  • After the outbreak of COVID-19 in China, the national health commission of the people's republic of China distributed guidelines for the diagnosis and treatment of COVID-19. Based on that, each region of China made guidelines for traditional Chinese medical treatment of COVID-19 applicable to clinical field. Under the hypothesis that each region's guideline contains regional characteristics, a comparison was made on pattern identification among each region's guidelines and central guidelines. At the beginning of the analysis of the cases, opinions on pattern identification vary from region to region, and the diversity is mainly reflected in the early stage of the disease. When the guideline is organized to a certain level due to the accumulation of clinical cases, there is a strong tendency to enumerate various types of pattern identification. It means that as a specific infectious disease progresses, it can appear in various cases due to variables. In some guidelines, disease stages were analyzed by only a limited pathological mechanism, but no regional characteristics were found here. Rather, it may mean that unique characteristics for disease can be derived.

  • Objectives : This study reviews whether the traditional medical thought process reflected in the Traditional Chinese Medical Treatment Plan for COVID-19 is based on existing classical texts, and examine concerns over the quality of evidence that the plan is based on. Methods : First, terminology and basic formulas composing the compound formulas in the COVID-19 TCM Treatment Plan were collected. Next, their usage in existing classical texts were searched in the medical classics database. Results : Infectious diseases similar to COVID-19 were understood as external disease due to Six Qi in the texts. Basic formulas used for treatment were those applied in Shanghan and Wenbing, among which cases where such formulas were applied in infectious diseases could be found in the classics. Conclusions : The level of evidence of the Treatment Plan suggested by various specialists could be evaluated as insufficient if we consider the literature. However, if application of such a plan could be supported institutionally, it could become a starting point for evidence generation.

  • Objective : The purpose of this study is to examine the treatment of allergic rhinitis introduced in the Chinese guideline, and to find out the direction of evidence-establishment and applicability in developing Korean Medicine clinical practice guideline for allergic rhinitis. Method : We studied Traditional Chinese Medicine treatment of allergic rhinitis introduced in the 2018 Chinese society of allergy guidelines for diagnosis and treatment of allergic rhinitis. The treatment are classified into three categories; herbal medicines, acupuncture and the others. And we compared this guidelines with other guidelines for how they differ in description of the database, evidence of level, and strength of recommendation. Results : Herbal medicines are presented based on syndrome differentiation. The basic acupoints for allergic rhinitis are introduced as follows; Fengchi(GB20), Yingxiang(LI20), Feishu(BL13) and Taiyuan(LU9). And in comparison with other guidelines for allergic rhinitis, the Chinese guideline showed lack of description in the database, evidence of level, and strength of recommendation, though they used evidence-based models. Conclusions : Clinical practice guideline projects are also underway in Korea for benefit expansion and improving quality of medical services. It is important to develop guidelines which should be evidence-based and reflect Korean medical environment.

Methods : We collected treatment guidelines published in different regions of China and categorized formulas for prevention and medical observation period listed in them according to the editions. The categorized preventive formulas were compared by type and target group. Results : Herbal medicines used for prevention included formulas derived from Korean medical classics, such as Okbyeongpungsan(玉屏風散) and Eunkyosan(銀翹散). The newly composed formulas, totaling over 100, were created by adding, subtracting, and combining formulas such as Sang-gukeum(桑菊飮), Eunkyosan(銀翹散), Sasammaekmundongtang(沙參麥門冬湯), Okbyeongpungsan(玉屏風散), Gwakhyangjeong-gisan(藿香正氣散), and Soshihotang(小柴胡湯). Patent medicines including Huoxiangzhengqi capsule(藿香正氣膠囊), Lianhuaqingwen capsule(連花淸瘟膠囊), Shufengjiedu capsule(疏風解毒膠囊), and Jinhuaqinggan granule(金花淸感顆粒) were frequently used, mainly targeting close contacts. These medicines were used differently depending on the specific population group, such as the general population, the elderly, children, pregnant women, and patients with underlying diseases, and were also applied differently according to the individual's constitution. Conclusion : We were able to identify various background factors contained in the guidelines for the use of preventive formulas presented by TCM group, and understand the social conditions that enabled the group to provide such guidelines. Through this, thorough preparation should be made so that the Korean Medicine can actively respond to another future pandemic.
