brain. Additionally, BYT significantly reduced c-Fos, Bax, and HSP72 positive cells in the peri-hematoma regions of the ICH-induced rat brain. These results suggest that BYT plays a neuroprotective role against ICH through suppression of inflammatory responses, apoptosis and cellular damage.

The Effect of Boyanghwano-tang for Diabetic Retinopathy: A Systematic Review and Meta-Analysis (당뇨병성 망막병증에 대한 보양환오탕(補陽還五湯)의 효과: 체계적 문헌 고찰 및 메타분석)

  • Objectives : The purpose of this study is to assess the effect of using Boyanghwano-tang(BYHWT) for diabetic retinopathy. Methods : We searched randomized controlled trials(RCTs) that used BYHWT for diabetic retinopathy in 10 databases(RISS, KISS, ScienceON, KCI, OASIS, DBpia, CNKI, Wanfang Database, PubMed, Cochrane Library) on August 12, 2023. Outcome measurements included total effective rate(TER) and visual acuity as the primary outcome measurements, and the secondary outcome measurements included fasting blood glucose(FBG), 2hPG, HbA1c, ET-1, NO, etc. The indicator of stability included adverse reactions. A meta-analysis was conducted according to TER, visual acuity, FBG. Results : A total of 8 randomized control trials were selected. The treatment group which used BYHWT showed significant improvement effects in TER and visual acuity compared to the control group which used western medicine or chinese patent medicine alone. TER: (RR: 1.22, CI 1.14 to 1.31, P=<0.00001), visual acuity: (RR: 2.06, CI 1.40 to 3.03, P=0.0002). Secondary outcome measurements including FBG also showed improvement in the treatment group compared to the control group. FBG: (MD: -0.15, CI -0.43 to 0.31, P=0.28). Adverse reactions were only mentioned in one study, which showed that no adverse reactions occurred in both the treatment and control groups. Conclusions : This study suggests that BYHWT is effective for diabetic retinopathy. However these findings should be interpreted cautiously due to the insufficient number of studies and unknown or high risk of bias in the included trials.

Improved axonal regeneration by Boyanghwano-tang treatment in mice given sciatic nerve injury (좌골신경 손상 모델에서 보양환오탕 처리에 의한 축삭 재생반응성 분석)

  • While axons in the peripheral nerve can regenerate and lead to functional recovery to a certain extent after injury, its efficacy varies depending on the severity and duration of the injury. Here, we investigated the effects of Boyanghwano-tang (BYHOT) treatment on the regenerative responses in the sciatic nerves after prolonged transection and coaptation surgery. In mice given crush injury, axonal regeneration was completed when analyzed 1 week later and did not show any difference in regenerative reponses in the distal portion of the nerve between saline- and BYHOT-treated groups. In animal models with transection and reconnection, axonal regeneration was markedly retarded compared to animals with crush injury. Regenerating axons were extended into the reconnected distal portion of the nerve more actively in animals treated with BYHOT than saline controls. Cdc2 protein was similarly induced in nerves with crush injury and with transection and recollection, and its level was lower in BYHOT-treated animal than saline control when measured 2 weeks after nerve reconnection. These results suggest that BYHOT may be useful to promote axonal regeneration in the peripheral nerve after severe injury.
