An Insufficient Preoperative Diagnosis of Borrmann Type 4 Gastric Cancer in Spite of EMR

  • Borrmann type 4 gastric cancers are notorious for the difficulty of finding cancer cells in the biopsy samples obtained from gastrofiberscopy. It is important to obtain the biopsy results for making surgical decisions. In cases with Borrmann type 4 gastric cancer, even though the radiological findings (such as an upper gastrointestinal series, abdominal computed tomography and positron emission tomography/computed tomography) or the macroscopic findings of a gastrofiberscopy examination imply a high suspicion of cancer, there can be difficulty in getting the definite pathologic results despite multiple biopsies. In these cases, we have performed endoscopic mucosal resection under gastrofiberscopy as an alternative to simple biopsies. Here we report on a case in which no cancer cells were found even in the endoscopic mucosal resection specimen, but the radiologic evidence and clinical findings were highly suspicious for gastric cancer. The patient finally underwent total gastrectomy with lymph node resection, and she was pathologically diagnosed as having stage IV gastric cancer postoperatively.

  • We report a case of gastric pseudoterranoviasis proven by gastrofiberscopy on Dec. 13, 1994. The 34-year-old male patient, residing in Chungju-shi, was admitted to Konkuk University Hospital complaining of prickling epigastric pain. The symptoms suddenly attacked him two days after eating raw marine fish at Chonan-shi. By the gastrofiberscopic examination, a long white-yellowish nematode was found from the fundus region of stomach. The worm was $34.50{\;}{\times}{\;}0.84{\;}{\;}mm$ in size. and was identified as a 3rd stage larva of Pseudoterranova decipiens judging from the position of the intestinal cecum. This is the 12th confirmed case of human pseudoterranoviasis in Korea.

  • Gastric complications following unintentional foreign body ingestion are extremely rare. Here, we report the case of a 59-year-old healthy woman who presented with nonspecific abdominal pain and an apparent gastric submucosal tumor that was incidentally detected by gastrofiberscopy. The patient underwent laparoscopic surgery, which revealed an intact gastric wall with no tumor invasion, deformity, or evidence of a gastric submucosal lesion. However, an impacted fish bone was found.

  • Esophageal leiomyosarcoma is a rare lesion. [0.5 % of all esophageal malignancy] The patient was 74 year old female and complained postprandial vomiting of 4 years, duration & hematemasis of 2 months` duration. On gastrofiberscopy, huge exophytic mass nearly occluding esophageal lumen located 30∼38 cm from upper incisor was found. Endoscopic biopsy was squamous cell carcinoma. Transthoracic esophagectomy and esophagogastrotomy was carried out. Result of biopsy was esophageal leiomyosarcoma.

  • 7 cases of Acute Gastric Anisakiasis have been reviewed at the Department of Internal Medicine. Yeungnam University Hospital from March, 1983 to December 1986. 1. Of 7 cases, the ratio of male and female was 3:4 and the age 30s occupied most (5 cases) the 50s' followed (2 cases). 2. Chief complains were in order of epigastric pain (6 cases), vomiting (3 cases), Nause (2 cases), Ulticaria (1 case) & epigastric fulling sensation (1 case). 3. In all 7 cases, the causative fish is Astroconcer myriaster. 4. The onset of symptoms was observed within 5 to 7 hours after eating the causative raw fish. 5. Gastrofiberscopic exam was undergo within 18 hours since onset of symptoms. 6. The gastrofiberscopic findings were gastric mucosal edema and redness in all 7 cases. One case also had mucosal erosion. The involved sites of the stomach were in frequency of order, the body (5 cases), the antrum (2 cases). 7. The symptoms of the patients relieved spontaneously without medical treatment after the extraction of the larva with biopsy forcep during gastrofiberscopy. 8. In Korea, many people take raw fish and there may be many patients of Anisakiasis so if much concern and careful observation are paid to whom, with severe cramping epigastric pain after taking of raw marine fish in about 10 hours, we could find out and confirm more patients suffering from Anisakiasis by immediate endoscopic gastrofiberscopy.

Cytomegalovirus-induced Childhood Menetrier's Disease with Peripheral Eosinophilia (호산구 증가증을 동반한 거대세포바이러스 감염 소아 Menetrier병 1례)

  • Herein, the case of a 5-year-old boy with cytomegalovirus-induced Menetrier's disease, with peripheral eosinophilia, presenting with abdominal pain and vomiting, followed by generalized edema, is reported. The initial laboratory findings, hypoalbuminemia and peripheral eosinophilia were noted, with no evidence of renal, hepatic, cardiac or allergic diseases. Gastrofiberscopy was performed under the suspicion of eosinophilic gastroenteritis with protein losing gastropathy. The gastrofiberscopy showed the characteristic features of giant hypertrophy of the gastric rugae, with large quantities of adherent gelatinous material on the gastric fundus and body. The histological findings revealed foveolar hyperplasia, compatible with Menetrier's disease, with massive eosinophilic infiltrations. The presence of cytomegalovirus infection was identified by serology and confirmed by urine PCR. His symptoms, gastrofiberscopic findings and peripheral eosinophilia resolved spontaneously, and he has remained well for 10 months.

A Case of Trichosporon beigelii Esophagitis Presenting Hematemesis with a Large Amorphous Material (토혈과 원통 모양의 토물을 주소로 내원한 Trichosporon beigelii 식도염 1례)

  • We experienced a case of Trichosporon beigelii esophagitis in a 16-month-old boy who was presented with hematemesis with a large amorphous material. A spit-out material was silky, $10{\times}1.2\;cm$ in size and like a part of hollow viscus organ. Emergent gastrofiberscopy revealed that this silky material was teared up from upper and to lower esophagus and was threw with hematemesis. It was suggested that pseudomembrane on esophagus was peeled off followed by mucosal bleeding. Pathologic study revealed this material was pseudomembrane with esophageal mucosa of T. beigelii esophagitis and was teared at lamina propria level from submucosa.

  • This BEF will be resolved with tissue adhesive application The bronchoesophageal fistula[BEF] is a rare lesion in thoracic surgical diseases & is difficult to be closed surgically. Tissue adhesives has been used widely in surgical fields, or in endoscopy, for some time and seems to be potentially useful in cardiothoracic surgery. We have experienced the closure of BEF with tissue adhesive Tisseel in 2 cases recently. One is 60 years old male who had taken the closure of BEF with Tisseel through right bronchotomy. The other is 57 years old female who had taken the closure of BEF with Tisseel with flexible gastrofiberscopy. The postoperative courses are uneventful for 4 months to now.

  • Papillary thyroid carcinoma frequently invades the lymph node, trachea, esophagus and perithyroid tissue. However, direct extension to posterior pharyngeal area is known to be rare. A 64-year-old male was referred to our clinic presenting as posterior pharyngeal mass during gastrofiberscopy. The neck CT scan showed soft tissue mass in retropharynx and lymph node in right level III with calcifications. We performed the total thyroidectomy with selective(level II, III, IV) and anterior compartment neck dissection. In operative findings, the right thyroid mass were connected to the retropharynx through the posterior portion of inferior constrictor muscle. Histopathologic findings revealed the papillary thyroid carcinoma extended to retropharynx. We report a unique case with a literature review.

Multiple Myeloma Combined with Stomach Cancer - A case report - (위암이 동반된 다발성 골수종 1례)

  • A case of multiple myeloma combined with stomach cancer and related literatures were reviewed. A 67 year-old male patient entered the hospital with dysphagia and weight loss for 3 months. Peripheral blood examination revealed anemia with rouleaux formation. Total protein of the serum was 9.9g/$d{\ell}$ with hyperglobulinemia(albumin 2.7g/$d{\ell}$, globulin 7.2g/$d{\ell}$, A/G ratio 0.375). On the electrophoresis and immunoelectrophoresis of the serum, the abnormal protein with the pattern of monoclonal gammopathy(IgG-K type) was shown. There were multiple variable sized osteolytic lesions on skull X-ray and abnormal hot uptakes of ribs on bone scan and result of rib biopsy was plasmacytoma. Gastrofiberscopy was performed to search for the cause of upper gastrointestinal bleeding, revealed stomach cancer, and the result of the gastric mucosal biopsy proved to be well-differentiated adenocarcinoma.
