Clinical diagnostic study of Physiological Signal data measured on 58 cases of numbness with EAV(Electro-puncture According to Voll) (비증환자(痺證患者) 58례(例)에 대(對)한 EAV측정치(測定値)의 진단적(診斷的) 고찰(考察))

  • Background and Objective : Most diagnostic method for numbness were invasive and complex. So we need to simplify and objectify diagnostic method for numbness. Some study with EAV which is one of Physiological Signal Measuring Instruments, report significantly result as objective diagnostic method for other clinical symptom. By using EAV, we have obtained some physiological signal data from meridian-acupoints of 58 numbness cases. Objective and Methods : This study researched into the clinical statistics for 58 case who ware in numbness, and they ware treated with oriental medical care at the Dong-shin university oriental hospital during 1 year from April 3 2000 to March 30 2001. The data were analyzed and interpreted to compare with traditional differentiation of symptom-complexes, then further evaluated as the Five Evolutive Phases to make them differentiated. The EAV valus of Five Evolutive Phases were identified with the sequence of wood(木), fire(火), earth(土), steel(金), water(水). Results and Conclusion : These values of physiological signal were identical with standard differentiation of symptom-complexes of numbness which is the main cause of dishannonious flow of Qi and blood of the in the liver and deficiency of Qi and blood of the bladder with stagnancy of dampness. Among Five Evolutive Phases, Earth and wood values were increased, steel, fire and water were decreased significantly. This data imply the possibility of somewhat generalization from measuring instruments.

  • The purpose of this study was to identity the distribution of post-traumatic stress types and coping methods and to find the relationship between the post-traumatic stress types and the coping methods, for firefighters who experienced in Dea-Gu Subway Fire Disaster. The Subjects of this study were 126 firefighters who experienced Deagu Subway Disaster. Q questionnaire developed by Q-study and coping methods instrument based on that of Folkman & Lazaruswas revised and complemented by Kim Jung Hee was used. Data were analyzed by t-test, ANOVA using SPSS. The results of this study were as follows : 1. The distribution of post-traumatic stress types were 52.4% of Emotional arousal trauma, 34.1% of Trauma experience persistence and 13.5% of Physiological symptom experience. 2. The difference of post-traumatic stress types according to the general characteristics were significantly related to the physical injury(p = .010). 3. The minimum score of coping with post-traumatic stress types was 0.07, the maximum was 2.96 and the mean score was 1.27. 4. The coping methods according to the general characteristics were significantly different at active coping method according to educational level(p = .001), passive coping method according to educational level(p = .003) and passive coping method according to diagnosis(p = 0.20). 5. The mean score of active coping method according types were Emotional arousal trauma(1.505), trauma experience persistance(1.322) and Physiological symptom experience(1.276). The mean score of passive coping method related with types were Emotional arousal trauma(1.328), trauma experience persistance(1.254) and Physiological symptom experience(1.219).

  • Wei symptom(痿證) is symptom that reveals muscle relaxation without contraction an muscle relaxation occurs in the lower or upper limbs, in severe case, leads to death. The symptoms of Wei symptom are weakness, atropy of muscle, gait disturbance, etc. We studied to evaluate the oriental medical treatment on a patient with Wei symptom. We applied various methods of oriental medical treatment including herb medicine, acupunture, moxibustion, herb acupunture. The clinical symptoms that were both lower limbs weakness, gait disturbance, etc. were improved after the methods of oriental medical treatment. The results suggest that oriental medicare is an effective treatment for Wei symptom, but more extensive research is needed.

  • Wei symptom(?證) is symptom that reveals muscle relaxation without contraction and muscle relaxation occurs in the lower or upper limb, in severe case, leads to death. This is the clinical report about the Wei symptom(?證)-patient doubt as Transverse Myelitis and Conversion Disorder. The patient was treated by acupuncture, moxibustion, herb medication(十全大補湯), electriccal stimulation theraphy, Bee Venom acupuncture, and had significant improvement in Wei symptom(?證). these results suggest the surface temperature differ remarkably from before being treated. The temperature is measured by using Digital Infrared Thermal Imaging(DITI). The results suggest that oriental medicare is an effective treatment for Wei symptom. We expected that therapeutic value of treatment of both leg Paralysis in the oriental medicine will be higher if more clinical studies and researches are accomplished.

Factors influencing Symptom Experience in Patients with Liver Cirrhosis (간경변 환자의 증상경험에 영향을 미치는 요인)

  • Purpose: The purpose of this study is to examine the factors influencing symptom experience in patients with liver cirrhosis(LC). Method: A descriptive correlational study design was used. A convenience sample of 129 subjects was recruited from the gastroenterology department at two university hospitals in Seoul, Korea. Symptom experience in LC was measured with the instrument developed by the researcher based on Theory of Unpleasant Symptoms(Lenz et al, 1995) and the Child-Pugh Score, the Korean version of Profile of Mood States, and the Family Support Questionnaire were used to identify the factors influencing symptom experience. Results: The mean score of symptom experience was relatively low(M=41.67, SD= 24.71). Among individual symptoms, fatigue had the highest score in all dimensions. Fatigue, abdominal distension and/or peripheral edema, muscle cramps, dry mouth, and change in appearance were explored as symptoms needing management. In the regression analysis, symptom experience was found to be influenced significantly by anxiety/depression($R^2=.418$, p=.000) and the severity of LC(Child-Pugh Score)($R^2=.125$, p=.000). These variables explained 54.3% of the variance in symptom experience(F=63.607. p=.000). Conclusion: It suggests that nurses need to take into consideration psychological factor as well as physiological factor in symptom management for patients with LC.

  • A majority of menopausal women experience serious depressive symptom because they are disturbed by their menstruation stop and by multiple physiological symptoms. The purpose of this study is to analyze the related factors of depressive symptom of menopausal women in Korea. A multiple regression model was used to study in association with depressive symptom, controlling for socio-economic characteristics, health status and behavior characteristics, women's health, and family support characteristics. For the data, Menopausal women were 4,680 peoples were selected for this study. PASW 18.0 was used to study in association with affecting factors of depressive symptom in the menopausal women The study shows that Education level, Marital status, Income, Employment status, Activity limitations, Perceived health status, Perceived body Image recognition, Current smoking, AUDIT, Frequency of induced abortion, Frequency of eating out, Breakfast with family, Lunch with family were associated with depressive symptom of menopausal women in Korea.

  • Bian-zheng(辨證) of Korean Medicine(KM) is based on four examinations(四診) of Korean medical doctor. The interrogation or questioning(問診) provides the most information of four examinations. The symptom obtained from the interrogation or questioning is the main basis of the Bian-zheng. KM is understood in the whole state of the body of a specified time without seeing the disease exist. So the observable symptom is disease itself. Symptom in KM is used as an important basis for the diagnosis. But if the interview when memories are not sure of the correct answer does not get much easier to find exactly the symptoms. So when recording original symptom(素證) and daily subjective symptom can be helpful for care. In this paper, we propose daily living symptom record system as a method that can be applied to the health care according to the importance of collecting the symptom in the KM. Daily living symptom record system can record the symptom in the individual to awaken daily. The system stores the symptom in structure and provides an open shared services. So it can be used as a symptom of other systems, such as PHR, EMR, CDSS. In addition, Doctor may be able to help in the treatment determined by reference to shared symptom.

Comparisons of Physiological and Psychosocial Symptoms for Breast Cancer Patients with and without Lymphedema (유방암 환자의 림프부종 유무에 따른 신체적 및 심리사회적 상태)

  • Purpose: The purpose of this study were to examine the incidence of lymphedema and to compare to physiological and psychosocial symptoms between breast cancer patients with lymphedema and without lymphedema. Method: Four hundreds fifty women with breast cancer receiving mastectomy were recruited from two out-patient breast clinics of A and S university hospitals in Seoul, Korea from October, 2004 to May, 2005. Lymphedema was defined by circumferential measurement. Differences greater than 2cm between the affected arm and normal arm were considered to be lymphedema. A descriptive design was used for this study and data was collected using a structured questionnaire. The data were analyzed using descriptive statistics and independent t-test. Results: Lymphedema developed in 112(24.9%) of the 450 subjects. Subjects with lymphedema reported more severe sensation change of arm and more depression and confusion than subjects without lymphedema. Range of motion of shoulder as a physical indicator and social support, however, were not significantly different between the two groups. Conclusion: Lymphedema is recognized as an unpleasant and uncomfortable consequence of breast cancer-related treatment. Therefore, more research is needed to evaluate the impact of lymphedema on quality of life in patients with lymphedema and the efficacy of nursing intervention.

  • To see whether this rare clinical case of patient with guillain Barre syndrome which is a type of acute inflammatory demyelinating polyneuropathy could provide further insight in categorizing Wei symptom(위증) in general. To treat Wei symptom(위증) using traditional herbal medicine Bojoongikgi-tangkami(補中益氣湯加味) and electronic acupuncture applied on the yangmyung channel(陽明經) selected in the Yellow Emperor's of internal medicine and on Panggwang chanel considered as painful lesion. There was significant improvement in motor grade of patient in spite of the period it took to recover and there was sequoia left behind as well. The basic concept of ‘treating yangmyung channel(陽明經) most of all(獨取陽明)’ is emphasized in treatment of Wei symptom(위증) and contains nourishment of middle warmer energy(補中益氣), clear yangmyung(淸化陽明).
