Somatostatin Receptors 3, 4 and 5 Play Important Roles in Gallbladder Cancer

  • Expression changes of somatostatin receptor subtypes (SSTRs) including SSTR1, SSTR2, SSTR3, SSTR4 and SSTR5 in the development of gallbladder cancer were assessed with attention to relationships with clinical pathological characteristics. SSTRs in 29 gallbladder cancer and 25 normal gallbladder tissue specimens were examined by immunohistochemical staining. Differences between SSTRs expressions and clinical pathological parameters were analyzed by chi-square test. The five subtypes of SSTR were all expressed in gallbladder cancer tissues and SSTR3 presented the highest expression. SSTR5 expression was increased significantly in gallbladder cancer (P<0.05) compared with that in normal gallbladder tissue. SSTR3 expression in highly and moderately differentiated gallbladder cancer was significantly higher than that in poorly differentiated lesions (P<0.05). SSTR4 expression was lower in gallbladder cancer with lymph node metastasis than that in gallbladder cancer without lymph node metastasis (P<0.05). Therfore, these results indicated that SSRT5, SSTR3 and SSTR4 may play important roles in the formation and development of gallbladder cancer.

Delayed Clinical Symptoms of Gallbladder Rupture by Gallbladder Mucocele in a Dog (담낭 점액낭종에 의한 담낭 파열의 만성 경과 증례)

  • An 11-year-old, intact male Cocker Spaniel dog was presented with history of abdominal distension, dyspnea for 10 days and lethargy for 1 day. Abdominal radiographs showed decreased serosal detail with abdominal distension. Abdominal ultrasounds revealed gallbladder mucocele with generalized peritonitis showing stellate-like sludge in the gallbladder with echogenic fat degeneration of cranial abdomen and abdominal free fluid containing echogenic materials. Loss of gallbladder wall integrity was shown clearly on computed tomography but ambiguously on ultrasound. Ultrasound-guided abdominocentesis was performed and showed amount of yellowish-bloody peritoneal fluid with vegetable matter and mucoid substance. On peritoneal fluid analysis, bilirubin level was elevated over three times than those of the serum. On exploratory laparotomy, gallbladder rupture and generalized bile peritonitis with intestinal adhesions were confirmed and cholecystectomy with peritoneal lavage was performed. One day after operation, patient died. This report describes delayed clinical symptoms of gallbladder rupture by gallbladder mucocele. In addition, this is the first case report using computed tomography made a diagnosis gallbladder rupture in a dog. Computed tomography might be helpful to diagnose gallbladder rupture.

Cholelithiasis as a Risk Factor for Gallbladder Cancer (담낭암 발생의 위험인자로서의 담석증)

  • Although the incidence of gallbladder cancer is relatively low, Korea is one of the countries with the highest incidences of gallbladder cancer in the world. Gallbladder cancer is also often diagnosed in an advanced state, and has poor prognosis. A gallstone is one of the major risk factors for gallbladder cancer and a majority of epidemiological studies support correlation between cholelithiasis and gallbladder cancer. Clear mechanism, however, regarding how gallstones cause gallbladder cancer is not known. In this article, results of studies about the relationship between gallstone and gallbladder cancer were thoroughly reviewed. Also, it was discussed whether prophylactic cholecystectomy is necessary to prevent gallbladder cancer in asymptomatic gallstone patients without other risk factors such as a gallbladder polyp or porcelain gallbladder.

Study on 'All Articulations are Controlled by the Gallbladder' in Chimgugyeongheombang (『침구경험방(鍼灸經驗方)』 '제절개속담(諸節皆屬膽)'에 대한 고찰)

  • Objectives : This paper aims to study how 'All articulations are controlled by the gallbladder' as urged by Heoim in Chimgugyeongheombang was reflected in the clinical field, and look into the reason for his assertion. Methods : The paper looked into Chimgugyeongheombang and 1) found examples of how acupoints related to gallbladder were used to treat articulation-related diseases, and 2) investigated the relation between gallbladder and articulations through analyzing the acupoints used for treating articulation-related diseases. These are then compared with Zhenjiuzishengjing to see if the assertions made in 'All articulations are controlled by the gallbladder' were actually applied in clinical fields. Results & Conclusions : Heoim dealt with 20 points out of the 44 acupoints for gallbladder, and used 14 of them for treatments of articulation-related diseases. Gallbladder acupoints were used for ten illnesses, and four of them were related to articulation. Chimgujasaenggyeong showed a difference in that it did not utilize gallbladder acupoints to treat these illnesses. Out of 33 articulation-related illnesses, gallbladder meridians and acupoints were suggested as a means to treat them in 15 cases, and this is an increase in the usage of these points compared to Chimgugyeongheombang. In other words, Heoim actively utilized gallbladder meridian points in treating articulation-related illnesses. The reason Heoim drew a line between gallbladder and articulations is because gallbladder is categorized as a lesser yang pivot and articulations itself functions as a pivot by acting as a pivot for human body movement.

Gallbladder Carcinoma: Analysis of Prognostic Factors in 132 Cases

  • Objective: To evaluate the prognostic factors of gallbladder carcinoma. Methods: Presentation, operative data, complications, and survival outcome were examined for 132 gallbladder carcinoma patients who underwent gallbladder surgery in our unit during 2002-2007, and follow-up results were obtained from every patient for univariate and multivariate survival analysis. Results: The univariate analysis showed that gallbladder lesion history, tumor cell differentiation, Nevin staging, preoperative lymph node metastasis and the surgical approach significantly correlated with the prognosis of the patients (p<0.05). The results of the multivariate analysis (Cox regression) showed that gallbladder lesion history, Nevin staging and the surgical approach were independent predicators with relative risks of 6.9, 4.4, 2.8, respectively (p=0.002, 0.003, 0.008). Conclusion: Gallbladder lesion history, Nevin staging and the surgical approach are independent prognostic factors for gallbladder carcinoma, a rapidly fatal disease. Therefore, early diagnosis, anti-infective therapy and radical surgery are greatly needed to improve the prognosis of gallbladder carcinoma.

  • Objective: A case report on the improvement of gallbladder polyps through Ortho-Cellular Nutrition Therapy Methods: On ultrasound, gallbladder polyps were detected in a 54-year-old male Korean patient. In addition to the polyps, there were also calcific tendonitis and various inflammatory conditions of the shoulder. Results: Ultrasound revealed the absence of gallbladder polyps 8 months after nutritional therapy. Conclusion: It is believed that nutritional therapy is beneficial for patients with gallbladder polyps who do not require surgical treatment and are under observation.

  • In 3 dogs with gallbladder mucocele, rupture of gallbladder was diagnosed and treated. Althoughthe diagnosis of gallbladder is challenging due to non-specific clinical signs and physical examinationfindings, mucoceles are ultrasonographically characterized by the appearance of the stellate or finely striatedbile paterns and difer from biliary sludge by the absence of gravity dependent bile movement. In thiscase, gallbladder rupture caused by mucocele can be diagnosed based on not only above characteristicsbut secondary lesion such as ascites around gallbladder and swollen hyperechoic mesentery.

Non-Operative Management of Traumatic Gallbladder Bleeding with Cystic Artery Injury: A Case Report

  • Gallbladder injuries are rare in cases of blunt abdominal trauma and are usually associated with damage to other internal organs. If the physician does not suspect gallbladder injury and check imaging studies carefully, it may be difficult to distinguish a gallbladder injury from gallbladder stone, hematoma, or bleeding. Therefore, in order not to miss the diagnosis, the clinical findings and correlation should be confirmed. In the present case, a 60-year-old male presented to a local trauma center complaining of pain in the upper right quadrant and chest wall following a motor vehicle collision. Abdominal computed tomography (CT) showed a hepatic laceration and hematoma in the parenchyma in segments 4, 5, and 6 and active bleeding in the lumen of the gallbladder. Traumatic gallbladder injuries generally require surgery, but in this case, non-operative management was possible with cautious follow-up consisting of abdominal CT and angiography with repeated physical examinations and hemodynamic monitoring in the intensive care unit.

Premalignant Lesions of Gallbladder Carcinoma and Treatment (담낭암 발생 관련 질환과 치료)

  • Carcinoma of the gallbladder is an uncommon but highly malignant tumor with a poor five year survival rate. Early detection is very important for successful treatment because this tumor is very hard to cure in cases where it has advanced beyond the reach of surgical treatment. The purpose of this review was to evaluate risk factors for carcinoma of the gallbladder and determine the best management approach.. Solitary polys, more than one cm are considered to be predisposing factors for gallbladder carcinoma when they are found to be echopenic, sessile, and with a high cell density. Anomalous union of the pacreato-biliary duct(AUPBD) without a choledochal cyst is also considered to increase risk for gallbladder carcinoma. A polyps size of more than one cm and an AUPBD are indications for prophylactic cholecystectomy. The presence of gallstones is a well-established risk factor for the development of gallbladder carcinoma; risk appears to correlate with the stone size and the duration of chronic cholecystitis. Metaplastic changes of the gallbladder epithelium present with chronic cholecystitis and may indicate a premalignant lesion. Abnormal forms of cholecystitis such as xanthogranulomatous or a porcelain gallbladder also have malignant potential; cholecystoenteric fistula as well as bacterial infection of the gallbladder(typhoid, helicobacter species) also has malignant potential. In this review, the risk factors associated with carcinoma of the gallbladder are summarized with special attention to gallstones, polyps, AUPBD, and chronic inflammation.

A Review of Porcelain Gallbladder Mimicking a Gallbladder Stone on Ultrasonography (초음파 검사에서 담석으로 오인될 수 있는 자기양담낭에 대한 고찰)

  • Porcelain gallbladder is a rare disorder by deposition of calcium in the wall of gallbladder. The chronic cholecystitis is reported to produce mural calcification and obstruction to the cystic duct by stone. Since porcelain gallbladder is commonly associatedwith gallbladder cancer, cholecystectomy is prerformed to prevent it. We report here a case of a patient with porcelain gallbladder. This case showedthe typical ultrasonogaphic, computer tomogaphic and radiaographic findings of the disease.
