This paper describes a methodology for the development of models of discrete event system(DES). The methodology is based on transformation of continuous state space into discrete one to homomorphically represent dynamics of continuous processes in discrete events. This paper proposes a formal structure which can couple DES models within a framework. The structure employs the DEVS formalism for the DES models. The proposed formal structure has been applied to develop a DEVS model for the human cardiovascular system. For this, the cardiac cycle is partitioned into a set of phases based on events identified through VisSim simulation in the CS of the electrical analog model. VisSim is the simulation tool of visual environment for developing continuous, discrete, and hybrid system models and performing dynamic simulation. For each phase, a CS of the electrical analog model for the cardiovascular system has been simulated by VisSim 2.0. To validate this model, first develop the DEVS model, then simulate the model in the DEVSIM++ environment. It has same simulation results for the data obtained from the CS simulation using VisSim. The comparison shows that the DEVS model represents dynamics of the human heart system at each phase of cardiac cycle.
Physiological agents trigger a signaling process called "inside-out signaling" and activated platelets promote adhesion, granule release, and conformational changes of glycoprotein IIb/IIIa (αIIb/β3). Activated αIIb/β3 interacts with fibrinogen and initiates a second signaling step called "external signaling". These two signaling pathways can cause hemostasis or thrombosis, and thrombosis is a possible medical problem in arterial and venous vessels, and platelet-mediated thrombosis is a major cause of cardiovascular disease (CVD). Therefore, modulating platelet activity is important for platelet-mediated thrombosis and cardiovascular disease. Esculetin is a coumarin-based physiologically active 6,7-dihydroxy derivative known to have pharmacological activity against obesity, diabetes, renal failure and CVD. Although some studies have confirmed the effects of esculetin in human platelet activation and experimental mouse models, it is not clear how esculetin has antiplatelet and antithrombotic effects. We confirmed the effect and mechanism of action of escultein on human platelets induced by collagen. As a result, esculetin decreased Ca2+ recruitment through upregulation of inositol 1, 4, 5-triphosphate receptor. In addition, esculetin upregulates cyclic adenosine monophosphate (cAMP) and cyclic guanosine monophosphate (cGMP)-dependent pathways and inhibits fibrinogen binding and thrombus contraction. Our results demonstrate the antiplatelet effect and antithrombotic effect of esculetin in human platelets. Therefore, we suggest that esculetin could be a potential phytochemical for the prevention of thrombus-mediated CVD.
Oxygen-derived free radicals have been implicated in many important functions in the biological system. Electrical field stimulation (EFS) causes arterial relaxation in animal models. We found that EFS applied to neither muscle nor nerve but to Krebs solution caused a relaxation of rat aorta that had been contracted with phenylephrine. In the present study, therefore, we investigated the characteristics of this EIRF (electrolysis-induced relaxing factor) using rat isolated aorta. Results indicated that EIRF acts irrespective of the presence of endothelium. EIRF shows positive Griess reaction and is diffusible and quite stable. EIRF-induced relaxation was stronger on PE-contracted aorta than on KCl-contracted one, and inhibited by the pretreatment with methylene blue. Zaprinast, a cGMP-specific phosphodiesterase inhibitor, potentiated the EIRF-induced relaxation. $N^G-nitro-L-arginine$, NO synthase inhibitor, did not inhibit the EIRF-induced relaxation. Deferroxamine, but not ascorbic acid, DMSO potentiated the EIRF-induced relaxation. These results indicate that electrolysis of Krebs solution produces a factor that relaxes vascular smooth muscle via cGMP-mediated mechanism.

Inflammatory and fibrotic responses are accelerated during the reperfusion period, and excessive fibrosis and inflammation contribute to cardiac malfunction. NecroX compounds have been shown to protect the liver and heart from ischemia-reperfusion injury. The aim of this study was to further define the role and mechanism of action of NecroX-5 in regulating inflammation and fibrosis responses in a model of hypoxia/reoxygenation (HR). We utilized HR-treated rat hearts and lipopolysaccharide (LPS)-treated H9C2 culture cells in the presence or absence of NecroX-5 ($10{\mu}mol/L$) treatment as experimental models. Addition of NecroX-5 significantly increased decorin (Dcn) expression levels in HR-treated hearts. In contrast, expression of transforming growth factor beta 1 ($TGF{\beta}1$) and Smad2 phosphorylation (pSmad2) was strongly attenuated in NecroX-5-treated hearts. In addition, significantly increased production of tumor necrosis factor alpha ($TNF{\alpha}$), $TGF{\beta}1$, and pSmad2, and markedly decreased Dcn expression levels, were observed in LPS-stimulated H9C2 cells. Interestingly, NecroX-5 supplementation effectively attenuated the increased expression levels of $TNF{\alpha}$, $TGF{\beta}1$, and pSmad2, as well as the decreased expression of Dcn. Thus, our data demonstrate potential antiinflammatory and anti-fibrotic effects of NecroX-5 against cardiac HR injuries via modulation of the $TNF{\alpha}/Dcn/TGF{\beta}1/Smad2$ pathway.

Background: This study aimed to develop a new risk prediction model for operative mortality in a Korean cohort undergoing heart valve surgery using the Korea Heart Valve Surgery Registry (KHVSR) database. Methods: We analyzed data from 4,742 patients registered in the KHVSR who underwent heart valve surgery at 9 institutions between 2017 and 2018. A risk prediction model was developed for operative mortality, defined as death within 30 days after surgery or during the same hospitalization. A statistical model was generated with a scoring system by multiple logistic regression analyses. The performance of the model was evaluated by its discrimination and calibration abilities. Results: Operative mortality occurred in 142 patients. The final regression models identified 13 risk variables. The risk prediction model showed good discrimination, with a c-statistic of 0.805 and calibration with Hosmer-Lemeshow goodness-of-fit p-value of 0.630. The risk scores ranged from -1 to 15, and were associated with an increase in predicted mortality. The predicted mortality across the risk scores ranged from 0.3% to 80.6%. Conclusion: This risk prediction model using a scoring system specific to heart valve surgery was developed from the KHVSR database. The risk prediction model showed that operative mortality could be predicted well in a Korean cohort.
Objectives : This study was performed to evaluate the relationship between C-reactive protein(CRP) and carotid intima-media thickness(carotid IMT) in a population of middle-aged Koreans. Methods : A total of 1,054 men and 1,595 women(aged 40-70 years) from Kanghwa County, Korea, were chosen for the present study between 2006 and 2007. We measured high-sensitivity CRP and other major cardiovascular risk factors including anthropometrics, blood pressure, blood chemistry, and carotid ultrasonography. Health related questionnaires were also completed by each study participant. Carotid IMT value was determined by the maximal IMT at each common carotid artery. The relationship between CRP level and carotid IMT was assessed using multiple linear and logistic regression models after adjustment for age, body mass index, menopause(women), systolic blood pressure, total/HDL cholesterol ratio, triglyceride level, fasting glucose, smoking, and alcohol consumption. Results : Mean carotid IMT values from the lowest to highest quartile of CRP were 0.828, 0.873, 0.898, and 0.926 mm for women(p for trend<0.001), and 0.929, 0.938, 0.949, and 0.979 mm for men(p for trend=0.032), respectively. After adjustment for major cardiovascular risk factors, the relationship between CRP and carotid IMT was significant in women(p for trend=0.017), but not in men(p for trend=0.798). Similarly, adjusted odds ratio of increased IMT, defined as the sex-specific top quartile, for the highest versus lowest CRP quartiles was 1.55(95% CI=1.06-2.26) in women, but only 1.05(95% CI=0.69-1.62) in men. Conclusions : CRP and carotid IMT levels appear to be directly related in women, but not in men.
In this paper, we consider the aortic sinus baroreceptor, which is the most representative baroreceptors sensing the variance of pressure in the cardiovascular system(CVS), and propose heart activity control model to observe the effect of delay time in heart period and stroke volume under the regulation of baroreflex in arotic sinus. The proposed heart activity baroreflex regulation model contains CVS electric circuit sub-model, baroreflex regulation sub-model and time delay sub-model. In these models, applied electric circuit sub-model is researched by B.C.Choi and the baroreflex regulation sub-model transforms the input, the arotic pressure of CVS electric circuit sub-model, to outputs, heart period and stroke volume by mathematical nonlinear feedback. We constituted the time delay sub-model to observe sensitivity of heart activity baroreflex regulation model by using the variable value to represent the control signal transmission time from the output of baroreflex regulation model to efferent nerve through central nervous system. The simulation object of this model is to observe variability of the CVS by variable value in time delay sub-model. As simulation results, we observe three patterns of CVS variability by the time delay. First, if the time delay is over 2.5 sec, arotic pressure, stroke volume and heart rate is observed nonperiodically and irregularly. Second, if the time delay is from between 0.1 sec and 0.25 sec, the regular oscillation is observed. Finally, if time delay is under 0.1 sec, then heart rate and arotic pressure-heart rate trajectory is maintained in stable state.

Objectives: C-reactive protein (CRP), an inflammatory biomarker, has been widely used as a preclinical marker predictive of morbidity and mortality. Although many studies have reported a positive association between CRP and mortality, uncertainty still remains about this association in various populations, especially in rural Korea. Methods: A total of 23 233 middle-aged participants (8862 men and 14 371 women) who were free from cardiovascular disease, cancer, and acute inflammation (defined by a CRP level ${\geq}10mg/L$) were drawn from 11 rural communities in Korea between 2005 and 2011. Blood CRP concentration was analyzed as a categorical variable (low: 0.0-0.9 mg/L; intermediate: 1.0-3.0 mg/L; high: 3.1-9.9 mg/L) as well as a continuous variable. Each participant's vital status through December 2013 was confirmed by death statistics from the National Statistical Office. Cox proportional hazard models were used to assess the independent association between CRP and mortality after adjusting for other risk factors. Results: The total quantity of observed person-years was 57 975 for men and 95 146 for women, and the number of deaths was 649 among men and 367 among women. Compared to the low-CRP group, the adjusted hazard ratio for all-cause mortality of the intermediate group was 1.17 (95% confidence interval [CI], 0.98 to 1.40) for men and 1.27 (95% CI, 1.01 to 1.61) for women, and the corresponding values for the high-CRP group were 1.98 (95% CI, 1.61 to 2.42) for men and 1.41 (95% CI, 1.03 to 1.95) for women. Similar trends were found for CRP evaluated as a continuous variable and for cardiovascular mortality. Conclusions: Higher CRP concentrations were associated with higher mortality in a rural Korean population, and this association was more prominent in men than in women.
배경: 흉부 및 흉복부 대동맥의 수술 중 대동맥 차단은 허혈성 척수 손상에 의한 하반신 마비와 같은 심각한 합병증을 유발할 수도 있어 수술 중 허혈성 척수손상을 예방하기 위한 여러 방법의 연구가 계속 되고 있다. 최근에 허혈성 대뇌 손상 모델에서 신경조직의 막전위 의존성 나트륨채널 길항제가 대뇌 보호 효과가 있다는 보고가 있다. 본 연구는 토끼의 허혈성 척수손상 모델에서 나트륨채널 길항제인 페니토인과 저체온의 척수보호효과를 비교해 보고자 시행되었다. 대상 및 방법: 뉴질랜드산 토끼의 신동맥직하부에서 복부대동맥을 25분간 차단하는 방식으로 척수허혈을 유도하였으며 각 군당 8마리씩 네 군으로 나누었다. 대조군과(S39) 저체온군은(S37) 대동맥 차단시간 동안 직장온도를 각기 $39^{\circ}C$와 $37^{\circ}C$로 일정하게 유지하면서 $22^{\circ}C$ 생리적 식염수만 2 mL/min의 속도로 연속 주입하였으며, 정상체온 및 저체온 페니토인 군은(P39, P37) 앞의 두 군과 동일한 방법으로 하되 생리적 식염수에 페니토인을 녹여 주입하였다(100 mg/50 mL). 수술 후 24시간 및 72시간이 경과한 다음 Tarlov scoring을 통해 신경학적 평가를 시행하였고 마지막 평가 후에는 객관적으로 신경손상의 정도를 정량화하기 위해 척수를 고정 처리하였다. 결과: 페니토인의 역행성 주입에 따른 심각한 문제는 없었으며 대조군에(S39) 속한 모든 동물은 완전 또는 심한 하반신 마비 소견을 보였다. 페니토인과(P39) 저체온(S37)군 모두 대조군에 비해 신경학적 평가는 유사한 정도로 우수한 결과를 보였다(p<0.05). 조직 병리학적 검사 결과, 대조군에 속한 모든 동물은 척수 회백질에서 심한 신경조직 괴사 때 보이는 전형적인 특징을 보여주었으며, TUNEL 염색에 양성인 신경세포도 높은 빈도로 관찰되었으나, 저체온 또는 페니토인 투여 군에서는 괴사현상이 유의한 정도로 감소하였으며, 상대적으로 매우 낮은 빈도의 TUNL 염색 양성세포가 관찰되었다(p<0.05). 그러나 저체온과 페니토인을 병용했을 때의 부가적인 척수보호효과를 조사해 본 결과 신경학적 평가와 조직병리학적 결과 모두 유의한 수준의 부가적인 효과는 없었다. 걸론: 결론적으로, 토끼의 허혈성 척수 손상 모델을 이용하여 페니토인과 저체온의 신경보호효과를 알아본 결과 신경학적 평가와 조직병리학적 검사 결과 모두 부가적인 효과는 보여주지 못했지만 각각의 경우 유사한 정도의 신경보호효과를 보여주었다.
배경: 혈관 내피층에서 분비되어 평활근층 이완을 일으키는 물질의 본체는 산화질소(nitric oxide, NO)이며 NO synthase(NOS)에는 뇌형(brain NOS, bNOS), 내피세포형(endothelial constitutive NOS, ecNOS) 및 유도형 (inducible NOS, iNOS) 등 세 가지 동위효소가 있음이 알려져 있다. 고혈압은 혈관 내피층 기능 이상을 보임이 알려져 있으나 NOS 동위 효소의 변화를 포함한 세포내 기전은 아직 확실치 않다. 저자들은 고혈압 기전을 구명하기 위한 일환으로 고혈압 혈관에서 NOS 동위효소가 어떻게 변화되는가 조사하고자 하였다. 대상 및 방법: 흰쥐에서 two-kidney, one clip (2K1C) 고혈압과 deoxycorticosterone acetate(DOCA)-salt 고혈압을 일으켰다. 4주 뒤 고혈압이 일어난 것을 확인하고 적출 흉부 대동맥 표본에서 Western blot 분석에 의한 NOS 동위효소 발현 조사 및 비색법에 의한 조직내 산화질소 정량을 하였다. 결과: 2K1C 및 DOCA-salt 흰쥐에서 실험군은 각각의 대조군에 비해 유의하게 높은 혈압을 보였다. 두 고혈압군에서 모두 적출 대동맥 표본의 bNOS 및 ecNOS 단백 발현이 감소되었다. iNOS 단백은 DOCA-salt 고혈압에서 변화를 보이지 않으나 2K1C 고혈압에서는 역시 감소를 보였다. 혈관조직내 산화질소 함량은 두 고혈압에서 모두 유의하게 감소되었다. 결론: 2K1C 및 DOCA-salt 고혈압에서 혈관의 NOS 발현과 산화질소 함량이 감소되어 있으며 이는 고혈압의 유지 기전에 공헌하리라 추측되었다.

② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
