Effects of Cheongyoung-tang on CHT-Induced Brain Edema and Neurological Disturbance Symptom in Rats (청영탕이 CHT로 유발된 백서의 뇌부종 및 신경장애 증상에 미치는 영향)

  • This study was carried out to evaluate Effects of Cheongyoung-tang on CHT-Induced Brain Edema and Neurological Disturbance Symptom in Rats. we make a comparative study of the such parameters as neurological severity score(NSS) and changes in water content, lactate, glucose and hematological substance. 1. ΔNSS increased in CYT1 and CYT2 as compared with the control group, which showed no efficacy. 2. In water content of both hemispheres, CYT1 and CYT2 as compared with the control group have no difference. 3. Water content of the left hemisphere decreased in CYT1 and CYT2 as compared with the control group. 4. The lactate level in serum decreased in CYT1 and CYT2 as compared with the control group. 5. In hematological changes, RBC, PLT and MCHC increased in CYT1 and CYT2 as compared with the control group, but MCV decreased. According to these results, CYT has an effect on CHT-induced brain edema and neurological symptom.

A Case of Multiple System Atrophy-Parkinsonism Type (MSA-P) with Gait Disturbance and Posture Instability Treated with Korean Medicine (보행 장애와 자세 이상 등을 호소하는 다계통 위축증(MSA-P) 한의 치험 1례)

  • This case report suggests the potential effects of Korean medicine for the gait disturbance and posture instability of Multiple System Atrophy-Parkinsonism (MSA-P). A patient with MSA-P who had gait disturbance and posture instability was given a series of Korean medicine, including acupuncture, pharmacopuncture, electro-acupuncture, moxibustion, and herbal medicine, for 15 days of hospitalization. During the hospitalization period, the severity and progression of the disease was measured with the Unified Multiple System Atrophy Rating Scale (UMSARS) and a video recording of a 20-meter round timed walking test. The UMSARS Part I and II scores decreased from 14 to 5 and 9 to 5, respectively. An improvement was also noted in standing and walking posture, and the walking time was shortened from 30 to 25 seconds in the 20-meter round-trip walking test. Dysarthria, dysphagia, and non-motoring symptoms like sleep behavioral disorders and constipation were also improved. The findings of this case study suggest that this series of Korean medicine has potential therapeutic effects for patients with MSA-P with motor and non-motor symptoms.

Effect of Systematic Educational Program for the Application of National Institutes of Health Stroke Scale (NIHSS) as a Neurologic Assessment Tool in Stroke Patients (뇌졸중의 신경학적 사정 도구인 NIHSS 적용을 위한 체계적인 간호사 교육 프로그램의 효과)

  • Purpose: In assessing patients' neurological status following a stroke it is very important to have a valid tool for early detection of neurological deterioration. NIHSS is considered the best tool to reflect neurological status in patients with ischemic stroke. An education program on use of NIHSS was planned for nurses caring for these patients and the effects of the program were evaluated. Methods: The NIHSS education program (NEP) which includes online and video lectures, and practical education was provided to the nurses from April to July, 2010. To examine the effect of NEP, nursing records of patients with ischemic stroke who were admitted to a stroke center were analyzed. Two groups, a historical control group (n=100) and the study group (n=115) were included. Results: Nursing records for neurologic symptoms for each patient increased (41.0% versus 100.0%, p<.001), and especially, visual disturbance, facial palsy. limb paralysis and ataxia, language disturbance, dysarthria, and neglect symptoms significantly increased (all p<.001). Nurse notification to the doctor of patients with neurological changes increased (21.0% versus 39.1%, p=.004), and nurses' neurological deterioration detection rates also increased (37.5% versus 84.6%, p=.009). Conclusion: NEP improved the quality of nursing records for neurological assessment and the detection rate of neurological deterioration.

Persistent Accelerated Idioventricular Rhythm in a Shih Tzu with Central Neuropathy (중추신경 장애를 가진 시추 종의 개에서 지속적인 가속성 심실고유 율동)

  • A 7-year-old neutered male Shih Tzu dog weighing 5.2 kg was presented because of severe neurological signs (paddling, unconsciousness, blindness, seizure). ECG revealed accelerated idioventricular rhythm (AIVR) with ~140 bpm ventricular rate. Based on magnetic resonance imaging (MRI) study and cerebrospinal fluid (CSF) analysis, the neurological condition was tentatively diagnosed as disseminated granulomatous meningoencephalomyelitis (GME). The neurological signs were managed with steroids, anticonvulsive drugs, diuretics and antibiotics with 20% mannitol infusion. The rhythm disturbance (AIVR) was managed with oral medication of mexiletine. Because the heart rhythms were unstable without anti-arrhythmic therapy in spite of improvement of clinical signs after emergency treatment for neurological problems, the anti-arrhythmic therapy was maintained till the neurological signs were abolished.

Application of Gait Analysis to the Patients with Cervical Myelopathy (경척수증 환자에 대한 보행분석의 적용)

  • Objective : To investigate objectively the postoperative improvement of gait disturbance in patients with cervical myelopathy through a gait analysis. Patients and Methods : Ten patients who underwent cervical decompression and fusion for cervical myelopathy caused by spondylosis, OPLL, or concomitant hypertrophy of ligamentum flavum were studied. Preoperatively, gait disturbance was present in all patients. The patients were evaluated by gait analysis using three dimensional motion analyzer to collect data of linear and kinematic parameters before surgery, 1 week and 3 months after surgery. Statistical analysis of the related pre-and post-operative data were performed. Results : In the linear parameters, average value of cadence, walking speed, stride length, step time, width and double support were increased postoperatively compare to preoperative value. In the kinematic parameters, average value of knee flexion during initial swing phase, plantar flexion of ankle and range of motion of hip joint were increased as well. These differences were statistically significant(p<0.05). Conclusion : This study suggests that gait analysis can be used as a method of quantitative analysis of postoperative gait improvement in patients with cervical myelopathy.

Two Cases of Korean Medicine Treatment for Patients with Parkinson's Disease Evaluated Using a Three-Dimensional Gait Analysis System (3차원 보행분석기로 평가한 보행장애 및 자세불안정을 주소로 하는 파킨슨병 환자 한의 치험 2례)

  • Objective: This study examined the effectiveness of Korean medicine treatments in two patients with Parkinson's disease complaining of discomfort stemming from postural instability and gait disturbance (PIGD). Methods: Two patients were treated for 3 months. They visited the clinic once a week for the first month and thereafter once every 2 weeks. The Unified Parkinson's Disease Rating Scale (UPDRS) and a three-dimensional gait analysis were performed at the first visit and at 1, 2, and 3 months thereafter. Results: In Case 1, gait speed, stride length, and swing phase increased. Double support decreased until 2 months after treatment but increased slightly after 3 months. Among the kinematic parameters, tilt and rotation increased. The total UPDRS Part III score decreased from 51 points to 29 points after 3 months of treatment. In Case 2, gait speed, stride length, and swing phase increased, but double support decreased. Among the kinematic parameters, tilt, rotation, and obliquity decreased. The total UPDRS Part III score decreased from 11 points to 7 points after 3 months of treatment. Conclusions: This study suggests that Korean medicine can be an effective treatment for patients with Parkinson's disease who experience discomfort due to PIGD.

Clinical Studies on 5 Cases of Multiple Sclerosis by Acupuncture Therapies (다발성 경화증 환자 5례에 대한 임상보고)

  • Background : Multiple Sclerosis is characterized by chronic inflammation, demyelinization and neurogliosis(plaque) in optic nerve, brain and spinal cord, which is recurrent visual disturbance, sensory disturbance, motor disturbance and sphincter disturbance, etc. Objective : This study was performed to evaluate the treatment of acupuncture therapy including Herbal Acupuncture and Bee venom Herbal Acupuncture in Multiple Sclerosis. Methods : We treated 5 cases of Multiple Sclerosis patients with mainly Bee venom Herbal Acupuncture therapy, and herbal Acupuncture, sa-am acupuncture form 18th september, 2001 to december, 2002. Results : 1 Of 5 cases who were treated with above, 3 patients were mildly reduced symptoms and 2 patients were no changes. 2. There are two patients who were definitely appeared the recurrent symptoms on treatment period, but decreased the remission period than before. 3. It has not shown noticeable recurrent symptoms of 3 patients. Conclusions : Bee Venom Herbal Acupuncture therapy and other acupuncture therapies were effective in reducing the recurrent symptoms and remission perod, but it was not evaluated the chief complaints on 5 cases of Multiple Sclerosis. We think that it need the further study and clinical trial for Multiple Sclerosis and other neurological diseases.

Visual Disturbance Caused by a Nail Gun-Induced Penetrating Brain Injury

  • Penetrating brain injury caused by a nail gun is an uncommon clinical scenario reported in the literature. A 36-year-old male presented with a nail that had penetrated through the occipital bone. He was alert and neurologically intact except for visual disturbance. Computed tomography (CT) of the brain showed the nail lodged at the occipital lobe and the parietal lobe, with minimal intracerebral hemorrhage. The nail was placed in the occipital lobe close to the superior sagittal sinus. We removed the nail with craniotomy since the entrance of the nail was close to the superior sagittal sinus. There were no newly developed neurological deficits postoperatively. Immediate postoperative CT showed no newly developed lesions. The patient recovered well without any significant complications. Two weeks postoperatively, magnetic resonance imaging showed no remarkable lesions. The visual disturbance was followed up at the outpatient department. To summarize, we report a rare case of penetrating head injury by a nail gun and discuss relevant aspects of the clinical management.

Clinical Analysis about Complications of Stroke Treated Intensively in the Intensive Care Unit (뇌졸중으로 중환자실에서 집중치료한 환자들의 합병증에 관한 임상적 고찰)

  • Purpose : Cerebrovascular disease is the most frequent cause of death in Korea and it remains severe disabilities disturbing normal life, According to the previous studies, mortality of the stroke in the first one week is up to the 20% and 95% of stroke patients in the acute stage are accompanied by more than one complications. These complications affect not only the acute stage mortality but also the late stage rehabilitations. In Korea the oriental medicine is preformed in the treatment of stroke. Therefore it is important to recognize thoroughly the complications in the acute stage of stroke and to prevent them. But studies about complications of acute stage stroke is rarely presented in the fields of oriental medicine. So this study is prepared for investigating the characteristics and frequency of complications in the acute stage of stroke. And we are to assess the importance of theses acute complications by systemic reviewing the previous studies. Methods : Fifty one patients are included who had been admitted to ICU(intensive care unit) of Chenju Hospital of Wonkwang Oriental Medicine. Twenty nine patients are diagnosed as ischemic stroke and twenty one patients are diagnosed hemorrhagic stroke. Medical and neurological complications were investigated retrospectively based on medical notes excluding primary symptoms of stroke Le. motor weakness, sensory disturbance and speech disorder. And risk factors of stroke such as D.M. or hypertension are excluded. Results : Medical complications are more frequent than neurological ones. Most frequent medical complication is dysuria(61%) and constipation(45%), fever(30%) and aspiration pneumonia(22%) are followed in order of frequency. In Neurological complication dysphagia(56%), the exacerbation of infarction due to increased intracranial pressure(24%), irritability or insomnia(21%) is most frequently complicated in order of frequency. Conclusions : These complications are mostly caused by bed rest state in acute stage stroke. It is supposed that more aggressive management can prevent theses ones. And it is possible to improve the medical and neurological conditions by sticking theses study results.

A Case Report of Persistent Drug-Induced Parkinsonism After Drug Discontinuation (약물 중단에도 지속되는 약인성 파킨슨증후군 환자의 한의 치험 1례)

  • This study examined the effectiveness of Korean medical treatment in a patient with persistent drug-induced parkinsonism after drug discontinuation. The changes in symptoms were assessed using the unified parkinson's disease rating scale (UPDRS), postural instability-gait disturbance (PIGD) score, and the 20 m gait time and steps. After 22 days of hospitalization, the UPDRS, PIGD score, and 20 m gait time and steps showed clinically significant improvement. The improvement persisted after discharge. This study indicated that Korean medical treatment could be an effective alternative therapy for treating persistent drug-induced parkinsonism after drug discontinuation.
