A Research on Exterior Heat Syndrome Diarrhea of Taeumin(太陰人) (태음인(太陰人) 표열증(表熱證) 설사(泄瀉)에 대한 고찰)

  • Objectives : The first object is to reveal the mechanism of diarrhea based on exterior heat syndrome for Taeumin as explained in Donguisusebowon's Taeeumingansuyeoliyeolbyeonglon. The second objective is to review the treatment for Taeumin as explained in Leejema's Galgeunnabogjatang because the drug composition of Galgeunnabogjatang, the drug that treats this syndrome, is not written. Methods : As the first step, the paper will review the disease pathology for Taeumin written, and check the existence of disease path that can be viewed as exterior heat syndrome. As the second step, the paper will review the probability of exterior heat syndrome deriving from Taeumin disease path and morph into the exterior heat syndrome. Results : Using the first step to discover the direct correlation between exterior heat syndrome and the theory of disease for Taeumin led to a failure, but through the second step, the paper was able to confirm the probability that exterior heat syndrome could derive from Wiwansuhanpyohanbyeong's Wiwanhanjeung. Based on this, the paper was able to determine the Galgeunnabogjatang suitable for the treatment of exterior heat syndrome. Conclusions : Exterior heat syndrome is created when healthy qi is stimulated to quickly build up the pathogen of Taeumin exterior heat syndrome, but only exterior heat is generated and the cold remains. Its main symptoms are fever, excessive sweating, and difficult painful red-urination. Exterior heat syndrome diarrhea refers to the diarrhea which is generated from the Taeumin's disease pathology. The paper determined that Galgeunnabogjatang, which is listed in Donguisusebowon the seventh edition Boyubang.

  • 1. Backgrounds: There are a lack of comprehensive study on Taeeumin Exterior Heat syndrome diarrhea(表熱證泄瀉). Thus, The author examines the content of various literatures related to Tae-eumin Exterior Heat syndrome diarrhea. 2. Methods: Literature examination is used Lee, Je-ma's writings and Sasang Constitutional Medical doctor's writings. Syndrome examination is used Tae-eumin's study on Syndrome of DonguisusebowonGaboGu-bon("咸山沙村東醫壽世保元甲午舊本") and Donguisusebowon("東醫壽世保元") and treatises of the academic world. 3. Results: and Conclusions: 1) "Profuse sweating and reddish and difficult urination"(汗多而小便赤澁) is regard as Taeeumin Interior Febrile Disease(裏熱病). 2) There is no possibility of a misprint about Taeeumin Exterior Heat syndrome diarrhea. Taeeumin Exterior Heat syndrome diarrhea is engaged in both the Exterior and Interior(表裏俱病). 3) It is difficult to prescribe GalgeunHaegi-Tang(葛根解肌湯) instead of Galgeunnabokja-Tang(葛根蘿葍子湯). 4) Tae-eumin Exterior Heat syndrome diarrhea is formed in the early period and absorbed into Donguisusebowon ("東醫壽世保元") in the latter period. "Profuse sweating and reddish and difficult urination" is estimated to be added in the latter period.

  • Warm disease: Any of various heat disease characterizer by rapid onset and shifts, pronounced heat signs, and a tendency to form dryness and damage yin. Exterior heat sign: exterior heat patterns are characterizer by pronounced heat signs, such as a red sore pharynx and a relatively red tongue with dry fur, the pulse is floating and rapid, cough and the production of thick white or yellow phlegm. If wind-heat evil exist in weifen, it becomes exterior syndrome, and a remedy about that is dispelling wind-heat but when wind-heat evil invades in nasal and throat part so the disease occurs, you need to add relieving sore throat worsens invades in lung it makes disharmony of diffuse in lungs. So a remedy about it is diffuse the lung. disharmony of diffuse in lungs makes metabolic disorder of qi and liquid and humor malfunction therefore it occurs cough and heat-phlegm syndrome. heat from weifen invades the whole of lungs and form lung heat. So a remedy about lung heat is clearing away lung heat, this lung heat makes inevitably bleed in lungs, therefore a remedy in this case is clearing the lung to stop bleeding, or moistening the lung. Exterior heat sign means that exterior syndrome coexists with heat syndrome and it means that a remedy of this syndrome need to mix prescriptions for relieving exterior syndrome and heat-clearing prescriptions to treat this syndrome.

  • The causes of disease as well as the mechanisms that result in the presenting signs and symptoms of patients are basic elements in oriental medicine. The location of the disease is the region on which the cause and the nature of the disease acts. Syndrome reflects the cause of the disease and the pathological change and the location of the disease. It is customary to classify the location of the disease into two major categories. First is based on the exterior and interior and the second is based on the five internal organs. Exterior syndrome is caused by the invasion of exopathogen in skin and hair, channels and collaterals. Interior syndrome refers to the syndrome located deeply in the interior of the body such as five internal organs. The location of the disease is very important to distinguish different syndromes in the diagnosis and the treatment of a disease.

  • In "Sanghanlon(傷寒論)", there are several articles to explain about Oryeongsan's syndrome. Most important articles of them are No. 72 and No. 75. Oryeongsan is explained by many doctors as a prescription to control exterior and interior, because they considered that symptoms of 72 and 75 articles were caused by exterior and interior's syndrome. Accordingly, they explained that RAMULUS CINNAMOMI(桂枝) and warm water remove exterior pathogens, and PORIA(茯笭) and others remove interior fluid retention. But considering the origin of a fluid retention, RAMULUS CINNAMOMI's effects are rather activating Yang(通陽) and warming Yang(溫陽) than removing exterior pathogens. The fluid retention is cauesd by insufficiency of Yang energy, because body fluid's spreading depends on Ynag energy. RAMULUS CINNAMOMI's effects of activating Yang and warming Yang improve the fluid retention and spread body fluid to the whole body. Some doctors use CORTEX CINNAMOMI(肉桂) instead of RAMULUS CINNAMOMI, because CORTEX CINNAMOMI's effects of activating Yang and warming Yang are better than RAMULUS CINNAMOMI's. There are many opinions about what is the main symptom of Oryeongsan's syndrome, but the difficulty in urination is understood by most doctors as the main symptom of Oryeongsan's syndrome. From the viewpoint of that, they understand that Oryeongsan's main effect is the induce diuresis. But the induce diuresis does not only mean a urination. The induce diuresis must be understood as the removing fluid retention by sweat and urine through the spreading body fluid.

  • 1. Objectives: This research was proposed to reinterpret "Soeumin Bumron(少陰人 泛論)"through development of Soeumin exterior disease in Sasang Constitutional Medicine(SCM). 2. Methods: We chronically compared the conceptions, symptoms, mechanisms and treatments of Soeumin exterior disease from "Donguisusebowon-GabObon(東醫壽世保元甲午本)"(DGO) to " "Donguisusebowon-ShinChukbon(東醫壽世保元辛丑本)"(DSC) 3. Results and Conclusions: (1) The classification standard of Soeumin exterior disease was different in DGO and DSC. (2) Soeumin exterior disease classified Taeyang Disease(太陽病), Yangmyoung Disease(陽明病) in DGO and Ulkwang-syndrome(鬱狂證), Mangyang-syndrome(亡陽證) in DSC. (3) Based on the changes of understanding about Soeumin exterior disease from DGO to DSC, we attempted to reinterpret "Soeumin Bumron(少陰人 泛論)" in view of DSC. In view of DSC, the classification standard of Soeumin exterior disease focused on Warm Yang Qi(陽煖之氣), Healthy energy(保命之主) of Soeumin.

A Study about the Distributional Characteristics of Dyspepsia according to Sasang Constitution at a Korean Medical Clinic (일개 한의원에 내원한 소화불량 환자의 사상인 분포에 관한 연구)

  • 1. Objectives: The purpose of this study was to find out the distributional characteristics of dyspepsia according to Sasang Constitution. 2. Methods: The medical records of 108 subjects who have received prescriptions and diagnosis of Sasang constitution from June 2007 to April 2009 at a Korean Medical clinic in Seoul were reviewed. General factors and the frequency of the prescriptions according to Sasang constitution are compared and analyzed. 3. Results: Among the 108 subjects, 57(52.78%) persons were Soyangin, 27(25.00%) Taeumin, and 24(22.22%) Soeumin. The frequencies of the prescriptions of interior and exterior disease were compared. Exterior disease(89.47%) was higher than interior disease in Soyangin and interior disease was higher than exterior disease in Taeumin(88.89%) and Soeumin(91.67%). (p-value<0.001) The most common disease was Mangeum-syndrome(73.68%) of exterior disease in Soyangin, Dry-febrile symptom(88.89%) of interior disease in Taeumin and Taeum-syndrome(66.66%) of interior disease in Soeumin. 4. Conclusion: There were significant differences on the distribution of dyspepsia in interior or exterior disease according to Sasang Constitution.

A Study on the Absence of the Zhuniao decoction among the Four Directions Sishen decoction in 『Shanghan Lun』 (『상한론(傷寒論)』의 사방(四方)·사신탕(四神湯) 중(中) 주조탕(朱鳥湯)의 부재(不在)에 관한 고찰)

  • Among the ancient ideas of East Asia, there is a myth about the symbol of the Four Gods who commands all directions in the east, south, west, and north, and this is also reflected in the medical field. However, although there is discussion about Qinglong Decoction (青龍湯), Baihu Decoction (白虎湯) and Xuanwu Decoction (玄武湯) in the 『Shanghan Lun (傷寒論)』, there is no content focusing on Zhuniao decoction (朱鳥湯). Considering the consistent perspective of 『Shanghan Lun』, which recognizes the disease-syndrome and seeks prescriptions based on yin and yang thinking, this is very unusual. Therefore, in this study, it was revealed that the concept of 'Sishen Decoction (四神湯)' itself was a concept that emerged when the authors of 『Shanghan Lun』 established the 'eight-principles syndrome differentiation (八綱辨證)' system in their recognition and response to diseases-syndromes. Based on this, I considered the following possibilities: They were able to present Qinglong decoction, Baihu Decoction and Xuanwu decoction, as appropriate prescriptions for 'exterior cold excess syndrome (表寒實證)', 'interior heat deficiency syndrome (裏熱虛證)', and 'interior cold deficiency syndrome (裏熱實證)'. However, it is possible that the name of the prescription 'Zhuniao decoction' was not intentionally used, because it was not possible to provide an appropriate prescription for 'exterior cold excess syndrome (表熱實證)'.

  • Objectives : Through the study on the complex efficacy of Sosihotang(小柴胡湯), it would be expected to comprehend the concept of diseases and syndromes that will be treated and principle of composing formula. Methods : It has been done to compare and analyse provisions of Shanghanlun related with Sosihotang(小柴胡湯). Results : Sosihotang(小柴胡湯) is fit to treat symptoms based on diseases having half exterior and half interior, deficiency and excess syndromes, because it is well-composed to focus on Soyangbyeong(少陽病) and Sosihotang(小柴胡湯) syndrome caused from lose of homeostasis in human body. Conclusions : Sosihotang(小柴胡湯) is one of the multipurpose formula that can be used to treat the syndromes and diseases of exterior and interionr(表裏), cold and heat(寒熱), deficiency and excess(虛實). Therefore it is necessary to research and develop the formula with the type.

  • External pathogens such as wind, cold can easily invade the external parts of the body when host's external defense ability is not secure. Herein, we consider the underlying mechanisms against the external contraction at the body surface. During the early period after primary invasion, external defense mechanisms are gradually activated. The classic clinical manifestations are aversion to cold, fever, headache, generalized pain, and nasal congestion. This condition is called by invasion of external pathogen into the body surface. As the disease progress, lung qi is stagnated and thereby up-outward and downward movement action of lung become disturbed. Therefore, when doctor administrate formula to treat the exterior syndrome, doctor must keep in mind not only materia medica, but also underlying mechanisms through which many clinical symptoms appear.
