조구(S38)-승산(B57)혈의 전침 자극이 동결견 환자의 관절가동범위에 미치는 영향 (The Effect of Electro-acupuncture Stimulation of Tiaoko(S38)-Chengsan(B57) on Shoulder ROM with Frozen Shoulder Patient)

  • Objectives : The purpose of this study was to identify the effect of electro-acupuncture stimulation regarding restoration of range of motion on patients with frozen shoulder. Methods : Experimental group measured with shoulder flexion and abduction angle at pre and post electro-acupuncture stimulation. Results : There were statistically significant difference regarding change of flexion angle of electro-acupuncture stimulation(p<0.01) and the testing value of all levels had a significant difference with the value of final post-test(p<0.01). There were statistically significant difference regarding change of abduction angle of electro-acupuncture stimulation(p<0.01). Conclusions : Electro-acupuncture stimulation of distal part increased range of motion of frozen shoulder. Therefore, application of electro-acupuncture should be developed as a therapeutic method in treatment of frozen shoulder. Also, it can contribute as one of good therapeutic methods offered to patients through approach of oriental rehabilitation medicine.

  • Objectve : Recent stuides suggested that there is a strong correlation between acupuncture stimulation and its cortical activation. Another study showed that either positive or negative BOLD effects could be observed depending on anatomical structure in acupuncture. Methods : 1) Subjects and paradigms: Two separate stimulation paradigms were performed in this study. To see the effects of electro-acupuncture stimulation on K7(Fuliu), the experiment was carried out on six healthy volunteers, using the gradient echo sequence with the 3.0T whole-body MRI system(ISOL). After the needle insertion on right K7(Fuliu), 2 Hz of electric stimulation was given for 30 seconds, repeated five times, with 30 seconds' intervals. During the intervals while there was no electro-stimulation, the subjects performed motor task as a reference. The image analysis including motion correction, talairach transformation, and smoothing was done using SPM99. Results: The electro-acupuncture stimulation on K7(Fuliu) activated Brodmann's Areas of 9, 19, 23, 24, 31, 32, 39 which may be the central pathways of the electro-acupuncture stimulation on K7(Fuliu) and those brain areas may be related with the function of the electro-acupuncture stimulation on K7(Fuliu).

  • Objective: Recently, many studies have showed the evidences of the effect of the acupuncture treatment through scientific methods. One of these methods is functional MRI. We performed electro-acupuncture on Liv3 and observed the change of brain activation using fMRI. Methods: To see the effect of electro-acupuncture stimulation on Liv3. the experiment was carried out on 12 healthy volunteers. using the gradient echo sequence with the 3.0T whole-body MRI system(ISOL). Brain region activated by motor stimulation activates Brodman Area 4, 6, 13, 19, 42.

  • Objective : The meridian theory in oriental medicine explains that Sp9(Yinlingquan) has been considered very important for gynecological disorders, spleen and stomach disorders, and water metabolic disorders. Functional MRI is the best method of showing the evidences of the effect of the Electro-acupuncture treatment through scientific methods. On this supposition, Electro-acupuncture stimulation on Sp9(Yinlingquan) was carried out on six healthy volunteers, using the gradient echo sequence with the 3.0T whole-body MRI system(ISOL). We also hypothesize that those brain areas may be directly or indirectly related with the mechanism of the effect of electro-acupuncture stimulation on Sp9(Yinlingquan).

Zusanli(St36) is located on the lateral side of the lower leg. 3 cun(寸) inferior to the Patella of the lower border. Theoretically and clinically. this point has been considered very important for gynecological disorders. spleen and stomach disorders. and psychological disorders. To see the effects of electro-acupuncture stimulation on Zusanli(St36). the experiment was carried out on twelve healthy volunteers. using the gradient echo sequence with the 3.0T whole-body fMRI system(ISOL). After the needle insertion on right Zusanli(St36). 2 Hz of electric stimulation was given for 30 seconds. repeated five times. with 30 seconds' intervals. The Image analysis including motion correction. talairach transformation, and smoothing was done with SPM99. Results ad conclusion : The electro-acupuncture stimulation on Zusanli(St36) activates Brodmann Area 6, 13, 2, 19, 21, 22, 39, 40, 38, 3which indicates the pathways of the electro-acupuncture stimulation on Zusanli(St36) and the possibility of the relationship of the electro-acupuncture stimulation on Zusanli(St36) with autonomic nervous system, internal organic system.

  • Objective : Recently, many studies have showed the evidences of the effect of acupuncture treatment through scientific methods. We performed electro-acupuncture on Taixi(K3) and observed the change of brain activity using fMRI. Methods : To see the effect of electro-acupuncture stimulation on Taixi(K3), the experiment was carried out on twelve healthy volunteers, using the gradient echo sequence with the 3.0T whole-body fMRI system(ISOL). After the needle insertion on K3, 2 Hz of electric stimulation was given for 30 seconds, repeated five times, with 30 seconds' intervals. Result & Conclusions : Unilateral electro-acupuncture stimulation on K3 increased the brain activity in Brodman Area 8, 6, 9, 13, 21, 22, 38, 40, 43, 2, 5, 10, 20, 24, 32, 34, 37, 39, 41, 42. and decreased the brain activity in Brodman Area 9, 13, 38, 22, 31, 3, 6, 10, 21, 25, 29, 30. Group Averaged Brain activity induced by Unilateral Electro-acupuncture Stimulation on K3 was increased in Brodman Area 10, 22, 42, 43, 44 and was decreased inn Brodman Area 6 and pyrogen test of Bee venom. Bilateral electro-acupuncture stimulation on k3 increased the brain activity in Brodman Area 22, 3, 19, 4, 13, 17, 21, 41, 42, 2, 5, 6, 7, 18, 23, 29, 30, 31, 40 and decreased the brain activity in Brodman Area 18, 6, 39, 4, 13, 2, 10, 19, 36, 40, 45, 46, 47. Group Averaged Brain activity induced by Bilateral Electro-acupuncture Stimulation on K3 was increased in Brodman Area 22, 13, 4, 3, 29, 38, 43 and was decreased in Brodman Area 18, 3, 19, 22, 39. Motor Stimulation activated Brodmann Area 6, 43, 18.

극하근과 대원근에 적용한 전침이 견관절 기능 개선 및 통증 경감에 미치는 영향 (Effect of the Electro-acupuncture Stimulation into the Infraspinatus and the Teres Major for the Shoulder Function Improvement and Pain Reduction)

  • Purpose: This study examined the effect of electro-acupuncture stimulation of the infraspinatus and teres major on the shoulder function reduction. The subjects consisted of 20 female patients with a shoulder dysfunction. Methods: The 20 female patients with shoulder dysfunction participated voluntary in this research. 10 minute electro-acupuncture stimulation (frequency: 5Hz, Intensity: below the pain threshold) with Infra-red therapy, 3 times per week given to the patients' Trigger point of the infraspinatus and teres major. The Visual Analog Scale (VAS), Apley's Scratch Test and Global Perceived Effect Scale (GPES) were examined before alter six applications. Results: There was a significant decrease in the VAS (p<0.05) and Apley's Scratch Test (p<0.05). In addition, there was statistically significant increase in the Global Perceived Effect Scale (p<0.05). Therefore, the infraspinatus and teres major is a very important muscle for the shoulder function. Conclusion: This data will be used to develop a treatment method for patients with a shoulder dysfunction. These results suggest that the electro-acupuncture stimulation into the infraspinatus and the teres major Trigger point is an effective intervention for patients with shoulder dysfunction and pain.

경피전기자극을 이용한 연접 전 억제가 만성 뇌졸중 환자의 global synkinesis에 미치는 영향 (The Effect of Presynaptic Inhibition Using the Transcutaneous Electrical Stimulation for Global Synkinesis on the Post-Stroke Hemiplegic Patients)

  • Purpose : This study was performed as follows in order to investigate the effect of presynaptic inhibition mechanism using the transcutaneous electrical stimulation (TES) for global synkinesis (GS) on the post-stroke hemiplegic patients. Methods : The subjects consist of 38 post stroke hemiplegic patients; experiments were performed on thirty patients excluding eight. The experiment was performed on sham group, sensory level stimulation group, and motor level stimulation group for 20 minutes a day 5 times a week for 6 weeks total. We compared the differences in GS levels and walking ability. The measurements were carried out pre, immediated, post 10th, and 20th, for a total of four measurements. Results : The GS level using sEMG found significant differences between groups at the post 10th and post 20th in dorsiflexion, and post 20th in plantarflexion (p<0.05, p<0.01). The motor level group indicated more significant differences when the number of electrical stimulations increased. TUG and 10 m walking test indicated a significant difference at immediated, post 10th, and post 20th. The motor level group showed more significant decreasing tendency than the sensory level group. Conclusion: From these results, electrical stimulation using presynaptic inhibition mechanism of transcutaneous electrical stimulation (TES) had positive effects for walking ability on inhibition of muscle tone in lower extremity. The motor level stimulation group experienced a more significant effect than the sensory level stimulation group. Therefore, the transcutaneous electrical stimulation (TES) is considered to be effective on walking ability increasing through inhibition of muscle tone in lower extremity for rehabilitation of post stroke hemiplegic patients.
