A case report of dysfunctional uterine bleeding with abdominal pain (복통(腹痛)을 동한한 붕루(崩漏) 환자 치험(治驗) 1례)

  • Purpose : Dysfunctional uterine bleeding is an abnormal uterine bleeding without an organic disease. A hormone therapy or anti-depressant medicine is chosen to treat dysfunctional uterine bleeding. However sometimes it didn't work. Hysterectomy is recommended to stop bleeding but a lot of women are afraid of it. The purpose of this study is to show the possibility of oriental medical treatment for dysfunctional uterine bleeding in this case report. Methods : The 30-year-old female patient who had suffering from uterine bleeding was treated with hormone therapy and anti-depressant medicine. However she was bleeding again. She had no organic disease in some medical examination, finally she was recommended a hysterectomy. But she didn't want the operation. In addition, she started acute abdominal pain. She wanted a oriental medical treatment and we treated her from the May 14th 2005 to the June 4th 2005 in admission. We used Yikweseungyangtang(益胃升陽湯), Ojuksan(五積散), Kwibitang hap Gyoesamultang(歸婢湯合膠艾四物湯) for herb-medication. In acupuncture therapy, ST36(Chok-Samni 足三里) SP1(Eunbek, 隱白) SP6(Samumgyo, 三陰交) SP10(Hyulhae, 血海) Liv1(Daedon, 大敦) Liv3(Taechung, 太衝) LI4(Hapgok, 合谷) was selected. In moxa therapy CV4(Kwanwon, 關元), CV6(Kihae, 氣海), CVl2(Chung-wan, 中脘) was used. Results : After oriental medical treatment for 22 days, she stopped uterine bleeding and recovered her health. Conclusion : Oriental medical treatment is expected to have an effect on dysfuctional uterine bleeding with abdominal pain.

  • The purpose of this study is to present the two treatment case of patient with dysfunctional uterine bleeding. Each patient has received the Korean Medicinal treatment including acupuncture, moxibustion, and herbal medication twice a week for a month or six months. This efficacy of treatment was evaluated with the degree of bleeding by the number of pad used per day. After finishing treatment, two patients had stopped bleeding, and their menstrual cycle returned to the normal range. Moreover, additional clinical manifestations including leukorrhea, hot flush, bloating, feeling tired and irritability which refers to the premenstrual syndrome were also alleviated. This case report showed that the acupuncture, moxibustion, and the herbal medicinal treatment can be effective for the treatment of the dysfunctional uterine bleeding.

  • Purpose : This study was designed to evaluate the effects of oriental medicine therapy on a patient of metrorrhagia and metrostaxis with endometrial polyp. Methods : The clinical data was analyzed on a patient of metrorrhagia and metrostaxis with endometrial polyp whose main symptoms were dysfunctional uterine bleeding. A 27-year-old female who had been treated hormone replacement therapy for dysfunctional uterine bleeding at a obstetrics and gynecology became worse, so refused any more hormone replacement therapy and visited Dongsin university oriental hospital. The patient was admitted to the obstetrics and gynecology department of Dongsin university Oriental Medical Hospital, on Febrary 23, 2004 and remained until Apri 11, 2005. we treated the patient with a herb-medication, acupunture therapy, moxa therapy and etc. Results : After treatment, improvement was seen in dysfunctional uterine bleeding, generalized weakness. Conclusion : The study suggests that oriental medicine therapy is significantly effective in the treatment of metrorrhagia and metrostaxis with endometrial polyp.

  • The purpose of this study is to report the hemostatic effects of Kungkwikyoae-tang for abnormal uterine bleeding. Kungkwikyoae-tang has been known as a useful herbal medication for uterine bleeding. We treated patients who had been suffering from abnormal uterine bleeding with herb-medication, acupuncture therapy, pharmacopuncture therapy and moxa therapy. We prescribed Kungkwikyoae-tang three times a day. The improvement of symptoms were measured by checking patient bleeding volume, frequency and other physical signs. After Kungkwikyoae-tang was given, abnormal uterine bleeding volume and frequency was reduced and other physical symptoms were improved. This case study shows that Kungkwikyoae-tang has a hemostatic effects against the abnormal uterine bleeding.

Traditional Herbal Medicine for Dysfunctional Uterine Bleeding versus Western Medicine: A Systematic Review of Randomized Controlled Trials (기능성 자궁출혈에 대한 한약치료 : 체계적 문헌 고찰)

  • Objectives: To evaluate the effectiveness and safety of traditional herbal medicine (THM) in the treatment of dysfunctional uterine bleeding (DUB) versus conventional western medicine. Methods: Randomized Controlled Trials (RCTs) comparing THM vs. conventional western medicine for DUB, were obtained from PubMed, Cochrane Library, Embase, CNKI, RISS, NDSL, KISS and OASIS. The risk of bias was assessed by using Cochrane's risk of bias tool. Results: 16 RCTs with 1,659 patients were identified and reviewed. 10 RCTs reported THM was statistically effective than control group in effective rate. Also recurrent rate was estimated in 6 RCTs and was lower than control group. 7 studies observed adverse events (AEs) and severe AEs were not reported. Conclusions: Despite several limitations, this review suggested that THM was safe and effective in the treatment of DUB. THM may also decrease the recurrence rate. However, this could not be proven conclusively. To ensure evidence-based clinical practice, more sternly designed trials are warranted.

  • A 50-year-old female patient who had anovulational uterine with acute heavy uterine bleeding from the 18th., October, 2006 was hosptalized from the 7th November, 2006 to the 18th, November, 2006 in Dongshindae suncheon Oriental Medical hospital. The patient had been treated with herb-meidcation, acupuncture therapy and moxa therapy. In acupuncture therapy Sp10(Hyulhae, 血海,) Liv3(Taechung, 太衝) Sp6(Samumgyo, 三陰交) Cv6(Gihae. 氣海) Cv12(chunwan, 中脘) L14(Hapgok. 合谷) L7(列缺), were used on the 7th, November, 2006. In the Herbal Therapy, Seungynaggihyultang, Gamiguibitang, was used, In the Moxa therapy, Cv6(Gigae, 氣海) Cv12(chunwnas, 中脘) were used.

  • Uterine bleeding is a physiologic monthly event occuring during the reproductive period of the nonpregnant woman. As with any other physiologic event, normal women have different menstrual patterns and at times, it is difficult to define when normality ends and abnormality begins. Dysfunctional uterine bleeding is an all-inclusive term: its diagnosis usually bepends on the exclusion of local or systemic organic causes. The term dysfunctional refers to alterations in the physiologic mechanism controlling a multitude of cyclic events in the women's reproductive function. These mechanism are primarily neuroendocrine in nature. According to oriental medicine, the Metrorrhagia(崩漏) means an abnormal bleeding of femail genetalia, and is devided to bungjung and bungha. The etiological factors of metrorrhagia are heat of blood, asthenic coldness, asthenia of chong and ren channels, deficiency of blood, deficiency of vital energy, stagnated blood, and asthenia of spleen & stomach.

  • The age of puberty represents a very critical time in the life history of every young woman. The menarche stands as primary indicator of the onset of sexual maturation in females. By late adolescence, 75% of girls experience some problem associated with menstruation. Delayed, irregular, painful, and heavy menstrual bleeding are leading reasons for physician office visits by adolescents, and dysmenorrhea is the leading reason for school absenteeism among girls. This article discusses normal menstrual function during adolescence, then reviews the clinical presentation, evaluation, and management of adolescent dysmenorrhea, dysfunctional uterine bleeding, amenorrhea, and polycystic ovary syndrome.

Exfoliation of Endometrial Cells on Cervicovaginal Smears (자궁경부 질 세포검사에서 관찰되는 자궁내막세포의 의의)

  • The significance of endometrial cells on cervicovaginal smears is underestimated. The aim of this study is to evaluate the detection rate of endometrial cells on cervicovaginal smears. The materials consisted of two groups. Group I was 701 cervicovaginal smears from patients with no gynecological problems. Group II was 208 cervicovaginal smears from patients with abnormal uterine bleeding followed by endometrial curettage; 31 cases of endometrial adenocarclnoma(CA), 19 cases of endometrial hyperplasia(HP), 83 cases of dysfunctional uterine bleeding(DUB), and 75 cases of normal endometrium. Cervicovaginal smears were reviewed according to the criteria of The Bethesda System. Endometrial cells were identified in 15 of 701 cases(2.1%) in group I and 64 of 208 cases(30.8%) in group II. Among group II, detection rate of endometrial cells was the highest in CA (51.6%) compared to HP(26.3%), DUB(41.0%), and normal endometrium(12.0%) (p<0.05). Cytologic atypia of endometrial cells was not found In group I, but was more frequently identified in CA(87.5%) than in HP(10.5%) or DUB(14.7%) (p<0.05). Exfollatlon of endometrial cells might be related to abnormal endometrial lesion, and reporting of endometrial cells in the cervicovaginal smear may increase a chance to detect endometrial lesions especially in patients with abnormal uterine bleeding.
