Anti-inflammatory action by Gamisangryosamul-tang and The effect of Ziyang-Go on atopic dermatitis-like lesion and pruritus in NC/Nga mice (기미생료사물탕의 항염증효과와 지양고의 아토피피부염 손상 및 지양 효과에 미치는 영향)

  • This research was performed to examine an anti-inflammatory effects of Gamisangryosamul-Tang(GSS) and anti-pruritus effects of Ziyang-Go(Salve). This study was processed by three experiments; Experiment 1: Inhibitory activity of GSS extract on the degranulation of mast cell and histamine release in plasma induced by compound 48/80 i.p, injection after the pretreatment of GSS extract i.p, injection in Sprague-Dawley rats, Experiment 2: Anti-inflammatory effect of GSS extract on macropharge raw 264,7 cells treated by LPS 250 ppm (before 2 hours). Experiment 3: Measurement of passive cutaneous anaphylaxis and atopic dermatitis using NC/Nga mice, GSS extract inhibited histamine release by 70% compared to compound 48/80 treated control group and histologically significantly reduced (P<0,01) the degranulation of mast cell in SD rats. In GSS extract treated group, the expression of TNF-α in macropharge cell showed the remarkable inhibitory effect about 62% (P<0,01) compared to LPS treated control group. The expression of IL-6 appeared more effective by 46% than the LPS treated control group and by 6% compared to hydrocortison treated group, Comparing with steroid (0.05% prednisolon) ointment, Ziyan-Go treated group showed the significant(30%) recovery on skin response index in atopic dermatis like anaphylaxis mice(NC/Nga), Finally, in scratching behavioral tests of NC/Nga mice for three weeks, Ziyang-Go treated group significantly (P<0.05) suppressed the pruritus on the face, neck, ears and dorsal skin than inbred NC/Nga mice. However, the change of IgE and IFN-γ from the spleen cell of NC/Nga mice was not significantly different between the oral intake of GSS extract group and of saline intaked control group. Summary and Conclusion: This study demons trates that Ziyang-go have the equal anti-pruritus effect to steroid ointment and GSS extract have the notable immunologic activity on inflammatory in vivo and in vitro model. Advanced experiment of this study will be required for more reliable information about the correlation between the lymphokine (i.e. IgE) and the anti-allergic effects of GSS.

Anti-atopic dermatitis effects of Poncirus trifoliata Rafinesque via regulation of immune response and nerve growth factor (면역반응 및 신경성장인자 조절을 통한 지실의 항아토피 효능)

  • Objectives: Poncirus trifoliata Rafinesque has been known to have anti-allergic effects in skin diseases. However, anti-atopic dermatitis effects of P. trifoliata Rafinesque have not been studied yet in skin diseases. The present study evaluated the anti-atopic dermatitis effects of P. trifoliata Rafinesque (PTR) using external treatments on AD. Methods: AD lesions were induced by the repeated application of 2, 4-Dinitrochlorobenzene (DNCB) on the shaved back of BALB/c mice. $100{\mu}{\ell}$ of PTR extracts was applied to the AD lesions for 11 days. Histological assessments, mast cells count and serum levels of IgE were analyzed. The anti-pruritic effects of PTR were examined by the change of scratching frequency and nerve growth factor (NGF) expression. In addition, the anti-inflammatory effects of PTR were examined by the expressions of Th2/Th1 cytokines and pro-inflammatory in dorsal skin. Results: Histopathological findings showed that topical application of PTR decreased the thickness of dermal and epidermal skin compared with the DNCB group. PTR also notably decreased the mast cells count and serum IgE. The scratching behavior of mice and expression of NGF were significantly reduced. In addition, PTR group significantly suppressed the IL-4, IL-6, IFN-${\gamma}$ and TNF-${\alpha}$ cytokines compared to the DNCB group. Conclusions: These results indicated that P. trifoliata Rafinesque possess anti-pruritus and anti-atopic dermatitis properties. Therefore, P. trifoliata Rafinesque might be used for treatment of pruritus and atopic dermatitis.

Comparative Research of Pruritus Group and Non Pruritus Group in Hemodialysis Patients (혈액투석환자 중 소양증군과 비소양증군의 특성 비교연구)

  • Purpose: The purpose of this study was to compare characteristics of two groups (pruritus and non pruritus group), in hemodialysis patients. Method: The degree of ichting in 130 patients treated at the D Medical Center Hemodialysis Unit was examined by itching severity & sites tool, itching observation tool and itching NRS tool. The data were collected from October 1 to 12, 2007 and analyzed by student t-test and Mann-Whitney test. Result: Sixty three patients (48.3%) of hemodialysis patients had pruritus. The severity and number rating scale score of pruritus in one day before hemodialysis was higher than during and after hemodialysis. The frequency of dialysis and usage of anti-histamine drug were significantly lower in nonpruritis group than in pruritus group (F=5.209, p=.022, F=6.549, p=.010). The other general characteristics, hemodialysis-related characteristics and laboratory data were not significantly different between the two groups. The depression score was significantly higher in pruritus than in nonpruritus group (t=-2.505, p=.017). But there were no significant differences in hostility and anxiety between the two groups. Conclusion: Residual renal function and depression were associated with pruritus in hemodialysis patients. Efforts for preservation of residual renal function and emotional support may decreased severity and frequency of pruritus in hemodialysis patients.

Anti-pruritic Effect of Botulinum Toxin Type A against Histamine-induced Pruritus on Canine Skin (개에서 Histamine으로 유발한 피부소양증에 대한 보툴리늄 톡신의 항소양 효과)

  • Botulinum toxin type A (BoNT/A) has been proven to be a safe and effective treatment for wrinkles in human. As well as the anti-wrinkle effects, the anti-pruritic effect of BoNT/A has been revealed from several researches for new therapy. The aim of this study was to investigate the anti-pruritic effect of BoNT/A against histamine-induced pruritus on canine skin. Five clinically healthy beagles were included in the study. All dogs were received 0.05 ml (5 Unit) of BoNT/A on the right dorsal thoracic region as an experiment and the same volume of saline solution was injected on the left dorsal thoracic region as a control, respectively. Intradermal histamine injections were performed four times (before treatment and days 1, 3 and 7 after BoNT/A injection). The severity of pruritus, the diameter and thickness of wheal, the erythema index and cutaneous temperature were assessed. The severity of pruritus was reduced on BoNT/A treated sides, compared with saline treated sides (p < 0.05). BoNT/A decreased wheal size, in both diameter and thickness (p < 0.05). Although, erythema index of both sides were increased after first histamine injection, BoNT/A treated sides showed the low-value as compared with saline treated sides. Cutaneous temperature was decreased significantly on BoNT/A treated sides. These results indicates that BoNT/A reduce histamine-induced pruritus on canine skin and suggested a possibility of application of BoNT/A for local intractable dermatologic problem in dogs.

Analysis of Potential Active Ingredients and Treatment Mechanism of Atractylodes Lancea(Thunb.) D.C and Magnolia Officinalis Rehder et Wilson for Dermatitis Accompanied by Pruritus Using Network Pharmacology (네트워크 약리학을 이용한 소양증을 동반한 피부 염증에 대한 창출(蒼朮) 및 후박(厚朴)의 잠재적 치료기전 탐색)

  • Objectives : To investigate the active compounds and therapeutic mechanisms of Atractylodes Lancea(Thunb.) D.C. and Magnolia Officinalis Rehder et Wilson in the treatment of dermatitis accompanied by pruritus, as well as their potential to complement or replace standard drugs. Methods : We conducted the network pharmacological analysis. We selected effective ingredients among the active compounds of research target herbs. Then we explore pathway/terms of the common target proteins among research target herbs, fexofenadine and disease. Results : We selected 9 active compounds are selected from Atractylodes lancea and identified 231 target proteins. Among them, 74 proteins are associated with inflammatory skin diseases that cause pruritus. These proteins are involved in various pathways including, 'Nitric-oxide synthase regulator activity', 'Hydroperoxy icosatetraenoate dehydratase activity, Aromatase activity', 'RNA-directed DNA polymerase activity', 'Arachidonic acid metabolism', 'Peptide hormone processing', 'Chemokine binding' and 'Sterol biosynthetic process'. Additionally, coregenes are involved in 'IL-17 signaling pathway'. Similarly, we selected 2 active compounds from Magnolia officinalis and identified 133 target proteins. Among them, 33 proteins are related to inflammatory skin diseases that cause pruritus. These proteins are primarily involved in 'Vascular associated smooth muscle cell proliferation' and 'Arachidonic acid metabolism'. There is no significant difference between the pathways in which coregenes are involved. Conclusions : It is expected that Atractylodes Lancea will be able to show direct or indirect anti-pruritus and anti-inflammatory effects on skin inflammation accompanied pruritus through suppressing inflammation and protecting skin barrier. Meanwhile, it is expected that Magnolia Officinalis will only be able to show indirect anti-inflammation effects. Therefore, Atractylodes Lancea and fexofenadine are believed to complement each other, whereas Magnolia Officialinalis is expected to provide supplementary support on skin disease.

Anti-inflammatory and Anti-Pruritonic Effects of WSY-1075 composited with Medicinal Plants on the Activated Rat Peritoneal Mast cells and Mouse Pruritus (활성화된 렛트 비만세포와 마우스 소양증에 대한 한약재로 조성된 WSY-1075의 항염증 및 항소양 효과)

  • Objectives : This study was to evaluate the anti-inflammatory and anti-pruritic effects of WSY-1075 composited with Corni Fructus, Angelica gigantis Radix, Lycii Fructus, Ginseng Radix, Cervi parvum Cornu and Cinnamomi Cortex in rat peritoneal mast cells (RPMCs) and scratching mouse model. Methods : WSY-1075 was prepared by extracting with 30% ethanol. In the present study, we investigated the effect of WSY-1075 on the production of tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$), interleukin-$1{\beta}$ (IL-$1{\beta}$) and histamine in rat peritoneal mast cells (RPMCs) activated with phorbol 12-myristate 13-acetate (PMA) plus calcium ionophore A23187, and on the scratching behavior in mice treated with pruriogens. Results : WSY-1075 was not cytotoxic effect in used all concentration. PMA plus A23187 treatment significantly increased TNF-${\alpha}$, IL-$1{\beta}$ and IL-6 production compared with media control in RPMCs. However, TNF-${\alpha}$, $IL1{\beta}$ and IL-6 production increased by PMA plus A23187 treatment were significantly inhibited by WSY-1075 (200 ${\mu}g/mL$ and 400 ${\mu}g/mL$). WSY-1075 also inhibited the histamine release from RPMCs stimulated by compound 48/80, which promotes histamine release. Moreover, WSY-1075 administration had an inhibitory effects on the scratching behavior induced by pruritogen (compound 48/80, histamine, serotonin and substence P) in ICR mice. Conclusion : These results suggest that WSY-1075 administration (200 mg/kg or 400 mg/kg) has the anti-inflammatory and anti-pruritic effects on the activated rat peritoneal mast cell and mouse pruritus. WSY-1075 has a potential use as a composition of medicinal plants for treatment against inflammation- and pruritus-related disease.

Effects of topical application of realgar on pruritus and inflammation of atopic dermatitis (웅황(雄黃) 외용 도포의 아토피성 피부염 소양증 완화 및 항염증(抗炎症) 효능)

  • Objectives: Realgar has been frequently used for skin disorders in history of herbal medicine. However, the efficacy of realgar has not been examined in atopic dermatitis(AD). In this study, the effects of realgar on AD were investigated, especially on pruritus and inflammation. Methods: AD lesions were induced in the shaved backs of BALB/c mice through repeated application of DNCB. The mice were treated for 11 days with 1% realgar ($100{\mu}L/day$). Histological changes in skin thickness were observed. The anti-pruritic effects of realgar were evaluated by the change in numbers of scratching behavior of mice and expression of substance P. The expressions of cytokines IL-4 and IL-6 were measured. Also, anti-inflammatory effects of realgar were examined on expressions of NF-${\kappa}B$, phospho-$I{\kappa}B{\alpha}$ and mitogen-activated protein kinases (MAPKs). Results: Realgar decreased skin thickness (both dermal and epidermal) 38% and 17% respectively, compared to positive control, DNCB group. The scratching behavior of mice was reduced by 42% and expression of substance P was significantly less. Cytokines IL-4 and IL-6 were significantly reduced by 52.6% and 77.6%, respectively. The expressions of NF-${\kappa}B$, phospho-$I{\kappa}B{\alpha}$ and MAPKs (phospho-ERK1/2, -p38 and -JNK) were significantly suppressed with marked effects on phospho-ERK1/2. Conclusions: The collective results suggest that realgar shows anti-pruritic and anti-inflammatory effects on AD. And realgar might be a potential therapeutic candidate for treatment of atopic dermatitis.

Analysis of Potential Active Ingredients and Treatment Mechanism of Ponciri Fructus Immaturus for Dermatitis Accompanied by Pruritus Using Network Pharmacology (네트워크 약리학을 이용한 소양증을 동반한 피부 염증에 대한 지실(枳實)의 잠재적 치료기전 탐색)

  • Objectives : To identify the active ingredient of Poncirus Trifoliata Immaturus and to explore the mechanism expected to potentially act on dermatitis accompanied by pruritus. Methods : We conducted the network pharmacological analysis. We selected effective ingredients among the active compounds of Poinciri Fructus Immaturus. We found the target protein of the selected active ingredient, disease(dermatitis accompanied by pruritus) and fexofenadine. Then we established the network between the proteins which Poinciri Fructus Immaturus and fexofenadine intersected with disease respectively, and the coregene was also extracted. After that, the active pathways in the human body involving the groups and coregenes were searched. Results : Total of 7 active ingredients were selected, and 202 target proteins were collected. There were 756 proteins related to inflammatory skin disease accompanied by pruritus, and 75 proteins were related to fexofenadine. 42 proteins crossed by Poinciri Fructus Immaturus with a disease, and 31 proteins crossed by fexofenadine with a disease. 12 proteins were found as a coregene from the proteins that cross Poinciri Fructus Immaturus and disease. Coregenes are involved in 'Nitric-oxide synthase regulator activity', 'Epidermal growth factor receptor signaling pathway'. 2 groups that extracted are invloved in 'Fc receptor signaling pathway', 'Central carbon metabolism in cancer', 'Phosphatidylinositol 3-kinase complex, class IB', and 'omega-hydroxylase P450 pathway'. Conclusion : It is expected that Poinciri Fructus Immaturus will be able to show direct or indirect anti-pruritus and anti-inflammatory effects on skin inflammation accompanied by pruritus in the future. And it is also expected to have a synergy effect with fexofenadine on skin disease.

Anti-inflammatory and Anti-itching Effects of Herbal Medicine Complex Extracts(NI-01) (한약복합추출물(NI-01)의 항염증 및 소양감 억제 효과)

  • The purpose of this study was to investigate the possibility of herbal medicine complex extract (NI-01), which were prepared from 6 natural materials (Cinnamomum cassia Blume, Lonicerae Flos, Paeonia suffruticosa Andrews, Arctium lappa Linne, Schzandra chinesis Bailon, Elsholtzia ciliata Hylander), as a functional material for inhibition of atopic dermatitis. anti-oxidative activity was confirmed by measuring DPPH electron donating ability and ABTS+ radical scavenging ability. Cytotoxicity and NO inhibition were measured using RAW 264.7 cells to confirm anti-inflammatory efficacy. The test substance was orally administered to the pruritus-induced ICR mice to confirm the inhibition of pruritus. The bovine cornea opacity and permeability (BCOP) assay was performed to confirm safety for irritation. NI-01 showed high antioxidant activity in DPPH and ABTS+ methods. In the anti-inflammatory effect tests with RAW 264.7 cells, NO production was inhibited at NI-01 concentrations of 50 (14.9%) and 100 (4.2%) ㎍/mL, which indicated that the anti-inflammatory effect was increased in a concentration-dependent manner. NI-01 also showed anti-itching effect after inducing of itching by compound 48/80 in ICR mice. NI-01 was proved to be a non-irritant substance in BCOP assay. The results of this study suggested that the herbal medicine combined extract (NI-01) has high antioxidant, anti-inflammatory and anti-itching effects, and safety for irritation. Therefore, herbal medicine complex extract (NI-01) is thought to be highly applicable for the inhibitory ingredients of the atopic dermatitis.

Anti-Allergy and Anti-Pruritic Effects of Diospyros lotus L. Leaf Extract (고욤(Diospyros lotus L.)잎 추출물의 항알레르기 및 항가려움 효과)

  • The worldwide prevalence and severity of allergic diseases including atopic and contact dermatitis has increased dramatically over the past decade, especially in developed countries. Mast cells are important effector cells in allergic reactions. The purpose of this study was undertaken to investigate the anti-allergic and anti-pruritic effects of Diospyros lotus leaf extract (DLE). DLE was prepared by extracting with distilled water. In the present study, we investigated the effect of DLE on the production of tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$), interleukin-$1{\beta}$ (IL-$1{\alpha}$) and histamine in rat peritoneal mast cells (RPMCs), and on the skin lesion, leukocyte infiltration and scratching behavior in mice. Phorbol 12-myristate 13-acetate (PMA) plus calcium ionophore A23187 significantly increased TNF-${\alpha}$ and IL-$1{\beta}$ production compared with media control. However, TNF-${\alpha}$ and IL-6 production increased by PMA plus A23187 treatment were significantly inhibited by DLE in a dose-dependent manner. DLE also inhibited the histamine release from RPMCs stimulated by compound 48/80, which promotes histamine release. Moreover, DLE administration had an inhibitory effects on the scratching behavior induced by pruritogen (compound 48/80, histamine) in ICR mice. Furthermore, DLE inhibited the skin lesions, inflammatory and mast cells in hairless mice sensitized by 2,4-dinitrofluorobenzene (DNFB). DLE administration reduced the IL-4 and IgE production induced by DNFB sensitization in hairless mice. These results suggest that DLE has a potential use as a herb medicine for treatment against allergy and pruritus-related disease.
