A Study on the Concordance Rate of the Diagnostic Results Among Sasang Constitutional Medicine(SCM) Specialists (사상의학(四象醫學) 전문의(專門醫)의 체질진단(體質診斷)결과 일치도에 관한 연구)

  • 1. Objectives The purpose of this study was to investigate the concordance rate of the diagnostic results among Sasang Constitutional Medicine(SCM) specialists. 2. Methods We carried out two studies in order to investigate the concordance rate of the diagnostic results between SCM specialists. We calculated the kappa value for analyzing concordance rate(using the SPSS program). 1) Study A : We investigated 142 clients who had taken health examinations and diagnosis of Sasang constitution from March 9, 2005 to March 29, 2005 at a health examination center of Kyung-Hee medical center. Every SCM specialist diagnosed 142 clients directly. Each SCM specialist don’t know the other specialist’s diagnosis. 2) Study B : We investigated 176 clients who had taken health examinations and diagnosis of Sasang constitution from January 2005 to March 2005 at the department of Occupational and Environmental medicine of Kyung-Hee medical center. One SCM specialist diagnosed 176 clients directly and the other specialist knowing previous specialist’s diagnosis, diagnosed each person indirectly 3. Results 1) The concordance rate of the diagnostic results among SCM specialists in study A was relatively high(kappa value=0.700). 2) The concordance rate of the diagnostic results among SCM specialists in study B was high(kappa value=0.895). 4. Conclusions We can confirm the concordance rate of the diagnostic results among SCM specialists is relatively high.

Comparative Studies on the Concordance Rate of Pulse Condition Interpretation between Interpreters and Pulse Analyser (맥진기 결과에 대한 판독자간의 판독 일치율 비교 연구)

  • Objectives : The purpose of our investigation is to determine degrees of concordance rate among interpreters. Furthermore, we have examined how much concordance rate to come out when beginners have been compared with the pulse analyzer. Methods : Thirty-nine volunteers were enrolled for this study. These subjects took a 5-minute rest in a sitting position as instructed by the protocol. As they were not allowed to move or speak, radial artery pulse conditions were measured on the lower arm of each subject by means of the pulse analyzer under investigation. Two Korean medical doctors, who did not know the status of default pulse conditions, were also instructed to intuitively choose the most corresponding one in comparison with 13 default pulse conditions. Subsequently, we investigated results between interpreters as well as results between interpreter and pulse analyzer. Results : The total concordance rate, with similar concordance rates being included, between interpreters, between interpreter A and pulse analyzer, and between interpreter B and pulse analyzer was 56.4%, 79.5%, and 71.8% respectively. In faint fine weak pulse(微細弱脈) case, interpreter A and B selected 6 and 7 cases respectively, matched the concordance rate 5, and corresponded separately with the pulse analyzer interpreting 8 cases. Conclusions : In case of skipping pulse 2(促2脈), short pulse(短脈), faint fine weak pulse(微細弱脈), the concordance between interpreters also matches with results drawn from the pulse analyzer. The concordance rate goes higher in proportion with such smaller pulse conditions as faint fine weak pulse(微細弱脈) and short pulse(短脈).

Electrical Detection of Ear Acupuncture Points and Musculoskeletal Pain (경혈탐측기에 반응한 이혈(耳穴)과 신체 동통 부위와의 관계 연구)

  • Objectives : The objectives of this study were to investigate the relationship between electrical detection of ear acupuncture points and musculoskeletal pain. Methods : 18 adults who have musculoskeletal pain without trauma factorsparticipated in this study. They answered the questionnaire and their ear acupuncture points were examined with electrical detectors. We analyzed the relationship between electrical detection of ear acupuncture points and musculoskeletal pain with concordance rate and validity. Results : Total concordance rates of the head region was 68.00%(questionnaire) 32.08%(investigation), that of vertebral region was 67.86%, 59.38%, that of both upper limbs was 86.67%, 39.69%, and that of both lower limbs was 50.00%, 23.46%. The true positive rate was 0.704, the true negative rate was 0.492, the false positive rate was 0.508, and the false negative rate was 0.296 in the validity test. In the head, two concordance rates of the temporal and occipital regions were relatively higher than those of the parietal and frontal regions. In the vertebral region, two concordance rates of the cervical and lumbar regions were relatively higher than those of the thoracic and sacrum regions. In the upper limb, two concordance rates of the shoulder and shoulder joints were relatively higher than those of the others. In the lower limb, concordance rates of investigation were relatively low at all areas. The right lower limb was relatively higher than the left in concordance rates of the questionnaire. Conclusions : The results suggest that electrical detection of ear acupuncture points can be used in the diagnosis and treatment of musculoskeletal pain.

  • Objectives This study aimed to assessing the performance of Sasang constitutional analysis Tool (SCAT) for Chinese as one of method to diagnose Sasang Constitution and evaluating the possibility of applying SCAT to foreign people. Methods This cross-sectional study was approved by IRB (Independent Review Board) at J Hospital in Yanbian. Informed consent to take part in the study was obtained from each of the participants. The performance of SCAT was assessed through Kappa coefficient and the concordance rate between SCAT and expert diagnosis. Results The concordance rate between SCAT and expert was 61.1% in total and the Kappa was 0.408. When the constitutional probability increased, the concordance rate and Kappa showed a increasing tendency. The concordance rate of Chosun race was 62.1% and others 60.7%. Conclusions SCAT, as Sasang constitutional diagnostic supporting system, may help the expert to diagnose Sasang constitution in Yanbian region.

  • To develop the Korean Standard Differentiation of the Symptoms and sings, We investigate concordance rate of symptoms between Korean Medicine Doctors. Two Korean medicine doctor surveyed symptoms with the Korean Standard Differentiation of the symptoms and sings for the Stroke(KSDS) case report form in stroke patients within 1 month of onset. The concordance rate of inspection, auscultation and olfaction, inquiry, and pulse feeling and palpitation are respectively 0.79, 0.90, 0.91,0.80. The study of inspection, pulse feeling and palpitation will be continued to evaluate concordance rate.

A Scheme for Filtering SNPs Imputed in 8,842 Korean Individuals Based on the International HapMap Project Data

  • Genome-wide association (GWA) studies may benefit from the inclusion of imputed SNPs into their dataset. Due to its predictive nature, the imputation process is typically not perfect. Thus, it would be desirable to develop a scheme for filtering out the imputed SNPs by maximizing the concordance with the observed genotypes. We report such a scheme, which is based on the combination of several parameters that are calculated by PLINK, a popular GWA analysis software program. We imputed the genotypes of 8,842 Korean individuals, based on approximately 2 million SNP genotypes of the CHB+JPT panel in the International HapMap Project Phase II data, complementing the 352k SNPs in the original Affymetrix 5.0 dataset. A total of 333,418 SNPs were found in both datasets, with a median concordance rate of 98.7%. The concordance rates were calculated at different ranges of parameters, such as the number of proxy SNPs (NPRX), the fraction of successfully imputed individuals (IMPUTED), and the information content (INFO). The poor concordance that was observed at the lower values of the parameters allowed us to develop an optimal combination of the cutoffs (IMPUTED${\geq}$0.9 and INFO${\geq}$0.9). A total of 1,026,596 SNPs passed the cutoff, of which 94,364 were found in both datasets and had 99.4% median concordance. This study illustrates a conservative scheme for filtering imputed SNPs that would be useful in GWA studies.

A Study on Comparative Analysis of Mammography and Tc-99m MIBI Scintimammography for Dense Breast (치밀형 유방에서 유방특이감마영상검사의 유용성 평가)

  • Purpose: This study was to evaluate usefulness of $^{99m}Tc$-MIBI scintimammography for dense breast by comparing concordance in test results between Tc-99m MIBI scintimammography and mammography whose effect was proved the most as an imaging tool depending on breast density and at the same time by examining limitation on evaluation depending on density of breast tissue. [Materials and Methods] In the period from December 2010 to July 2011, this study targeted 150 patients who took both of $^{99m}Tc$-MIBI scintimammography and mammography conducted by using breast gamma camera in this hospital. Breast density was classified to the four levels of pattern 1~4 based on the results of mammography. $^{99m}Tc$-MIBI scintimammography was conducted with the LCC, the RCC, the LMLO, and the RML one minute after intravenous injection of 99mTc-MIBI 7400 MBq (20 mCi) while analysis was made for concordance in test results of $^{99m}Tc$-MIBI scintimammography and mammography. [Results] Among the 150 patients, pattern 1 was found in 3 patients, pattern 2 in 44 patients, pattern 3 in 61 patients, and pattern 4 in 37 patients. There were 5 patients who showed the case where it was impossible to determine density of breast tissue due to foreign body inserted to breast. The concordance ratio of the results between $^{99m}Tc$-MIBI scintimammography and mammography was 95.5% for pattern 2, 95.1% for pattern 3 and 94.6% for pattern 4. This demonstrated that the concordance rate decreased according to the increase in breast density. [Conclusion] When there was limitation on evaluation of breast specific gamma imaging test results due to increased intake in breast tissue or surgical site, the concordance rate was 6.8% for pattern 2, 16.3% for pattern 3 and 18.9% for pattern 4. This demonstrated that the degree of limitation on evaluation of breast specific gamma imaging test results increased according to the increase in breast density.

COMPARISON BETWEEN INTRAORAL AND PANORAMIC RADIOGRAPHS IN THE EVALUATION OF ALVEOLAR BONE LOSS (치조골소실의 평가에 있어서 구내 X선사진과 파노라마 X선사진의 비교)

  • The purpose of this study was to compare the interpretation results of the panoramic radiographs with those of intraoral radiographs in evaluation of alveolar bone loss. All radiographs were obtained from 100 patients who had visited the Dental Infirmary of Kyungpook National University Hospital for periodontal state evaluation. The results were as follows: The percentages of interpretable sites were 95.9%(94.6% in maxilla, 97.4% in mandible) on the intraoral radiographs, and 90.7%(84.0% in maxilla, 97.3% in mandible) on the panoramic radiographs. The concordance of interpretation scores of marginal bone loss between intraoral and panoramic radiographs was 66.3%(65.6% in maxilla, 66.8% in mandible). And according to the site, the highest concordance was in the distal surface of the mandibular 2nd premolar at 82.0%, and the distal surface of the mandibular 1st premolar(76.8%), the distal surface of the maxillary central incisor(75.8%), the mesial surface of the 2nd premolar (75.0%) in descending order of frequency. According to the interpretation scores of the marginal bone loss, the percentages of concordance between intraoral and panoramic radiographs were the highest on the score 10 at 76.4%, and the lowest on the score 8, 9. And the percentages of concordance were inverse proportional rate from the score 5 to the score 9. Number of the observed sites of the furcation involvement in bitewing and panoramic radiographs were 268 sites, and the percentage of interpretable sites was 92.9% in bitewing radiographs and 86.6% in panoramic radiographs. And the concordance rate of interpretation was 79.5%.

Significant Variables Affecting Follow-up after Psychiatric Consultation in General Medical Units (정신과 자문이 의뢰된 내과 환자들의 추적치료 결정요인)

  • This study was performed to identify the significant variables affecting Follow-up after psychiatric consultation in General medical units and the trends of psychiatric consultation at the Hallym University hospital in Chunchon. Subjects of this study were 107 patients from April 1, 1998 to August 31, 1998, who were referred for psychiatric consultation during hospitalization. The results were as follows ; 1) Referral rate of General medical units was 7.5% and ordered Gastroenterology, Cardiology, nephrology, Other part. 2) The rate of total follow-up was 58% and the rate of No-follow-up was 42%. 3) In the cases of referral timing over admission 8th days, the days hospitalized after consultation performed were lengthened, significantly. 4) Common reasons for no follow-up of consultation were 'early- discharge' and 'patient's unwillingness to psychiatric reconsult. 5) Significant variables affecting follow-up were 'treatment recommendation', 'concordance with psychiatric drug recommendation', 'concordance with laboratory recommendation', 'total days hospitalized', 'Days hospitalization after consult was performed' and 'referral reason'.

Comparison between Diagnostic Results of the Sasang Constitutional Analysis Tool (SCAT) and a Sasang Constitution Expert (체질점수 복용첩수 내원회수 연령에 따른 사상체질진단툴(SCAT)과 전문가의 체질진단 일치도 연구)

  • Objectives Recently, Sasang Constitutional Analysis Tool (SCAT) was developed using face, body shape, questionnaire, and voice information. The purpose of this paper is to analysis the characteristics of SCAT in detail for the subjects whose Sasang Constitutional (SC) type was confirmed clearly. Methods 1) The SC types of 108 patients were diagnosed by an expert of Sasang constitutional medicine through the observation of improvements after taking more than 20 packs of herbal medicine prescribed 2) The diagnostic results of SCAT were acquired for the patients. 3) Comparison between diagnostic results of the SCAT and expert was performed according to SC probability difference, the number of packs of herbal medicine prescribed, the number of hospital visits, and age. Results 1) The concordance rate was 65.9% and 59.4% in the male and female groups, respectively. 2) Overall, the concordance rate increased as the SC probability difference increased. In case of more than 5% SC probability difference, it increased up to 71.0% and 66.7% in the male and female groups, respectively. 3) The concordance rate did not show any trend as the number of packs of herbal medicine prescribed and the number of hospital visits increased. 4) The higher concordance rate was observed in Taeeumin, Soyangin, and 20s ~ 40s groups of male patients, and in Soyangin, 20s, and 40s groups of female patients. Conclusions The strong and weak points of SCAT were observed and they will contribute to the improvement of SCAT performance.
