The Clinical Study of Muscle Energy Technique (MET) Performed on Sternocleidomastoid Muscle of Acute Nuchal Pain Patients on Meridian Electromyography (급성 경항통 환자의 흉쇄유돌근에 대한 근 에너지 기법(MET) 시술 후의 경근전도 변화 연구)

  • Objectives To evaluate the clinical utility of MET performed on sternocleidomastoid muscle of acute nuchal pain patients, we measured a change of meridian electromyography. Methods We compared electrical activity before and after MET performed on sternocleidomastoid muscle of acute nuchal pain patients in same group (n=20) in isometric exercise state during five seconds. We analyzed amplitudes and areas of electrical activity and asymmetry index (AI). Results 1. After MET performed on sternocleidomastoid muscle of acute nuchal pain patients, maximum voluntary isomeric contraction (MVIC) was significantly increased more than before MET performed (p<0.005). 2. Comparing with before MET performed on sternocleidomastoid muscle of acute nuchal pain patients, muscle fatigue after MET performed on sternocleidomastoid muscle of acute nuchal pain patients decreased but there was no significant difference. 3. Comparing with before MET performed on sternocleidomastoid muscle of acute nuchal pain patients, asymmetry index (AI) after MET performed on sternocleidomastoid muscle of acute nuchal pain patients decreased but there was no significant difference. Conclusions According to above results, after performing MET on sternocleidomastoid muscle of acute nuchal pain patients, maximum voluntary isomeric contraction (MVIC) increased significantly, so it is certain that performing MET on sternocleidomastoid muscle has a clinical effect.

  • Objectives : The studies on variation in diagnosis on the neck and nuchal pain has not been done thoroughly as we can use it in clinical practice of these days. For this reason, I examined the variation in diagnosis mentioned in the classics of Oriental Medicine as the preceding study on standardization of variation in diagnosis on neck and nuchal pain. Methods : I gathered the twenty kinds of classics of Oriental Medicine that were computerized, the textbooks on Oriental Medicine which are being used these days, and the theses on current clinical research. After gathering these data, I analyzed these according to the variation in diagnosis. Results : The classics of Oriental Medicine on the neck and nuchal pain mentioned very much about the neck and nuchal pain occurred by the pathogenic factor of Wind, Cold, and Dampness, disharmony created by deficiency of Liver and Kidney, and pathogenic state of Meridians of Taiyang. According to the texts of these days, the differentiation of syndromes can be divided into four kinds of items such as Wind-Cold pathogen, Wind-Dampness pathogen, Phelgm-Heat, and disharmony between Qi and Blood. The theses of these days rarely mentioned about variation in diagnosis on the neck and nuchal pain. Conclusions : The differentiation of syndromes on the neck and nuchal pain can be divided into four kinds items as affection by exopathogen like Wind, Cold, Dampness, Heat, and so on, stagnation of Qi and the coagulation blood, deficient syndrome of Liver and Kidney, and deficient syndrome of Qi and Blood.

Systematic Review of Selection of Acupoints in Domestic Clinical Studies Related to Nuchal Pain (국내 경항통 연구에서의 경혈 선정에 관한 계통적 연구)

  • The purpose of this study is to figure out which acupoints are selected to treat nuchal pain and attempt to analyze the meaning of those acupoints in current clinical trials. We searched ten electronic databases and manually checked related Korean journals and references. We included randomized controlled trials(RCT), clinical controlled trials(CCT), case reports/series, and literature reviews using acupojnts to treat nuchal pain. We investigated the frequency of selected acupoints. We included 31 articles in this study. The most frequently adopted acupoints were GB20(77.41%), followed by GB21(61.29%). In distribution according to meridian system, BL was the most(25.60%), followed by Extra points of Dong-Shi(12.19%). Selected acupoints for treat nuchal pain were considered to be affected by flow of meridian system. It is necessary for more studies to provide clinical guideline of acupoint selection.

  • Objectives : The purpose of this study is to investigate the clinical application of chuna for temporomandibular joint(TMJ) in nuchal pain patients with temporomandibular joint disorder(TMD). Methods : Four patients were treated by chuna for TMJ to evaluate the effect of the treatment. The patient's symptoms were assessed by visual analogue scale(VAS), neck disability index(NDI), cervical lordotic curvature. Results : In all case, the pain was reduced according to VAS, NDI. cervical lordotic curvature of three cases were improved. Conclusions : These results suggest that chuna for TMJ might be an effective method to treat nuchal pain with TMD. But, it's necessary to have more observations and experiments.

The Clinical Studies for the effects of MyoFascial Releasement on Patients with Nuchal Pain Caused by Traffic Accidents (교통사고로 유발된 경항통 환자의 근막이완술의 효과에 대한 임상적 연구)

  • Objectives : The purpose of this study is to find out the effects of myofascial releasement on nuchal pain caused by traffic accidents. Methods : The 30 patients were divided into 2 groups : group A was treated except myofascial releasement and group B was treated with myofascial releasement. Both group were treated with acupuncture treatment, physical theraphy and herbal medication. Patients were evaluated by McGill Pain Questionnaire-Short Form(SF-MPQ), Visual Analogue Scale(VAS), Pain Disability Index(PDI) and Neck Disability Index(NDI). Results : 1. Both Group were significantly decreased in affective of SF-MPQ, VAS, after 10 times of treatment. 2. Group B compared with the Group A was significantly decreased in affective of SF-MPQ, NDI and PDI after 10 times of treatment. Conclusions : We found out that myofascial releasement is considered to be effective and useful on nuchal pain caused by traffic accidents.

  • Objectives : The purpose of this study is to find out the effects of muscle energy techniques on nuchal pain caused by traffic accidents. The Effects of Chuna for Thoracic in Patients with Nuchal Pain, Seven Cases Report (흉추의 추나요법을 시행한 경항통 환자 치험 7례 보고)

  • Objectives The purpose of this study is to investigate the clinical application of chuna for thoracic in the patients with nuchal pain. Methods Seven patients were treated by chuna for thoracic to evaluate the effect of the treatment. The patient's symptoms were assessed by visual analogue scale (VAS), neck disability index (NDI) and cervical lordotic curvature. Results In all cases, the pain was reduced according to VAS, NDI. Cervical lordotic curvature of 6 cases were improved in terms of Jackson's angle. 5 cases were improved in terms of Depth of cervical curve and Method of Jochumsen. 4 cases were improved in terms of Angle of cervical curve (C2~C7) and Ishihara index. 3 cases were improved in terms of Angle of cervical curve (C1~C7). Conclusions These results suggest that chuna on thoracic might be an effective method to treat nuchal pain with extension malposition of thoracic. But, it's necessary to have more observations and experiments.

  • Objectives : The purpose of this study is to find out the effects of kinesio taping on nuchal pain caused by traffic accidents. Methods : The 40 patients were divided into 2 groups : Group A was treated except taping and Group B was treated with taping. Both Group were treated with Acupuncture, Phy-Tx and Herb-med. Patients were evaluated and analyzed Visual Analog Scale(VAS), Pain Rating Score(PRS) and Neck Disability Index(NDI). Results : 1. Group B was significantly decreased in VAS, NDI after 3 days of treatment. 2. Group B was significantly decreased in PRS after 6 days of treatment. 3. Group A was significantly decreased in VAS, PRS and NDI after 9 days of treatment. 4. Group B compared with the Group A was decreased in VAS and NDI after 12 days of treatment. 5. Group B compared with the Group A was significantly decreased in PRS after 9 days of treatment. Conclusions : We found out that kinesio taping treatment is considered to be effective and useful on nuchal pain caused by traffic accidents.

  • The superiority of air versus saline for identifying the epidural space remains unestablished. Epidural anesthesia using a loss of resistance technique (LORT) with air is associated with increasing complications of dural puncture-induced headaches and neurological and hemodynamic changes. Here, we described a case of pneumocephalus with a large amount of air that was accompanied by severe headache and nuchal and chest pain occurring after epidural block using LORT with air for combined spinal-epidural anesthesia.

Clinical Study of the Effects of Gwibi-tang and Danggwisusan on Traffic Accident Patients with Acute Cervicolumbar Disorder (급성 경·요추 염좌로 진단된 교통사고 환자에서 귀비탕(歸脾湯)과 당귀수산(當歸鬚散)의 효과에 대한 임상 연구)

  • Objectives This study was designed to compare the effects Gwibi-tang herbal-medication with Danggwisusan herbal-medication for patients with acute cervical and lumbar sprains due to a traffic accident. Methods This study was carried out on 119 acute cervical & lumbar sprain patients by traffic accident. We divided these patients into 2 groups. Group A was treated with Gwibi-tang and Group B was treated with Danggwisusan three times a day for three days. Each patient was treated with acupuncture, physical therapy and two types of herbal-medication. We measured the numerical rating scale (NRS) at two time points : pre-treatment (Tx). and post-Tx. Results 1. The NRS scores significantly decreased after treatment in all patients. 2. The changed NRS scores of nuchal pain showed a borderline significant difference between Group A and Group B (p=0.067). 3. In sub-group analysis, Gwibi-tang showed a greater effect on NRS scores of lumbago pain in female patients (p=0.068) and patients under 30s (p=0.054) compared to Danggwisusan. Conclusions In this study, both types of herbal-medication; Gwibi-tang and Danggwisusan were highly effective on acute cervical and lumbar sprain patients. Especially Gwibi-tang (Group A) was more effective than Danggwisusan (Group B) in reducing the initial nuchal pain. Also Gwibi-tang was more effective for lumbago pain in female and patients under 30s.
