Characteristics of each Sasang Constitutional Type as assessed by the Temperament and Character Inventory (TCI) (기질 및 성격 검사(TCI)를 통한 사상체질 특성 연구)

  • 1. Objective: This study was designed to analyze the characteristics of each Sasang constitutional type using the Temperament and Character Inventory (TCI). 2. Methods: A total of 135 healthy subjects (age 19-30) participated in this study, consisting of 35 Soyang-type, 66 Taeeum-type, 30 Soeum-type, and 4 Taeyang-type subjects. A Sasang Constitutional Medicine specialist determined each subject's Sasang constitutional type based on the Two Step Questionnaire for Sasang Constitution Diagnosis (TS-QSCD) and comprehensive constitutional assessment. The subjects were also asked to complete the TCI. 3. Results: Among the four temperament dimensions, the Soyang- and Taeeum-type subjects showed significantly higher scores than the Soeum-type subjects in Novelty Seeking (NS). The Soeum-type subjects showed significantly higher scores than the Taeum- and Soyang-type subjects in Harm Avoidance (HA) (Soeum type > Taeeum type > Soyang type). The Soyang-type subjects showed significantly higher scores than the Soeum-type subjects in Reward Dependence (RD). The Soyang- and Taeeum-type subjects showed significantly higher scores than the Soeum-type subjects in Persistence (P). Among the three character dimensions, the Soyang-type subjects showed significantly higher score than the Taeeumand Soeum-type subjects in Self-Directedness (SD) (Soyang type > Taeeum type > Soeum type). The Soyang-type showed significantly higher scores than the Taeeum-type in Cooperativeness (C). 4. Conclusions: Differences in temperament and character were discovered across the Sasang constitutional types. The Sasang constitutional characteristics should be investigated in larger studies in the future. Similar studies could add to the growing knowledge of Sasang constitutional typology and contribute to a more accurate Sasang constitutional diagnosis.

Comparison of Sleep by Sasang Constitutional Type Using Pittsburgh Sleep Quality Index (피츠버그 수면척도를 이용한 사상체질별 수면 비교)

  • Objectives The purpose of this study was to identify differences in sleep by Sasang constitutional type using Pittsburgh Sleep Quality Index(PSQI). Methods The subjects of this study were 3,578 people who had information on Sasang constitutional type and PSQI among the data established at the Korean Medicine Data Center. We compared each item score in PSQI (subjective sleep quality, sleep latency, sleep duration, habitual sleep efficiency, sleep disturbance, use of sleep medication, daytime dysfunction) and PSQI global scores by Sasang constitutional type using analysis of variance. Logistic regression was conducted to calculate the odds ratio of poor sleep by Sasang constitutional type. Results & Conclusions The SE type showed statistically significantly higher scores than the TE type and SY type in subjective sleep quality, sleep latency, habitual sleep efficiency, sleep disturbance, and PSQI global score, indicating poor sleep compared to other constitutional types. The odds ratio for poor sleep also shows that even after adjusting sex, age, height, and weight, the SE type was significantly higher than other constitutional types, indicating that the SE type could be a risk factor for poor sleep.

A Study on the Constitution Type-Specific Presentation of Physical Symptoms (사상체질에 따른 신체적 증상에 관한 연구)

  • 1. Objectives: This clinical study was conducted to understand the differences between Sasang constitutional types and to identify the physical symptoms presentation specific to each Sasang constitutional type. 2. Methods: In this descriptive study, 2,629 subjects (1,061 Taeeum-type, 683 Soeum-type, 885 Soyang-type) were surveyed between Nov 1, 2007 and Jul 31, 2010. The subjective symptoms experienced by the subjects were collected using a Physical Symptoms questionnaire, and the subjects were interviewed by Sasang specialists who determined the subjects' constitutional type. The data (in crude number and percentage) was analyzed on the general characteristics, Sasang constitutional type, and physical symptoms using the SPSS 17.0 software. The symptomatic presentation in men and women were analyzed on Sasang constitutional distribution using the chi-square test. 3. Results: 1) The physical symptoms specific to each Sasang constitutional type were as follows: 'frequent aphthous lesions in the oral cavity', 'lingering fatigue after sleep', 'headache', 'common cold symptoms presenting as loss of appetite or indigestion', and 'physical deterioration presenting as problems with digestion' in the Soeum type; 'physical deterioration presenting as problems in perspiration' and 'swelling and puffiness' in the Taeeum type; and 'excessive forgetfulness' in the Soyang type. 2) The physical symptoms specific to each Sasang constitutional type in men were as follows: 'Unilateral or bilateral headaches, 'frequent aphthous lesions in the oral cavity', 'common cold symptoms presenting as rhinorrhea or nasal congestion', 'common cold symptoms presenting as loss of appetite or indigestion', and 'physical deterioration presenting as problems with digestion' in the Soeum type; and 'swelling and puffiness' in the Taeeum type. 3) The physical symptoms specific to each Sasang constitutional type in women were as follows: 'common cold symptoms presenting as headaches', 'common cold symptoms presenting as loss of appetite or indigestion', 'physical deterioration presenting as problems with digestion' in the Soeum type; and 'pain in knees', 'redness of eyes', 'dryness of mouth', 'common cold symptoms presenting as coughing', 'physical deterioration presenting as problems in perspiration', 'swelling and puffiness' in the Taeeum type. 4. Conclusions: This study demonstrates that physical symptoms present in constitutional type-specific patterns. Understanding of the personal Sasang constitutional type and systematic, personalized healthcare based on constitutional typology is anticipated to contribute to improved health management strategy.

A Literature Study on Five Type Personality (Wutairen) in China: characteristic, treatment and prevention (중국 오태인 체질이론의 문헌연구: 체질 특성과 치료, 양생법)

  • Objectives Because of growing interest in personalized medicine, constitutional medicine also becomes more important. Constitutional medicine has already developed in many countries. In this study, we tried to investigate five type personality (Wutairen), a kind of Chinese constitutional medicine. Methods We did a research on five type personality using CNKI (http://www.cnki.net) up to Jan 15, 2018. Total 204 papers were searched with the keyword "Wutairen". We excluded 170 papers away from the subject or clinically studied. As a result, 34 papers were selected and analyzed. Results and Conclusions Five type personality (Wutairen) originated from Huangdi Neijing-Lingshu. There are 5 types: greater Yin person (Taiyin), lesser Yin person (Shaoyin), greater Yang person (Taiyang), lesser Yang person (Shaoyang) and Yin-yang balanced person. These are classified according to Yin-Yang content. Each type has different characteristic, treatment and prevention. Five type personality theory considers internal factors more important than external factors. Also, it considers the balance of Yin-Yang important.

Approaching Chronic Tension-type Headache through Sasang Constitutional Therapy: A Series of 3 Cases who were classified as the Soeum Type and Taeeum Type (만성 긴장성 두통을 호소하는 소음인(少陰人), 태음인(太陰人) 환자 3례)

  • Objectives We report a series of 3 chronic tension-type headache cases who were constitutionally typed as the Soeum type, Taeeum type and treated with Dangguibaekhaoguanjung-tang, Chungshimyeonja-tang or Chunghyulganggui-tang. Methods In treating 1 patient who was constitutionally typed as the Soeum type, Case 1 fell into the category of the Taeeum symptomatology, 2 patients who were constitutionally typed as the Taeeum type, Case 2 and Case 3 fell into the category of Dry-Heat symptomatology. Each patient was given Sasang medication according to protocol: Dangguibaekhaoguanjung-tang to Case 1, Chungshimyeonja-tang to Case 2, Chunghyulganggui-tang to Case 3. On admission day and approximately 2 weeks later, patients evaluated their pain and quality of life using SF-36 and HIT-6. The patients symptoms were observed daily and NRS scores were recorded daily. Results and Conclusions 2 Patients(Case1, Case2) showed improvement in their NRS scores, SF-36 scores and HIT-6 scores. 1 patient(Case3) showed improvement in NRS scores, but worse SF-36 scores and no improvement in HIT-6 scores. Treating chronic tension-type headache based on Sasang Constitutional therapy was found to be effective for managing pain. The treatment protocol suggested in this investigation should be explored and verified in future studies.

Two Cases of Repetitive Episodic Tension-type Headache Patients who were Constitutionally Typed as Soyangin and Taeeumin (반복 발작성 긴장성 두통을 호소하는 소양인(少陽人) 태음인(太陰人) 치험 2례)

  • 1. Objectives: This study reports two cases of repetitive episodic tension-type headache patients who were each constitutionally typed as the Taeeum type and the Soyang type and respectively treated with Yanggyeoksanhwa-tang and Galgeunhaegi-tang. 2. Methods: The patients' subjective and objective symptoms were observed daily, and the VAS scores for the main symptoms were recorded daily throughout hospitalization period. When deemed necessary, other assessment tools were engaged (Migraine-Specific Quality of Life questionnaire, Migraine Disablity Assessment questionnaire, Headache Impact Test-6, etc.). 3. Results: The Soyang Type patient's headache improved by the end of hospitalization, with a dramatic drop in the VAS score from VAS 10 to VAS 2. The Taeeum Type patient's headache also showed improvement, with an equally dramatic decrease in the VAS score from VAS 10 to VAS 2. 4. Conclusion: Although the two patients were experiencing the same symptoms of repetitive episodic tension-type headache, they were each given different medicinal interventions in accordance to their respective constitutional types. This series of cases is notable in that the two widely different medications (Yanggyeoksanhwa-tang and Galgeunhaegi-tang) were applied to specifically conforming pathologies (the Sogal symptomatology and the Ganyeol symptomatology, respectively) to produce equally dramatic improvements.

Impact of the Sasang Constitutional Type on the High-Sensitivity C-Reactive Protein, a Predictor of Cardiovascular Disease (사상체질(四象體質)과 심혈관질환 예측인자인 High-sensitivity C-reactive protein의 상관성에 관한 연구)

  • 1. Objectives The purpose of this study was to investigate the high-sensitivity C-reactive protein (hsCRP) related characteristics found in each Sasang Constitutional type. 2. Methods This cross-sectional epidemiological study was conducted using data from 2,842 men and women. After typing each participant into one of the four Sasang constitutional types, they were analyzed on various cardiovascular disease-related variables. 3. Results 1) The predictors of cardiovascular disease (metabolic syndrome, high risk FRS group, high risk hsCRP group) was found to show the highest prevalence in the Taeeum type. 2) In the group with metabolic syndrome, the mean hsCRP concentration in the Taeeum and Soyang types were higher than in the Soeum type. In the group without metabolic syndrome, the mean hsCRP concentration in the Taeeum type were higher than in the Soyang and Soeum types. 3) In the FRS low risk group, the mean hsCRP concentration were higher in order of Taeeum type > Soyang type > Soeum type. In the FRS high risk group, the mean hsCRP concentrations were not significantly different among the Sasang constitutional types. 4. Conclusions The results of this study suggest that the Taeeum constitutional type is a risk factor for high hsCRP and cardiovascular disease. These findings suggest that the prevention of cardiovascular disease is more important in the Taeeum type compared to other constitutional types. Moreover, preventive measures are warranted even in the Taeeum-type persons with low clinical cardiovascular risk.

  • 1. Objective We report a series of 3 cases Euphorbiae Kansui. Radix(Gam-sui) which is based on Sasang constitutional Medicine for Tension-Type Headache in Soyangin. 2. Methods We treated a 3 Soyangin patients that had Tension-Type Headache. We prescribed Euphorbiae Kansui. Radix(Gam-sui) for their physical symptoms. The improvement of their Tension-Type Headache was evaluated 3. Results After the Euphorbiae Kansui. Radix(Gam-sui) was given, the patient's Headache and physical symptoms were improved. 4. Conclusions This cases studies showed an efficient result of using Euphorbiae Kansui. Radix(Gam-sui) in the Tension-Type Headache of Soyangin.

Investigation of Genetic Evidence for Sasang Constitution Types in South Korea

  • In Sasang constitutional medicine, both disease susceptibility and drug response are considered to be related to the characteristics of an individual's physiology and psychology: a theory which is central to traditional Korean medicine. Based on such observable characteristics, Sasang constitutional medicine classifies people into four constitutional types. Genetic studies of Sasang constitution would help reveal the inheritance patterns and models of the typological traits and, moreover, help with traditional medical diagnosis and treatment. To investigate the heritable aspect of Sasang constitution, we collected various pedigrees from South Korea. The study population has 101 pedigrees composed of 593 individuals. The determination of the Sasang constitution type of each individual was performed by doctors who diagnose the Sasang constitutional type of individuals as part of their professional practice. We calculated estimates of familial correlation and heritability. Parent-Offspring pairs showed the strongest familial correlation of Sasang constitutional type, with the correlation values of 0.21 and 0.28, followed by sibling pairs with the value ranging between 0.14 and 0.25. From the heritability analysis conducted with the Variance-Component method, the heritability of TE (Tae-Eum) type, SY (So-Yang) type, and SE (So-Eum) type were 55%, 41%, and 47%, respectively. This pattern of heritability was consistent with different set of analyses, which suggest the robustness of our result. Our result clearly shows that the Sasang constitution type is heritable, and further genetic analysis based on our result will shed light on the biological mechanism of Sasang constitution.

An Analysis of Subjective Appetite Ratings across Different Sasang Constitutional Types (사상체질(四象體質)에 따른 주관적(主觀的) 식욕(食慾) 비교(比較) 연구(硏究))

  • 1. Objectives : This investigation compares the subjective appetite ratings across male groups of different Sasang constitutional types. 2. Methods : We recruited 36 male participants aged 20-36 with BMI 18.5-30. The subjective appetite ratings was assessed using VAS before and after standard meal.(6 time points : 30 Minutes Pre-prandial, Immediately before Meal, 15, 30, 60, 120 Minutes Post-prandial) 3. Results : In comparison of subjective appetite ratings between Sasang constitutional types, Soyang-type showed a pattern like high appetite, high hunger, low satiety, high 'how much I can eat', Soeum-type showed a pattern like low appetite, low hunger, high satiety, low 'how much I can eat', and Taeeum-type showed a pattern like low appetite, low hunger. 4. Conclusions : The results of this study suggest that the subjective appetite ratings could be different across different Sasang constitutional types.
