Effect of Nutrition Counseling by Nutrition Care Process on Diet Therapy Practice and Glycemic Control in Type 2 Diabetic Patients (영양관리과정에 의한 영양상담이 제 2형 당뇨병 환자의 식사요법 실천과 혈당 조절에 미치는 영향)

  • Objectives: This study examined the effects of nutrition counseling by the nutrition care process (NCP) on diet therapy practice and glycemic control in patients with type 2 diabetes mellitus. Methods: The survey was conducted on 49 patients whose hemoglobin A1c (HbA1c) level ranged from 6.5% to below 10% among patients aged 30~60s with type 2 diabetes mellitus. Nutrition counseling by the NCP process was carried out twice: first nutrition counseling and follow up counseling. The questionnaires were composed of 54 questions in five fields (general characteristics, health-related behaviors, diet therapy-related items, dietary life, diet therapy-related knowledge, diet therapy-related barriers). Nutrition intervention in nutrition counseling was performed based on the individualized diagnosis of NCP. Results: All the subjects practiced self-monitoring of their blood glucose levels, regular exercise, and diet therapy after NCP-based nutrition counseling. Diet therapy-related knowledge and practice by the subjects were improved after nutrition counseling. While the intake of boiled white rice decreased, the intake of boiled brown rice and barley rice in the subjects increased significantly. After nutrition counseling, the weight and HbA1c of the subjects decreased. Conclusions: These results suggest that personalized nutrition counseling by NCP process is effective for diet therapy compliance and glycemic control of type 2 diabetic patients.

The effect of interventions in implementation of nutrition therapy for malnourished inpatients (영양 위험군 입원환자에 대한 영양치료 수행 중재 효과)

  • Objectives: Several studies on hospital malnutrition have reported that malnutrition among patients is highly prevalent and that more than 40% of hospitalized patients have nutritional risk factors. The purpose of this study was to assess the implementation of physicians' prescribed nutrition therapy before and after a protocol on nutrition therapy for malnourished inpatients was instituted. Methods: Data regarding the rates of physician implementation of nutritional therapy and the duration of therapy were collected and analyzed using SPSS version 20. Results: The percentages of physicians who prescribed nutrition therapy before and after the protocol was initiated were 47.5% and 74.6%, respectively, which statistically significant with 95% confidence. The rates of therapy lasting less than 24 hours before and after the protocol was in place were 100% and 86.4%, respectively, which was not statistically significant. This result suggests that the improvement activity was not effective. Conclusion : The percentage of physicians who prescribed nutritional therapy based on improved nutritional program was significantly increased. Nutrition therapy for malnourished inpatients can be advanced through this new model.

Evaluation of Cost-Effectiveness of Medical Nutrition Therapy : Meta-Analysis (메타분석을 이용한 임상영양서비스의 비용-효과성 평가)

  • Objectives: A meta-analysis of the literatures was conducted to evaluate the cost-effectiveness of medical nutrition therapy by dietitians. Methods : The 30 studies were identified from a computerized search of published research on MEDLINE, Science-Direct and the PQD database until May, 2002 and a review of reference lists. The main search terms were“dietitian”,“dietary intervention”,“nutrition intervention”, “cost”,“cost-effectiveness”and“cost-benefit analysis”. The subgroup analysis was performed by publication year, study design, intervention provider, type of patient (in/out-patient) and type of cost (total cost/direct cost). Two reviewers independently selected trials for inclusion, assessed the quality and extracted the data. Results : The 30 studies were identified using the electric database search and bibliographies. The 17 trials were eligible for inclusion criteria, then the systematic review and a meta-analysis were conducted on effectiveness and cost-effectiveness of medical nutrition therapy. The quality of the studies was evaluated using the quality assessment tool for observational studies. The quality score was 0.515 $\pm$ 0.121 (range : 0.279-0.711, median : 0.466). The meta-analysis of 17 studies based on the random effect model showed that medical nutrition therapy was highly effective in treating the diseases (effect size 0.3092 : 95% confidence interval 0.2282-0.3303). The vote-counting method, one of meta-analysis methods, was applied to evaluate the cost-effectiveness of medical nutrition therapy conducted by dietitians. Two criteria (method 1, method 2) for voting were used. The calculated p-values for method 1 (more conservative method) and method 2 (less conservative method) were 0.1250 and 0.0106, respectively. Medical nutrition therapy by dietitians was significantly cost-effective in the method 2. Conclusion. This meta-analysis showed that the effectiveness of medical nutrition therapy was statistically significant in treating disease (effect size 0.3092), and that the cost-effectiveness of medical nutrition therapy was statistically significant in the method 2 (less conservative method) of vote counting.   • Objective: A case report on improving the symptoms of patients with fissured tongues by Ortho-Cellular Nutrition Therapy (OCNT). Methods: A 61-year-old Korean female who not only felt pain during her daily life, but also had difficulty eating due to a fissured tongue that lasted for three years. Results: The practice of Ortho-Cellular Nutrition Therapy (OCNT) improved the patient's pain and xerostomia caused by a fissured tongue. Conclusion: Ortho-Cellular Nutrition Therapy (OCNT) can be effective in relieving the symptoms of patients with fissured tongues.

  • Objective: A case report on the improvement of metabolic syndrome by Ortho-Cellular Nutrition Therapy (OCNT). Methods: A 50-year-old Korean male with chronic fatigue and overlapping risk factors impaired fasting glucose, hypertension, and hypertriglyceridemia. Results: Diabetes and blood triglyceride levels improved after Ortho-Cellular Nutrition Therapy (OCNT). Conclusion: Ortho-Cellular Nutrition Therapy (OCNT) is effective in relieving the symptoms of metabolic syndrome patients.

  • Objective: Case report on the improvement of intestinal metaplasia through ortho-cellular nutrition therapy. Methods: A Korean woman in her 50s who is suffering from bloating, abdominal distension and frequent eructation after meals. Results: Symptoms improved one year after the introduction of nutrition therapy, and patient was diagnosed as fully recovered from reflux esophagitis, atrophic gastritis, and intestinal metaplasia through gastroscopy. Conclusion: In patients with complex gastrointestinal problems, the application of nutrition therapy may help improve and treat symptoms.

  • Objective: A case report on the improvement of patients with hematologic malignancies by Ortho-Cellular Nutrition Therapy (OCNT). Methods: A 69-year-old Korean female diagnosed with Stage 4 ACUTE LYMPHOBLASTIC Leukemia (ALL) had a very low platelet count of 8000/µl. Results: The platelet count improved to its normal level after Ortho-Cellular Nutrition Therapy (OCNT). Conclusion: Ortho-Cellular Nutrition Therapy (OCNT) is effective in relieving the symptoms of acute lymphoblastic leukemia patients with thrombocytopenia.

  • Objective: A case report on the improvement of patients with intestinal metaplasia by Ortho-Cellular Nutrition Therapy (OCNT). Methods: A 55-year-old Korean male suffering from the symptoms of the reduced digestive function, bloating, and hypoacidity. Results: The severity of the symptoms improved after Ortho-Cellular Nutrition Therapy (OCNT), and the patient was finally cured of intestinal metaplasia. Conclusion: Ortho-Cellular Nutrition Therapy (OCNT) is effective in relieving the symptoms of patients with intestinal metaplasia.

  • Objective: A case report on the improvement of hypertension patients by Ortho-Cellular Nutrition Therapy (OCNT). Methods: A 59-year-old Korean male with hypertension having bad habits as alcohol, smoking and overweight. Results: Weight loss and blood pressure improved after Ortho-Cellular Nutrition Therapy (OCNT). Conclusion: Ortho-Cellular Nutrition Therapy (OCNT) is effective in relieving the symptoms of patients with chronic symptoms to hypertension.
