  • This study aims to provide an opportunity to enhance public health through suggesting development of Health Promotion program for industrialization of Health Promotion Field in Oriental Medicine. Industial Model for Health Promotion Field in Oriental Medicine has been Development with following characteristics. Allowed easy application of Oriental Medicine through everyday life methods. efforts were made to industrialize and generalize Health Promotion Field in Oriental Medicine by shifting emphasis to mass training and edacation from individual training.

An Analysis on Actual Condition of Health Promotion Program through Oriental Medicine in Health Center (한방건강증진HUB보건소사업 실태분석)

  • This study was carried out to examine the actual condition of health promotion program through oriental medicine in the health center and to provide basic data to develop proper policy of oriental medical health promotion program for the community people. The data were collected from 26 health centers which have been implementing the oriental medical health promotion program, using selfadministered questionnaire for two weeks from 1 October to 15 October 2006. The results are as follows : Generally, the respondents have the positive views on the level of budget and facilities/equipments of the oriental medical health promotion program in health center. However, they have the negative views on the level of manpower and education/training of the program. And also more than 70% of the respondents have the negative opinion on capabilities of formulating and evaluating the oriental medical health promotion program. The respondents indicated that there was the lack of coordination between the oriental medical health promotion program and existing health promotion in health center, and that low rate of utilizing community resources. With regard to the method of selecting the target group for the program, there are differences according to the each program. Many programs tended to select the target group not through the criteria of life-course and illness group but through the efficiency of selecting group. And many programs such as stroke prevention program, constitutional medicine program, oriental medical prenatal program, oriental medical prenatal and postnatal program, oriental medical child care program are mainly composed of the development of educational program and lecture. Regarding the number of the present oriental medical health promotion programs, around 65% of respondents answered that the number of the programs was many and thus they needed to decrease to the proper level. And with regard to the priority of the need, effectiveness and the satisfaction for each programs, on the whole, Qui gong program, stroke prevention program, area-specialty program and oriental medical home visiting program have high score. In particular, oriental medical quit-smoking program has lowest score. From these results, it requires to develop and improve the oriental medical health promotion program in health center considering the need and characteristics of community.

A Study on Urinary Incontinence Clinic Operated by Local Public Health Center (일개 보건소의 요실금 치료 교실 운영 결과에 대한 고찰)

  • Purpose: Today, the proportion of the elderly is increasing especially in a province. For elder people, local government has conducted the health promotion act by oriental medicine. For oriental medicine hub health promotion act, Eumseong -gun carried out a program that is urinary incontinence clinic for the elderly. The purpose of this study is to show how effective the public health promotion program using oriental medicine is. Methods: The 42 patients had been treated for urinary incontinence during the 10 or 11 weeks. They were put on oriental medical treatment in the Eumseong-gun public health center. Results: 1. There was significant decreases in the degree of urinary incontinence after the treatment. 2. The treatment made the patients regain their mental security. 3. People who were treated by public health promotion program were the most satisfied. Conclusion: The results suggested that public health promotion program using oriental medicine is effective for urinary incontinence.

A Survey on Recognition about Oriental Childcare and Oriental Medicine in Parents Attending in Oriental Childcare Class in Health Center (보건소 한방육아교실 참여 부모들의 육아 및 한방에 대한 인식 조사)

  • Objectives: The purpose of this survey is to investigate the recognition of parents' attending oriental childcare class, as a part of an oriental health promotion programs in a health center. Methods: During 2010, 74 parents participated in the pre-survey and 61 parents participated in the post-survey at the Kimpo, Kyeongju and Ganghwa health center. Results: 1. 77% of the participants didn't recognize the existence of Oriental childcare center. 49% of participants were informed by health center. 2. In terms of preferences on oriental versus western medicine, 76% of participants preferred western medicine as compared to the oriental medicine but, some wanted to use both medical treatments 3. Recognition levels in importance of childcare education on post-survey were significantly elevated to the pre-survey(p<0.05), and the childcare education helped parents to breed children better. Participants want to recommend neighbors to participate in childcare class. 4. Levels of common senses about childcare on post-survey were significantly elevated compared to pre-survey(p<0.05), but levels of common senses about oriental childcare were not elevated. 5. 77% of the participants satisfied with the childcare class. Conclusions: The oriental childcare class sessions in health center have helped parents to recognize the importance of childcare education and common senses about childcare.

A Need Assessment on Establishment of Oriental Health Promotion Center (한방건강증진센터 설립에 대한 인식 및 요구조사)

  • The study attempts to examine the degree of cognition and demand on health promotion center of oriental nursing. It puts unique nursing intervention using traditional health promotion connected with oriental medicine to practical use for residents' health promotion and prevention of disease. With the study design of cross-sectional descriptive study, 516 residents who live in 26 Dongs, Dongdaemoon-gu were selected. The tool of study consists of 30 questions which the study team made for the degree of cognition and demand on health promotion center of oriental nursing. Cronbach's $\alpha$ in the degree of cognition was .8028. Collection of research data had been done from October 1 to October 30, 1999 with help of Dongdaemoon-gu office after pretest from 20 residents. Collected data were analyzed into the number and percentage in the characteristic of a subject and connected with demand on the establishment of center, the mean and the standard deviation in the degree of cognition and F-tests in the difference of the degree of cognition by characteristics. The results were as follow; 1) The characteristic of subject of this study was male 50.6%, and average age was 38.5 years old and 30-39 years old occupied the highest percentage with 31.6%. The married were 71.8%, over high school graduates was 85.6%, monthly income from 500 thousand won to 2 million won was 86.1%. 50.8% was the type living with parents, children and sibling. 2) When they were sick, the institution which residents used at first was a pharmacy(69.2%) and hospital(27.5%), but oriental medicine hospital was just 1.4%. As for subjective health condition, 82.5% answered over average, and 28.7% answered that they had chronic illness such as arthritis, chronic digestion problem, hypertension and so on. As for information collection on health, mass communication(34.9%) and medically concerned people(28.1%) occupied relatively high rate. Free health diagnosis system(36.8%) and establishment of health promotion center(31.5%) among welfare programs that residents want to enjoy were high ranked. The rate using a special institution for health was 17.8%, and among these institutions, the rate using aerobic exercises, health center(7.0%) and steamed room(5.4%) was high. Besides, other institutions such as breathing at the abdomen, finger-pressure therapy, meridian massage, foot massage, and so on were being used. 3) As the average of the degree of cognition on health promotion center of oriental medicine was 2.92, the degree of cognition was medium. The description, "health promotion center of oriental medicine is necessary for health keeping of healthy people, including people who have a problem in health" showed the highest degree of cognition(3.04, ${\pm}0.64$). 4) As for the intention on using health promotion center of oriental nursing, 61.4% said "yes", "no" was just 1.4%. The services that people relatively high wanted to be served from the center were measures reducing stress(68.0%) (relaxation therapy, meditation, breathing at the abdomen and so on), acupuncture(66.5%), finger-pressure(61.6%), moxibustion(57.6%), meridian massage(44.2%), postpartum care(40.3%) and so on. 5) As for the degree of cognition on the establishment of health promotion center of oriental nursing by characteristics of subject, there was significant difference(F=4.03, p=.046) between male(3.01) and female(2.91). But there was no significant difference by age, marital status, level of educational achievement and monthly income. As the above result, cognition on the establishment of health promotion center of oriental nursing was relatively low because people were not familiar with about the health promotion center of oriental nursing yet. However once the center will be established, the degree of demand on the center will be relatively high. So positive advertisement will be necessary, and the management of useful programs will be also required in order to make people recognize the advantage when they actually will use the center. On the other hand, as the subject of the study consists of many young people of below 30, the health problem came to be low. And in the case of sampling, the study using random sampling that can represent population will be required.

A Study on the Clinical Statistics of an Oriental Medical Office in a Public Health Center (대도시 보건소 한방 진료실 이용실태)

  • Objectives : This study investigated the actual condition of an oriental medical office in a public health center. Methods : We classified patients who visited an oriental medical office in the public health center of Gyeyang-gu over a 1-year period, according to age, sex, disease, etc. The number of patients was 6529. Results : Analyzing these 6529 persons, 65.8% were female, and 79.6% were seniors aged over 65. By disease category motor systemic disease or circulatory disease affected 84% of the patients. By medication or treatment category. O-Juk-San (五積散) at 38.3% was the greatest proportion, and Pal-Mul-Tang (八物湯) the second greatest. As to medical fee, 80.2% of the patients were not charged for treatment. Conclusions : The extract of herbal medication should be more expanded in the medical insurance, and should be improved in quality. The oriental medical office in a public health center should focus more on health promotion and disease prevention than patient care. For accomplishing this goal, the institutional position and rights of oriental medical doctors as civil officials should be equal to those of western medical doctors.

  • Objectives : The study investigated the actual condition of the class for Qi-gong gymnastics among health promotion service programs using oriental medicines, which the public health center has been conducting for local residents. Method : Of a total of 964 oriental medical physician in public health center across the country, it examined 27 places consisting of public health centers and public health sub-centers, and public hospitals, all of which were running the class for Qi-gong gymnastics. Then, it carried out the tele research of those oriental medical physicians in charge of the class there. Result : As a result, area that is enforcing the Class for Qi-gong Gymnastics was expose each attempt that difference is particularly, and appeared by thing which form and practice law that operate the classroom are various. Also, in many cases, the oriental medical physicians instructed the gymnastics directly instead of inviting an external lecturer. Conclusions : In conclusion, based on the contents surveyed, by making the well-organized system of the class for Qi-gong excercise; by developing the common excercise and texts; and by publicizing and educating the oriental medical physicians engaged in public health, that the Class for Qi-gong Gymnastics is real in health promotion of local residents.

A Survey on the Medical Conditions of Public Health Oriental Medical Doctors (공중보건한의사의 진료여건에 관한 조사연구)

  • Public health oriental medical doctor has played a great role in providing oriental medical treatment and oriental medical health program with public health medical services, the basic infrastructure, however, is not sufficient. In this study, the researcher surveyed the treatment working or service condition of public health oriental medical doctor. 1. The payment, allowance of doctors are fixed upon the law and guideline according to the financial status of local government. The branch of public health center has more support like an official residence with expenses. 2. The public health center mainly has assitants and ratio of full-timer is more than the branch public health center without any assistants if any, they are temporary employee 3. The public health center has 5.22 beds while the branch has 3.14 beds. The daily average number of patient for public health center is 15.01 while the branch has 8.7 More than half of outpatients are over sixties with musculoskeletal disease. 4. Regarding the traveling clinic, the public health center put into more operation than the branch. The 3rd year serving public doctor gives negative about the traveling clinic much more than 1st and 2nd year serving public doctor. The treatment service condition of public doctor of the public health center is better than the branch doctor, but we are on the point of role-changing as health promotion and preventive treatment to bring up motivation, education and competence strengthening for execution the local-bound health program.

Utilization and Improvement of 8 Principle TKM Public Health Services in Rural Areas of Korea (일부 한국농촌지역의 8대 한방공공보건사업에 대한 주민의 이용실태 및 개선방향;강원도 홍천군을 중심으로)

  • Health promotion program utilizing traditional korean medicine(TKM) is very unique modality uncommonly conducted throughout the world. Korea's TKM public health services went through initial stage from 2001 and spread throughout the country by 2003. 35 public health centers in the nation is appointed as 'TKM health promotion HUB' and in operation from 2007. But the program is still in the early state and evaluation of usage, satisfaction, and community service is still in demand. This study aims to verify current situations of TKM public health service by examining public awareness, usage, and satisfaction and suggest improvements based on findings. 2.5% of local residents were sampled and 1739 were chosen as subjects. Knowledge, attitude, satisfaction and others for TKM public health service were examined. TKM treatment service was most widely utilized(13.2%), followed by CVA prevention program(5.2%). For satisfaction level, CVA prevention program showed highest satisfaction(73.3%), closely followed by (Qi-gong program 63%). In general, awareness and utilization of TKM public health service were low but compensated by high satisfaction level. More active promotion and development of community specific programs should become available in the future.

A Study on the Status and Development Plan of the Korean Medical Public Health Program (한의약 건강증진 사업의 현황과 발전방안 연구)

  • Objectives: We conducted research on 100 cases of Korean medical health promotion programs to determine how they work. The objective was to report trends in such programs and, by extension, to suggest a development plan for the Korean medical public health program. Methods: To research 100 Korean medical health promotion programs, we analyzed source data published by the Korea Health Promotion Institute in 2014-2018 using 10 criteria. Results & conclusions: Based on effective trends, the development of a Korean medical public health program requires the following options. First, we should complement the systems with related legislation and deal with Korean medical doctors who mainly participate in public health programs. Second, we should improve the infrastructure of the program with internal support from the Korean medical association and clarifying the legal basis of the national budget. Third, we should seek substantiality of public health programs. Korean medical doctors should actively participate in the program, and related agencies should support the public health care center for the development of Korean medical public health programs in which many people can participate.
