만성 성인형 아토피 피부염의 한방 치험 3례 (Three Cases of Chronic Adult Atopic Dermatitis)

  • Introduction: Atopic dermatitis is chronic eczematous dermatosis which is related strongly to a family history of atopic disease and hereditary disposition. Its threshold of itching is low so that severe itching occurs and causes secondary eczema. Adult atopic dermatitis has mostly severe symptoms and progresses to chronic relapsing. Objective: The purpose of this study was to estimate the efficacy of oriental medical treatment and management on chronic adult atopic dermatitis. Subject: We studied 3 patients who visited Dongguk University Gangnam Oriental Hospital Dept. of Dermatol. with chronic adult atopic dermatitis from August 2001 to April 2002 and were observed for over 4 months. Methods: We treated them with herb-medication, acupuncture, negative therapy, aroma therapy, full spectrum visible ray therapy(by carbon arc) and external application(Jaun-go). We used the SCORAD index to assess the severity of atopic dermatitis. Results: There were 3 patients with chronic adult atopic dermatitis we could check for over 4 months by clinical scoring using the SCORAD system. 1. The grades of the severity of atopic dermatitis were lower only by the oriental medical treatments and management. 2. Intensities of relapse cases were lower then before the oriental medical treatments. 3. The secondary treatment term for relapse cases was shorter then the first treatment term after oriental remedies were used. Conclusion: Considering the above results, more study is needed on the chronic relapse cases with adult atopic dermatitis, the factors of increasing seventy of adult atopic dermatitis, the prognostic factors of adult atopic dermatitis, and the clinical interchange of chronic adult atopic dermatitis between oriental medicine and western medicine.

  • Background : Atopic Dermatitis is a chronic and recurrent skin disease. it is characterized as itch-scratch cycle and topical or systemic use of corticosteroids is required in western medicine. Though its effects on major symptoms are impressive, these treatments are prone to several side effects and tend to recur after months or years. So potential treatments have been actively studied in Estern medicine, especially Sasang constitutional therapy, which has fewer side effects. Objectives : The purpose of this study was to examine the effect of Sasang constitutional therapy on treating severe adult Atopic Dermatitis and preventing its recurrent symptoms. Methods : fifteen patients who had severe adult Atopic Dermatitis attended treatment of Sasang constitutional therapy and were followed up for one year. An oriental internal specialist first isolated Sasang constitution and a distinctive body type. Grading of Atopic dermatitis was measured by the guidelines offered by Rajka G. and Langeland T. Results : After treatment with Sasang constitutional therapy, the patients symptoms were improved and the mean Rajka and Langeland score reduced significantly from $7.73{\pm}0.79\;to\;3.4{\pm}10.91$. During the follow-up period of one year, the mean Rajka and Langeland score was $2.72{\pm}2.06$ which was lower than last treatment. Conclusions : The results of this study support Sasang constitutional therapy as an effective therapy for severe adult Atopic Dermatitis. Follow up data suggest that this Eastern medical approach is of greater clinical value than the western medical approach in the long term.

  • We observed and treated a 19 year-old female who had atopic dermatitis. Even though she had been treated with western medicine, she still complained of severe itching, erythema, lichenification, scaling, dryness, oozing and insomnia especially on her face and neck. We treated her with Sasang Constitutional Therapies(Kupoong-san, Teumchoui-tang, Kalkunsungki-tang), apcupuncture, herbal irrigation and moisture emulsion. Having been treated for 24 days in hospital admitted, her symptoms were reduced and changed from severe to mild grade in SCORAD(SCORing Atopic Dermatitis)

  • Background: Atopic dermatitis is chronic exzemaous dermatosis characterized by severe itching, licalized eruption and chronic relapsing progress and has familu history of atopic diseases and hereditary disposition. This Study is attempted in order to estimate of possibility of oriental treatment on atopic dermatitis. Objective: We studied 24 years old women with atopic dermatitis from June 2004 to July 2004 Method: We treated her with herb medication and used three index to assess the severity of atopic dermatitis. Results: Each index show us about 50-60$\%$ of improvement Conclusion: On the basis of this study, It should be considered that treatment of atopic dermatitis should be focused on fever of yang-ming.

  • Objectives : The purpose of this report is to investigate the effect of Taeeumin Cheonghyeolgangki-tang on chronic contact dermatitis caused by cement. Methods : A 52-year-old male patient was diagnosed with contact dermatitis with the Questionnaire for Sasang Constitutional Classification-II (QSCC-II) as well as the four methods of diagnosis (四診), and treated with Taeeumin Cheonghyeolgangki-tang (qīngxuèjiàngqí-tāng). We observed redness and scaling on both palms, and recorded photographs of any changes during the inpatient and outpatient treatment period. Results : The symptoms of contact dermatitis on both palms were significantly improved. Scaling and cracked skin disappeared and the redness almost resolved. Conclusions : Therefore, Taeeumin Cheonghyeolgangki-tang ( qīngxuèjiàngqí-tāng ) is effective on chronic contact dermatitis caused by cement.

  • Introduction: Atopic dermatitis is chronic eczematous recurrent disease of skin and till now its mechanism is not cleary known. It is related to a family history of atopic disease. It has severe itching and it causes eczema secondarily. Atopic dermatitis has mostly severe symptoms and progresses to chronic relapsing. Objectives: The purpose of this study was to estimate the efficacy of herb- medicine treatment on atopic dermatitis. Subjects: 1 treated 5 patients who visited Chungbalsan Kyunghee oriental medical clinic with atopic dermatitis from September, 2001 to October, 2002 partiticipated in the study and were observed for over 2 months.   • Background: Atopic dermatitis is chronic eczematous dermatosis characterized by severe itching, localized eruption and chronic relapsing progress and has family history of atopic diseases and hereditary disposition. This Study is attempted in order to estimate the efficacy of herb medication treatment on atopic dermatitis. Objective: We studied 35 patients with atopic dermatitis from October 2001 to September 2002. Methods: We treated them with herb-medication and used the SCORAD index to assess the severity of atopic dermatitis. Results: Of the 35 patients that were treated, the SCORAD index was lowered to $30.5{\pm}20(3.5-94)$ than before of which the SCORAD index was $46.8{\pm}18.1(14.9-97.5)$. Conclusion: On the basis of this study, It should be considered that treatment of atopic dermatitis should be focused on the gastrointestinal system. Moreover a balanced diet should be considered

Inhibitory Effect of Ginsenoside Rg5 and Its Metabolite Ginsenoside Rh3 in an Oxazolone-Induced Mouse Chronic Dermatitis Model

  • The effect of a main constituent ginsenoside Rg5 isolated from red ginseng and its metabolite ginsenoside Rh3 in a chronic dermatitis model was investigated. Ginsenosides Rg5 and Rh3 suppressed swelling of oxazolone-induced mouse ear contact dermatitis. These ginsenosides also reduced mRNA expressions of cyclooxygenase-2, interleukin $(IL)-1{\beta}$, tumor necrosis factor $(TNF)-{\alpha}$ and interferon $(IFN)-{\gamma}$. The inhibition of ginsenoside Rh3 was more potent than that of ginsenoside Rg5. These findings suggest that ginsenoside Rh3 metabolized from ginsenoside Rg5 may improve chronic dermatitis or psoriasis by the regulation of $IL-1{\beta}$ and $TNF-{\alpha}$ produced by macrophage cells and of $IFN-{\gamma}$ produced by Th cells.

  • The process of drug discovery and drug development consumes billions of dollars to bring a new drug to the market. Drug development is time consuming and sometimes, the failure rates are high. Thus, the pharmaceutical industry is looking for a better option for new drug discovery. Drug repositioning is a good alternative technology that has demonstrated many advantages over de novo drug development, the most important one being shorter drug development timelines. In the last two decades, drug repositioning has made tremendous impact on drug development technologies. In this review, we focus on the recent advances in drug repositioning technologies and discuss the repositioned drugs used for inflammatory diseases such as sepsis, asthma, and atopic dermatitis.

  • The results of the study about comparison of Eastern-Western medicine on the Atopic dermatitis were as follows. 1. Atopic dermatitis is chronic eczematous dermatosis which have severe itching, characteristic eruption, easily occur region and pass on chronic relapsing progress and have family history of Atopic disease and hereditary disposition. 2. Atopic dermatitis assume an remarkable clinical aspect and it's diagnosis depends on family history and clinical symptom. 3. In all cases of Western medical treatment is nothing but a symptomatic treatment because can not find out certainly the cause of Atopic dermatitis. 4. Atopic dermatitis is belong to the category of the 'Naesun(내癬)', 'Taeryumchang(胎斂瘡)', 'Samanpoong(四灣風)' etc. in Oriental medicine. 5. The etiology and pathogenesis of Atopic dermatitis in oriental medicine are congenital defect(稟賦不足), internal accumulation of damp and heat(濕熱內鬱), improper diet(飮食不節), exogenous pathogenic factors(外邪侵襲), etc.. 6. The treatments of Atopic dermatitis in oriental medicine are thought effective clear up heat and remove dampness with febrifugal arld diuretic drug(淸熱利濕), invigorate the spleen to resolve dampness(健脾燥濕), nourish Um and blood to relave dryness(滋陰養血潤燥), etc..
