Objectives : A rare case of dural tear ensuing after a cervical Chuna Manipulation Treatment leading to cerebrospinal fluid (CSF) leakage in the lower cervical and upper thoracic spine was found, so we report it. Methods : A 32-year-old woman presented with back and neck pain in 1 days earlier. The patient undertook a cervical Chuna Manipulation Treatment. After this maneuver, the patient complained of an orthostatic headache with nausea. The patient's headache worsened, and lying down gave the only measure of limited relief. In Brain CT and MRI study, nonspecific finding was detected. In Cistemography study, CSF leakage at lower cervical or upper thoracic area was detected. Results and Conclusions : It is supposed that this patient suffered a dural tear and CSF leakage secondarily due to a cervical Chuna Manipulation Treatment. From this case, we can understand the etiology of dural tear to some extent and consider the complication of Chuna Manipulation Treatment. In the future, more study, research and prospective trial for complications of a cervical Chuna Manipulation Treatment is needed.

Objectives : The purpose of this study is to explore the trends of Chuna(manipulation) treatment on neck pain and cervical disc displacement. Methods : We set up the two searching strategies and investigated the latest clinical trials on Chuna(manipulation) therapy for Neck disorders and cervical disc herniation through PubMed search. The searched papers were analyzed and the year, journals, article types of literatures were examined. Of the articles, Randomized controlled trials(RCTs) are assessed by Jadad scale. Results : Studies on Chuna(manipulation) treatment to cervical disc herniation were very few. Articles on Chuna(manipulation) treatment to neck pain were relatively more than those of cervical disc herniation. The most frequently published journal was J Manipulative Physiol Ther. The main design of study was RCT. The limited quality assessment of RCT was conducted by Jadad scale and the average Jadad score was 1.73, which indicates low quality in terms of Jadad evaluation criteria. Conclusions : International trends of studies on Chuna therapy of cervical disease is increasingly reviewed. Although this review has several limitations, analyzing the latest trends of studies on Chuna(manipulation) treatment on neck pain and cervical disc displacement is necessary for further studies.

The occurrence of neurologic symptoms after cervical manipulation(Chuna treatment) seems to be an uncommon event considering the large number of manipulations performed. The precise incidence is unknown, for only patients with serious complications seek medical care. We recently experienced a 3D-year-old woman who developed sudden neurologic complications after cervical Chuna treatment by a nonprofessional therapist. Whether the manipulation is the major causative factor in the subsequent stroke can be disputable and must be considered in each case. Physicians and patients, however, should be aware that cerebrovascular accidents may occur as a result of cervical manipulation.

This case report describes the effectiveness of cervical chuna manipulation in the management of traumatic temporomandibular joint disorder. A 14-year-old male developed trismus after being stuck. He received cervical chuna manipulation and acupuncture. The mouth opening range was assessed to measure disability, and the visual analogue scale to measure discomfort. This was carried out at the beginning, during and at the end of treatment. After treatment, the patient's mouth opening range was improved from 10 mm to 45 mm. Also, the visual analogue scale was decreased. This case study suggests that cervical chuna manipulation can be effective for the management of traumatic temporomandibular joint disorder.

Objectives : The os odontoideum is an uncommon abnormality which is due to non fusion of axis and odontoid process that exists as a separate ossicle apart from a hypoplastic dens. Its clinical significance lies in its potential to produce serious neurologic symptoms after cervical Chuna Manipulation Treatment. We experienced a patient diagnosed as os odontoideum which corresponds to absolute contra-indications of chuna manipulation treatment and report it. Methods : The patient was diagnosed as os odontoideum through cervical spine CT and MRI. Results and Conclusion : During cervical physical examination, we encountered L'hermitte's phenomenon from the patient and suspected one of contra-indications of chuna manipulation treatment. We could diagnose the patient as os odontoideum through cervical spine CT, MRI and prevent him from severe sequelae caused by chuna manipulation treatment.

Objective : We had good effects on two patients who had problem with temporomandibular joint by using upper cervical manipulation combined with acupuncture therapy. Methods : Two temporomandibular disorder cases were managed by using upper cervical manipulation, combined with acupuncture therapy. We used Toggle-recoil technique for the purpose of upper cervical manipulation. Results :Even though symptoms of two patients were not same, each cases showed clinical changes by using upper cervical manipulation. Two cases had effects on decreasing pain of temporomandibular joint, and at the first case, the range of motion of temporomandibular joint was increased. Conclusion : We could treat temporomandibular disorder patients by using upper cervical manipulation combined with acupuncture therapy.

The Klippel-Feil Syndrome (KFS) is characterized by congenital fusion of two or more vertebrae with hypermobile normal segment. According to this, a patient with KFS can be at risk of severe neurological symptoms after manipulation treatment. We had a KFS patient who suffered from neck pain and limited range of motion at cervical after manipulation treatment. The patient, 49-year-old woman was diagnosed as KFS through Cervical X-ray and MRI. The patient was treated by acupuncture therapy and SCENAR therapy. We measured neck pain by visual analog scale (VAS) and neck disability index (NDI) and checked range of motion at cervical before and after the treatments. After 8 times treatments, the patient's pain decreased and the range of motion increased. From this case, we can recognize the risk of Chuna manipulation treatment for KFS patient. So, we suggest that radiological examination is needed before cervical Chuna manipulation treatment to avoid adverse reactions.

Objectives: The present study examines the domestic trend of Chuna manual therapy on cervical spinal disorders in Korea. Methods: We investigated the studies on Chuna manual therapy for cervical spinal disorders via searching 6 Korean web databases. As a result, 51 research papers were found to be analyzed according to their published year, published institution, the titles of journals, the types of study, the techniques of Chuna, the instruments for assessment, the Chuna technique and the number of the treatment trials by the cases of cervical spinal disorders and ethical approvals. Results: The number of the research papers published tended to increase every year. The studies on Chuna manual therapy for cervical spinal disorders were mainly published in The Journal of Korea CHUNA Manual Medicine for Spine & Nerves. The most frequently adopted technique of Chuna in the examined studies was Chuna spine manipulation therapy - cervical spine technique. Visual analogue scale (VAS), neck disability index(NDI) were used as primary means of assessments. The ethical problems of the examined studies needed to be improved. Conclusions: Reviewing the domestic trend of studies on Chuna manual therapy for cervical spinal disorders and examining the strong and weak points of those treatments are essential for the future studies. It is anticipated that this review benefits the future in-depth study on the treatments for Chuna in Korean medicine.

Objectives : The aim of this systemic review was to summarize and compare adverse effect of spinal manipulation at domestic and abroad. Methods : We searched articles in Korean databases and Pubmed. We selected and analysed the articles met inclusion criteria. Results : 51 cases were included. 32 cases in Pubmed show that adverse effect related to cervical rotation and high velocity low amplitude(HVLA) thrust manipulation. It leads to severe adverse effect and sequela about vascular. While, 19 cases in Korean articles shows that domestic spinal manipulation is more safe becase of lower use of high velocity low amplitude(HVLA) thrust. It leads to low incidence of vascular adverse effect and no sequela. Conclusions : We concluded that Chuna is difficult manipulation because of possibility of adverse effect. Chuna is recommended to be taken by licensed medical practitioners who are skilled and complete the regular curriculum. Further studies like meta analysis about spinal manipulation are needed to estimate incidence of adverse effect. And adverse effects are going to be collected continually through randomized controlled trials(RCT) related to spinal manipulation.

Objectives : The purpose of this study is to investigate the effect of chuna treatment on cervical sprain caused by traffic accident in early stage. Methods : This study carried out on 20 patients who have received hospital treatment in Daejeon Univ. cheonan Oriental Hospital. Non-chuna group got acupunture-therapy, herbal medication, physical therapy and Chuna group got chuna treatment besides. We measured Heart Rate Variability(HRV) and Visual Analog Scale(VAS) on 2nd, 4th day. Results : After being treated by our methods, Chuna Group showed the inclination to balance the sympathetic and parasympathetic nerve. In chuna group, an autonomic nerve activity showed the inclination to increase. But there were no significant difference between both groups. Chuna group's VAS were significantly decreased(p=0.043). Conclusions : The results suggest that Chuna treatment help traffic accident patients in early stage to reduce pain. Refer to autonomic nerve system, chuna treatment seem to do positive effect but Further long tenn study in a large scale is needed.
