A Case Report of Panic Disorder with Functional Gastrointestinal Disturbance (기능성 위장장애를 동반한 공황장애 환자 치험 1례)

  • Objectives : This study was performed to report the effect of oriental treatments for panic disorder with functional gastrointestinal disturbance. Methods : In this case, a 30 year-old female patient has been suffered by panic disorder with functional gastrointestinal disturbance for 10 years. This case can be caused by KanKiUlKyul(肝氣鬱結), KanKiBeomYi(肝氣犯胃) and KanKiSeungBi(肝氣乘脾). We treated the patient with Herbal medication, Cognitive-behavioral therapy and Relaxation therapy. The effects of treatment were measured by GSRS(Gastrointestinal Symptom Rating Scale), BPI(Brief Pain Inventory) and VAS(Visual Analog Scale) Results : As a result of oriental treatments, the symptoms of Panic Disorder with Functional Gastrointestinal Disturbance was improved. Conclusions : These results suggest that oriental treatments have an effect on Panic Disorder with Functional Gastrointestinal Disturbance.

  • Objectives : The purpose of this study is to present two case reports on the treatments of psoriasis who has functional gastrointestinal disorder.Methods : We administered herbal medicine to two patients who showed psoriasis with functional gastrointestinal disorder and evaluated the results by Psoriasis Area and Severity Index(PASI).Results : After the treatment two patients showed improvements in PASI. PASI of patient 1 changed from 8.8 to 0.8; patient 2 from 7.0 to 0.2Conclusions : The results suggest that herbal medicine can be an effective treatment for psoriasis who has functional gastrointestinal disorder.

  • Although dysphagia and globus is a common comlication in Functional gastrointestinal disorder, there are few studies or reports about its treatment in oriental medicine. This study was to report the effect of Bokryungyeum-tang in Functional gastrointestinal disorder patients with dysphagia. Functional gastrointestinal disorder patients with dysphagia hospitalized in our hospital were treated by Bokryungyeum-tang every day. three times a day for two weeks. The patient showed constant improvement during the two weeks. Not only their objective measure, but also their subjective symptoms such as chest discomfort, insomnia, and depression improved too. This suggests that Bokryungyeum-tang is quite effective when treating functional gastrointestinal disorder patients with dysphagia.

  • Objectives: Since we found patients with dyspepsia as their major symptom had high rate of headache, would like to find out a rational diagnosis ground by using EAV correlation between headache and gastrointestinal disorder Methods: We did survey and EAV inspection on 128 patients came by gastrointestinal disorder who quoted 4 and 5 degree for their headache symptoms) Results & Conclusion The results of this study were as follows: 1. From dyspepsia, we saw higher rate by order of epigastric discomfort, bloating, stomach trouble, bleching, etc. 2. By EAV analysis, found functional deterioration by order of stomach, nerval degeneration vessel, gall bladder/bile duct, liver. 3. By EAV analysis, functional deterioration of stomach, gall bladder/bile duct, liver represent dyspepsia and nerval degeneration vessel represent headache. We found the correlation between dyspepsia and headache by observation at a time.

  • In patients with functional gastrointestinal disorders, dietary factors have an important effect on the development or worsening of gastrointestinal symptoms. Therefore, nutritional advice is often needed for the treatment of these patients. Although no firm conclusions can be drawn from current studies, the use of low-fat diets can be recommended in patients with functional dyspepsia. Constipation can be treated with a high-fiber diet. There are few studies in irritable bowel syndrome patients with diarrhea. The diet recommendations must be applied to each patient depending on symptoms.

  • Objectives The purpose of this study is to establish relationship between functional gastrointestinal disorders in children and Temperament and Character using objective criteria and questionnaires. Methods This study surveyed 3-6 years old 130 children who were attending the kindergarten or daycare center in the vicinity of Gangdong from May 15th, 2014 to November 30th, 2014. The Korean-translated Questionnaire on Pediatric Gastrointestinal Symptoms-Rome III (QPGS-Rome III) and The Junior Temperament and Character Inventory 3-6 (JTCI 3-6) questionnaires were used. Results 1. Between the groups, functional gastrointestinal disorder group scored significantly higher on NS, NS1, NS4, HA4, ST and ST1. 2. As the number of functional gastrointestinal disorders increase, the group scored higher on NS, NS1, NS4, HA4, ST and ST1. Conclusions This study showed functional gastrointestinal disorder group exhibited higher NS, NS1, NS4, HA4, ST and ST1 scores.

Banhasasim-tang Administration for Treatment of Functional Gastrointestinal Disorders after Motor Vehicle Accidents: Case Series (교통사고 이후 발생한 기능성 위장관장애에 대한 반하사심탕 투여 : 연속증례)

  • Objectives: These cases report the benefits of administration of Banhasasim-tang extracts or decoctions to patients with functional gastrointestinal disorders after motor vehicle accidents. Methods: The patients were treated with Banhasasim-tang extract or decoction three times daily. History-taking and imaging tests were used to differentiate other diseases. We evaluated the patients using the Gastrointestinal Symptom Rating Scale (GSRS), which is used to assess overall gastrointestinal symptoms. Results: Treatment with herbal medicine resulted in a decrease in the patients' complaints of symptoms. The GSRS scale showed improvement in all four cases. Conclusions: Banhasasim-tang, regardless of its form, has beneficial effects in patients with functional gastrointestinal disorders after motor vehicle accidents.

  • The pathophysiology of functional gastrointestinal disorder(FGID) is not completely understood, but the importance of the 'Brain-Gut Axis(BGA)' model in FGID is being increasingly recognized. The BGA model is a bidirectional, hard-wired and homeostatic relationship between the central nervous system(CNS) and the enteric nervous system(ENS) via neural, neurohormonal and neuroimmunological pathways. In addition, the BGA model would provide a rationale for the use of psychotropics on FGID. The authors experienced two cases in which duloxetine, a serotonin-norepinephrine reuptake inhibitor, was effective in relieving FGID symptoms as well as psychiatric symptoms such as depression and hypochondriacal anxiety. Therefore we discuss the vignettes from the perspective of BGA theory. Duloxetine showed efficacy in these two patients by reducing visceral hypersensivity (bottom-up regulation) and by relieving depression and anxiety(top-down regulation).

Characteristics of functional gastrointestinal disorders in children with chronic abdominal pain (소아의 만성 복통에서 기능성 위장관 질환의 양상)

  • Purpose : The aim of this study was to document the causes of chronic abdominal pain in children referred to a hospital setting and evaluate the frequency and characteristics of functional gastrointestinal disorder (FGID) classified by Rome III criteria. Methods : One hundred thirty two patients with chronic abdominal pain were evaluated. Examinations were performed in order to find organic causes in patients when organic disease was suspected. Results : Among the 132 patients, 20 patients (15.2%) had organic diseases and 112 patients (84.8%) were diagnosed as having FGIDs. Functional dyspepsia was the most common cause of FGIDs, followed by irritable bowel syndrome. Overlap of some FGIDs was observed in seven patients (5.3%). Conclusion : FGIDs are the main causes of chronic abdominal pain in children and functional dyspepsia was the most prevalent disorder.

  • Chronic abdominal pain, defined as long-lasting intermittent or constant abdominal pain, is a common pediatric problem encountered by primary care physicians and medical subspecialists. Chronic abdominal pain in children is usually functional, i.e., without objective evidence of an underlying organic disorder. Functional abdominal pain is categorized as functional dyspepsia, irritable bowel syndrome, functional abdominal pain, abdominal migraine, and aerophagia according to the Rome II criteria for pediatric functional gastrointestinal disorders. There is insufficient evidence to state that the nature of abdominal pain or the presence of associated symptoms can discriminate between functional and organic disorders. The presence of alarming symptoms or signs, such as weight loss, gastrointestinal bleeding, persistent fever, and chronic severe diarrhea, is associated with a higher prevalence of organic disease. Most children with chronic abdominal pain are unlikely to require diagnostic testing; such children often need pharmacologic and behavioral therapy.
