Comparative Study of Acute Dyspepsia, Functional Dyspepsia, Organic Dyspepsia by HRV(Heart Rate Variability) (심박변이도를 통한 급성, 기능성, 기질성 소화불량증의 비교연구)

  • Objectives : This study was designed to evaluate the correlation between patterns of dyspepsia(acute dyspepsia, functional dyspepsia, dyspepsia with the organic disease) and autonomic nerve system dysfunction using HRV analysis. Methods : The patient group consisted of 88 patients diagnosed as a dyspepsia (Acute dyspepsia group-35 patients, Functional dyspepsia group-28 patients, Dyspepsia with the organic disease group-28 patients) who visited in the Oriental Medical Hospital of Dong-eui University from 2005.3.OO to 2008.8.OO. And the control group consisted of 33 patients diagnosed as a normal state of stomach during the same period. We checked HRV of the 4 groups over 5 minutes and compared the HRV index between groups. Results : 1. HF, LF, VLF and TP were significantly lower in the acute dyspepsia, functional dyspepsia and organic dyspepsia patient group than in the control group. 2. HF, LF, VLF and TP were higher in the acute dyspepsia patient group than in the organic dyspepsia patient group, but the differences were not statistically significant. 3. HF, LF, VLF and TP were higher in the acute dyspepsia patient group than in the functional dyspepsia patient group, but the differences were not statistically significant. 4. HF, LF, VLF and TP were lower in the functional dyspepsia patient group than in the organic dyspepsia patient group, but the differences were not statistically significant. Conclusions : Compared to the control group, all of the dyspepsia patient groups showed the tendency that the overall activity of the autonomic nervous system and the activity of sympathetic nerves decreased. Although there was no significant difference in the suppression of the autonomic nervous system, chronic dyspepsia patient group was lower than acute dyspepsia patient group, functional dyspepsia patient group was lower than the organic dyspepsia patient group in HRV.

Clinical Study for Characteristics of Heart Rate Variability in Acute Dyspepsia Patients (급체 환자의 심박변이도 특성에 대한 임상적 연구)

  • Objectives: This research was designed to study the characteristics of heart rate variability in acute dyspepsia patients compared with normal group. Methods: The testing of HRV was carried out at Oriental Medical Center of ${\bigcirc}{\bigcirc}$University with the participation of 94 acute dyspepsia patients and 85 control group people. We checked HRV of the 2 groups that were checked on the first visit day and compared HRV index between groups. Results: In the frequency domain analysis, HF, LF, VLF, LF/HF ratio, TP were significantly lower than control group on acute dyspepsia patient group. HF, LF, VLF, LF/HF ratio, TP were not significantly associated with age. Conclusions: According to this study, autonomic nerve system was more decreased on the acute dyspepsia patient group compared with the control group. Though further studies will be needed, the present finding provide us the possibility that stress is a major cause of the acute dyspepsia along with food factor.

Comparative Study of Acute Dyspepsia Patients Depending on Stress Factor and Food Factor (과심상(過心傷), 음식상(飮食傷)에 따른 급성 소화불량증 환자간의 비교 연구)

  • Dypepsia is the most common gastrointestinal disease and mainly occurs by stress or food. According to the cause of dyspepsia, the clinical characteristics and the curative process are different in patients. The purpose of this study is to compare acute dyspepsia patients depending on food factor and stress factor. For this study, we analyzed the cause, and the symptoms of 59 acute dyspepsia patients and divided them into two groups: one group is 27 acute dyspepsia patient group caused by stress factor (stress factor group). The other group is 32 acute dyspepsia patient group caused by food factor (food factor group). In addition, we carried out HRV test and compared HRV index between two groups. There was no significant difference in past illness involved in gastrointestinal tract diseases between two groups. There were big differences in the frequency of chest discomfort, thurst, lack of appetite, and exhaustion after meals between two groups. There were many changes of tongue fur in food factor group. But there were many changes of tongue body in stress factor group. The pulse pattern is mostly wiry in stress factor group. But, The pulse pattern is mostly slippery in food factor group. HF, TP were significantly lower than food factor group in stress factor group, it means autonomic nerve system was more suppressd in stress factor group than food factor group. Food factor group improved sooner than stress factor group. This study presented the clinically different characteristics (past medical history, symptoms, tongue diagnosis, pulse pattern, curative process) of acute dyspepsia depending on food factor and stress factor. Thus, it provides the necessity of oriental medical diagnosis and treatment called pattern identification for acute dyspepsia.

The Correlation between Deviation Indexes of Ryodoraku and Acute Stress in Patients with Functional Dyspepsia

  • Objectives : The aim of this study was to investigate the relationship between stress type and Ryodoraku score (RS) in patients with functional dyspepsia. Methods : 92 (30 male, 62 female) patients with functional dyspepsia were included in this study. Degree of stress was evaluated by GARS and BEPSI scale which can assess acute and chronic stress, respectively. RS of the patients was measured, and mean, sum of gaps and standard deviation (SD) in their RS were taken. First, patients were divided into two groups by sex, and by mean RS $40{\mu}A$ (above and below). GARS and BEPSI scale of each group was compared. Second, correlation between stress scales (GARS and BEPSI) and variation indexes of RS (sum of gaps and SD) was examined. Result : 1. The mean RS is higher in male patients than in female ones. 2. No significant difference was noted in GARS and BEPSI scale associated with above end below the mean $40{\mu}A$ RS. 3. The sum of gaps and SD of RS has significant correlation with GARS scale but not with BEPSI scale. Conclusions : These results suggest that the sum of gaps and SD of RS are associated with acute stress in patients with functional dyspepsia. Thus, it is thought that RS can be useful in assessing acute stress in patients with functional dyspepsia.

  • Objectives : The purpose of this research was to examine that effect of Sosang($LU_1$)-Eunbaek($SP_1$) bloodletting was similar to effect of Sa-kwan $LI_4$) and Taechung($LR_3$)> acupuncture on pain and discomfort of acute dyspepsia in upper abdomen. Methods : We divided 36 patients with acute dyspepsia into two groups and treated Group A with Sosang($LU_{11}$)-Eunbaek($SP_1$) bloodletting and Group B with Sa-kwan $LI_4$) and Taechung($LR_3$)> acupuncture. Pain and discomfort in upper abdomen was measured by Visual Analogue Scale(VAS) scores of the two groups before treatment and after 15 minutes to treat. Results : Both group A and B represented effective improvement in VAS of pain and discomfort in upper abdomen. And there was no difference between A group and B group statistically. Conclusions : It is suggested that Sosang($LU_{11}$)-Eunbaek($SP_1$) bloodletting may have similar effect compared with Sa-kwan $LI_4$) and Taechung($LR_3$)> acupuncture on acute dyspepsia treatment.

  • A 64 year-old male patient with acute postrenal failure from obstructive uropathy was treated. His symptoms were difficulty voiding, general weakness, fever, chills and dyspepsia. We diagnosed his symptoms as acute postrenal failure from obstructive uropathy and administered Hwajung-tang(和中湯), Jasin-hwan(滋腎丸) and nelaton catheterization. After 30 days, these clinical symptoms had improved and results of BUN:creatinine also showed improvement, so I report this case, but more clinical case reports are needed.

  • Objectives: In comparison with functional dyspepsia, food accumulation is known to have wider concept. On this study, we will make a proposal on the concept of food accumulation by a statistical comparison between pyungweesan questionnaire and health related questionnaires. Methods: We studied the similarities between six factors of pyungweesan questionnaire and four health related questionnaires(Rome II criteria of functional dyspepsia, subjective symptoms of fatigue test, beck depression inventory, state-trait anxiety inventory, etc.) using multidimensional scaling. Results and Conclusions: 1. Physical-emotional axis and acute-chronic axis were labelled in a two-dimensional scaling solution. 2. Seup-dam(濕痰) factor and sik-che(食滯) factor of food accumulation were closely related to fatigue. 3. Bi-seup(脾濕) factor of food accumulation was closely related to depression and anxiety. 4. The findings suggest that a concept of food accumulation reflects some aspects of physical fatigue and emotional depression as well as dyspepsia.

  • 'Chung Shin Whan(靑神丸)' is a Chinese patent medicine, which has been used for stomachic disorder, acute and chronic indigestion, dyspepsia, vomiting and diarrhea in Korea. This medicine consists of 14 kinds of powdery mixtures, microscopic method be used advantageously as it requires only a small amount of specimens. In this paper, the effectiveness of this method is exemplified by the identification of the ingredients in "Chung Shin Whan' which contains 14 powdered crude drug ingredients.

  • Acute hepatitis is caused by virus, alcohol. toxins and drugs. Clinical symptoms of acute hepatitis are severe fatigue, jaundice, nausea, hepatomely, lymphadenopathy. We encountered two cases of acute hepatitis, one C type viral hepatitis and the other hepatitis by overuse of medicine. The symptoms of the latter are severe fatigue, petechia and heartburning. The symptoms of the latter are severe fatigue, nausea, dyspepsia, yellow urine and skin. We recognized that total bilirubin. direct bilirubin, serum transaminase, alkaline phosphatase and gamma glutamyl transferase were elevated. We treated both patients with herb medicine(Gagamsaenggan-tang). Gagamsaenggan-tang has been used to hepatic diseases and have been known to have beneficial effects. The patients' symptoms began to improve after about two weeks of treatment. After 3 weeks on medication, the clinical symptoms and liver function of two patients were improved.

Risk Factors for Recurrent Abdominal Pain in Children with Nonorganic Acute Abdominal Pain

  • Purpose: The purpose of this study was to identify the risk factors for recurrent abdominal pain (RAP) in children who presented with nonorganic acute abdominal pain. Methods: A retrospective, single study was conducted on 2-15-year-old children diagnosed with nonorganic acute abdominal pain at the pediatric outpatient department of Vajira Hospital, Nawamindradhiraj University, between January 2015 and December 2019. The potential risk factors were analyzed using univariate and multivariate analyses. Results: Of the 367 patients with nonorganic acute abdominal pain, 94 (25.6%) experienced RAP within three months. In this group with RAP, 76 patients (80.8%) were diagnosed with functional gastrointestinal disorders, including functional dyspepsia, irritable bowel syndrome, functional abdominal pain-not otherwise specified, and functional constipation. History of gastrointestinal infection (p=0.011), mental health problems (p=0.022), abdominal pain lasting ≥7 days (p<0.001), and change in stool frequency (p=0.001) were the independent risk factors associated with RAP in children with nonorganic acute abdominal pain; their odds ratios and 95% confidence intervals were 3.364 (1.314-8.162), 3.052 (1.172-7.949), 3.706 (1.847-7.435), and 2.649 (1.477-4.750), respectively. Conclusion: RAP is a common problem among children who first present with nonorganic acute abdominal pain. The identification of risk factors may provide proper management, especially follow-up plans for this group in the future.
