The Prevalence of Gout and Its Risk Factors in Healthy Workers

  • Purpose: This study was conducted to investigate the prevalence and risk of gout in healthy workers. Methods: This cross-sectional analysis was conducted in 1,876 subjects (1,515 men and 361 women) enrolled in the Healthy Worker Cohort Study at Daegu, South Korea. Relationships between serum uric acid levels and independent risk factors were evaluated using multiple linear regression analysis after adjustment for covariates. Results: Among study participants, the prevalence of gout was 22.9% in men and 2.5% in women (based on a serum uric acid level >7.0 mg/dL for men and >6.0 mg/dL for women). Among men, increased ALT, GGT, total cholesterol, and triglycerides were associated with increased risk of gout (p<0.05) by multiple linear regression analysis; however, increased age and HDL-cholesterol/triglyceride ratio were associated with decreased risk of gout (p<0.05). In women, increased triglycerides were associated with increased risk of gout (p<0.05). Conclusion: Our findings suggest that increased triglycerides were significantly associated with the risk of gout in men and women. Moreover, a high HDL-cholesterol/triglyceride ratio in men may reduce the risk of gout.

A Clinical Case of a Recurrent Polyarticular Gout Patient Treated with Gyeji-tang-gami and Soshiho-tang-gami (계지탕과 소시호탕의 가감방으로 호전된 만성 재발성 다관절 통풍 환자 1례)

  • Objectives: This study examined a clinical case of a recurrent polyarticular gout patient treated with Korean medicine. Methods: The patient was treated with Korean medicine (Gyejigagye-tang, Soshiho-tang-gami, Gyejigajakyak-tang, acupuncture, and moxibustion), Western medicine, and rehabilitative therapy. Their effects were evaluated using the Visual Analog Scale. Results: After receiving Korean medical treatment for two admission periods, the Visual Analog Scale score for joint pain improved or disappeared. Moreover, Korean medicine was able to manage the symptoms of the gout patient. Conclusions: Proper Korean medicine treatment could be effective in improving the symptoms of recurrent polyarticular gout pain.

  • Compare western medicine report and books which refer to Gout with Oriental medicine books which refer to Tongp'ung(痛風). The results of studying about Gout and Tongp'ung(痛風) were summarized as follows. l. Dangyesimbub(丹溪心法) was printed in 1481 and also the first book which referred to Tongp'ung(痛風). 2. Donwonsipjonguiso(東垣十種醫書) was printed in 1592 and also Tongp'ung(痛風) was recorded like gout especially in cause of disease. 3. Ancient times, Affection by exopathogen and six kinds of natural factors were persisted the cause of Tongpung(痛風), later generation drink, diet and labor were persisted. 4. In Beunjungrok(辨證錄) and susyebowon(壽世保元), there were similar expressions about gout, for examples interval phase and chornic tophaceous. 5. In Uihakjongjon(醫學正傳) and Manbeunghoechun(萬病回春) insisted that gout patients had to control their food especially meat, fish snd drink. 6. According to the cause(Pung(風), Han(寒), Sub(濕)), doctor classified Bi(痺) first was Hangbi(行痺), second was Tongbi(痛痺) and last was Chakbi(着痺). 7. Doctors thought that Tongp'ung(痛風,) was not Hangbi(行痺) and Chakbi(着痺) but it was Tongbi (痛痺). Especially in pain, they thought it was similar to Bakhoyeukjolp'ung(白虎歷節風).

Factors Influencing the Performance of Self-care in Gout Patients (통풍 환자의 자가 간호 수행에 영향을 미치는 요인)

  • Purpose: This study identified the factors influencing the self-care performance among gout patients to provide the basis of the development of a nursing intervention program that can improve the self-care performance. Methods: A survey was conducted of 132 gout patients who visited the departments of rheumatology and orthopedics from September to November 2013. The data were collected using the instruments for knowledge of gout, self-care performance, and self-efficacy. Results: The gout patients' knowledge, self-efficacy, exercise pattern, and drink pattern accounted for 71.2% of the self-care performance. Among them, the most important factor affecting on self-care behaviors was knowledge levels, showing the variance of 51.0%. Conclusion: The strategy for improving the knowledge and self -efficacy level should be considered in the development of educational programs for gout patients.

  • A diagnosis of gout is often straightforward because gout has well known clinical presentations, laboratory analyses, and radiologic features. On the other hand, gout can mimic other diseases by showing a range of atypical clinical manifestations. This paper reports a 35-year-old male with no prior history of gout who developed tophaceous gout at his previously repaired Achilles tendon 11 years after surgery. He was initially misdiagnosed with cellulitis because of his atypical clinical features. This case is presented with a review of the relevant literature.

  • Gouty arthritis is a common crystal arthropathy, but gout tophus in the bipartite patella is a rare condition. This report presented a traumatic separation of bipartite patellar fragment caused by mild trauma in a patient with comorbid gout. When a patient with bipartite patella and underlying gouty arthritis complains of pain after trauma, clinical suspicion is needed about fragment separation of the bipartite patella.

  • Objective : The aim of this study is to report the effect of Scolopendrae Corpus Herbal-Acupuncture on three cases of gout. Methods : Scolopendrae Corpus Herbal-Acupuncture was administered one time per 2 days. After Herbal-Acupuncture administrated conventional body-acupuncture was performed. We evaluated the patients through Visual Analogue Scale (VAS). Results : After the treatments, three cases of gout patients showed the improvement in VAS and various symptoms. Conclusions : According to the results, Scolopendrae Corpus Herbal-Acupuncture may have rapid effects on gout. But further studies are required to prove the effect of Scolopendrae Corpus Herbal-Acupuncture.

Objectives The primary purpose of this case study is to report that a Soyangin Yin Exhausted Syndrome Patient Diagnosed as Gout was treated with 'Soyangin Hyungbangsabaek-san(荊防瀉白散)' and then his symptoms and Laboratory test(serum) improved. 2. Methods We diagnosed him as Gout on Soyangin Yin Exhausted Syndrome. So we treated his with 'Soyangin Hyungbangsabaek-san(荊防瀉白散)'. 3. Results and Conclusions The constitutional treatment with herb-medicine(Soyangin Hyungbangsabaek-san(荊防瀉白散) on Soyangin Yin Exhausted Syndrome Patient Diagnosed as Gout, his symptoms and Laboratory test(serum) improved. 'Soyangin Hyungbangsabaek-san(荊防瀉白散)' may take efffect Soyangin Yin Exhausted Syndrome.

Flexor Tendon Rupture Secondary to Gout

  • Extra-articular deposition of monosodium urate crystals is a widely recognized manifestation of gout. However, gouty infiltration of flexor tendons in the hand resulting in tendon rupture is exceedingly rare. This case report highlights a patient with gouty infiltration of flexor tendons in the right middle finger resulting in rupture of both the flexor digitorum profundus and flexor digitorum superficialis. Given the extent of gouty infiltration and need for pulley reconstruction, the patient was treated with two-stage flexor tendon reconstruction. Febuxostat was prescribed preoperatively to limit further deposition of monosodium urate crystals and continued postoperatively to maximize the potential for long-lasting results. Prednisone was prescribed between the first- and second-stage operations to prevent a gout flare while the silicone rod was in place. In summary, tendon rupture secondary to gouty infiltration is the most likely diagnosis in patients with a history of gout presenting with tendon insufficiency.

Characterization of an Anti-gout Xanthine Oxidase Inhibitor from Pleurotus ostreatus

  • We selected Pleurotus ostreatus from among several edible mushrooms because it has high anti-gout xanthine oxidase (XOD) inhibitory activity. The maximal amount of XOD inhibitor was extracted when the Pleurotus ostreatus fruiting body was treated with distilled water at $40^{\circ}C$ for 48 hr. The XOD inhibitor thus obtained was purified by Sephadex G-50 gel permeation chromatography, ultrafiltration, $C_{18}$ solid phase extraction chromatography and reverse-phase high-performance liquid chromatography with 3% of solid yield, and its XOD inhibitory activity was 0.9 mg/mL of $IC_{50}$. The purified XOD inhibitor was a tripeptide with the amino acid sequence phenylalanine-cysteine-histidine and a molecular weight of 441.3 Da. The XOD inhibitor-containing ultrafiltrates from Pleurotus ostreatus demonstrated dose-dependent anti-gout effects in a Sprague-Dawley rat model of potassium oxonate-induced gout, as shown by decreased serum urated levels at doses of 500 and 1,000 mg/kg, although the effect was not as great as that achieved with the commercial anti-gout agent, allopurinol when administered at a dose of 50 mg/kg.
