현곡(玄谷) 윤길영(尹吉榮)의 "한방생리학(漢方生理學)의 방법론(方法論) 연구(硏究)" 재 조명 (Yoon Gil-Young's "A Study on the Methodology of Traditional Korean Medical Physiology" : Review from an Insider's Viewpoint)

  • Yoon Gil-Young(1911-1987) was a frontier of the field of Traditional Korean Medical Physiology(TKMP) who was academically active since mid 1950s until early 1970s. He wrote several important papers and books. Among his writings, "A Study on the Methodology of Traditional Korean Medical Physiology", which was published serially in the "Korean Oriental Medical Journal" from June 1966 until March 1967, played an important role in establishing the contemporary TKMP and showed his academic stance on Traditional Korean Medicine(TKM). This review will evaluate this paper's contribution to the establishment of TKMP in historical context, summarize the unique characteristics of his understanding of TKMP by analyzing the text, and then based on those information, examine his view on TKMP and TKM. Historically, this paper was written in transition period from the traditional TKM knowledge system to the modernized one. Aim of this paper was to provide a methodology for establishing TKMP as a scientific knowledge system like that of the western medicine. Based on his study of Yin-Yang theory and theory of five circuits and six qi in Huangdi's Internal Classics, he suggested those theories as methods to observing life phenomena and systematizing the observations. And he regarded these methods as of great value in determining the unique characteristics of TKM compared to those of western medicine. Through re-reading this paper, it was found that he had pride and confidence in the methodology of TKM, and also that he thought in scientizing and modernizing TKM it was very important to understand and efficiently put in use the methodology of TKM. It was also found that his view on TKMP and TKM was pro modernization.

한의학 이론 연구의 관점에서 살펴 본 '모형 의존적 실재론' (Investigating 'Model-Dependent Realism' from the Viewpoint of the Traditional Medical Theories Research)

  • In a essay that was published on 'Science' in December 2014 as a part of the supplement "The Art and Science of Traditional Medicine," the eastern and western medical theories are discussed with reference to the model-dependent realism suggested by Stephen Hawking and Leonard Mlodinow. This paper examines what the model-dependent realism is, and how it affects the future direction of researches in traditional Korean medical theories. The model-dependent realism holds a meaning in that it puts traditional medical theories in a perspective of models, and allows for application of recent studies in scientific philosophy for researches in traditional medical theories. Especially, the model studies by R. Giere et al. will help elaborate the traditional medical theories from a model perspective. From a model perspective, the 'visceral manifestation', 'meridian and collateral', 'qi-blood', 'eight principles' and 'constitution' theories of traditional medicine have the potentials to develop into valid models, and the traditional medical theory's phenomenological and holistic perspective distinguishes it from western medicine, giving it a competitive edge. In addition, the epistemological pluralism of model-dependent realism can serve as an alternative to relativism or rationalism perspective which put eastern and western medicine in opposition until now.

  • East Asian traditional medicine(Asian medicine) should be understood correctly on the basis of Asian medical network. For the correct understanding we analyzed Asian medical history which could be divided into three categories based on Dong-Eui-Bo-Gam. There are three mechanisms of human diseases, which are the pathogenesis of six external etiological factors(wind, cold, summer-heat, dampness, dryness and fire) due to affection by exopathogen [外感六氣], the pathogenesis of five viscera due to internal injury [內傷五藏], and the pathogenesis of physical constitution due to vital activity [生命體質]. While Chinese medicine has mainly developed the pathogenesis of the former two, Korean traditional medicine has mainly developed the pathogenesis of physical constitution.

한의학 객관구조화진료시험(OSCE)을 위한 진맥 시뮬레이터 개념 연구 (Study on Pulse Simulator of Oriental Medicine for Objective Structured Clinical Examination (OSCE))

  • Objectives: The aim of this study was to examine the controversial issues about objective structured clinical examination (OSCE) of pulse diagnosis and investigate the objectification of traditional Korean medicine and technical feasibility of pulse simulation through the suggestion of a pulse simulator. Methods: The concept, validity and reliability of OSCE and the current situation of medical simulation was described. The actual level and problems on the simulator development was presented through haptics and tactile technology in order to compose a pulse simulator for OSCE. Results: The pulse wave system of a pulse simulator should materialize through haptic technology and the classification of the differences between tactile sensation and tactile quality is essential for the development of the relevant pulse simulator for OSCE in traditional Korean medicine. Conclusions: The introduction and controversial issues of OSCE and methodology of pulse wave reappearance system are proposed for the objectification of Traditional Korean Medicine and the development of clinical technology.

  • Objectives: The aim of this study is to introduce the WFME Global Standards and Recognition process and to consider Improvement direction of Korean traditional medical curriculum. Methods: To Investigate the Standards and Recognition process of WFME and the traditional medical curriculum of each country(China, Taiwan, Japan, Korea). Results: The WFME Global Standards and Recognition process aims to train doctors who are educated and active in world standard medical Curriculum. The traditional medical colleges have not received recognition, but those colleges in Korea, China and Taiwan contain a lot of standards contents, and they need to be recognized if they belong to WDMS. Conclusions: Korea University of Oriental Medicine has a lot of subjects of WFME Standards and there is a medical education recognition association, which is advantageous for the standardization process of world medical education. Therefore, it is necessary to aim at world standard medicine while preserving the tradition of Oriental medicine, WFME Global Standards should be used to reorganize the curriculum and train a world-class medical professional.

Learning experience of undergraduate medical students during 'model preparation' of physiological concepts

  • Purpose: Learning physiological concepts and their practical applications in the appropriate contexts remains a great challenge for undergraduate medical students. Hence the present study aimed to analyze the learning experience of undergraduate medical students during an active learning process of 'preparation of models' depicting physiological concepts. Methods: A total of 13 groups, involving 55 undergraduate medical students with three to five individuals in each group, were involved in model preparation. A total of 13 models were exhibited by the students. The students shared their learning experiences as responses to an open-ended questionnaire. The students' responses were analyzed and generalized comments were generated. Results: Analysis of the results showed that the act of 'model preparation' improved concept understanding, retention of knowledge, analytical skills, and referral habits. Further, the process of 'model preparation' could satisfy all types of sensory modality learners. Conclusion: This novel active method of learning could be highly significant in students' understanding and learning physiology concepts. This approach could be incorporated in the traditional instructor-centered undergraduate medical curriculum as a way to innovate it.

1951년 국민의료법 한의사 제도 입법 과정 (Process of the Legislation of the National Medical Services Law for Traditional Korean Medicine Practitioners in 1951)

  • Objectives: The aim of this study was to reflect upon the process of the legislation of the National Medical Services Law for traditional Korean medicine practitioners (TKM practitioners), especially at the Assembly plenary session of 1951. Methods: Various primary sources related to the legislation were examined, especially those in National Assembly Records and newspapers. Results: In 1950, the National Assembly wanted to establish the National Medical Services Law (國民醫療法) replacing the colonial medical services law (朝鮮醫療令), but it ended in failure. So in 1951, the National Assembly tried again. First, legislator Han Gukwon (韓國源), with 83 other legislators, introduced a bill for the new national health care system. The Society and Health (社會保健委員會) and the Legislation and Judiciary subcommittees (法制司法委員會) deliberated on this bill, and each proposed an amendment to the National Assembly. In the process of careful deliberation of these three proposals, the Ministry of Health and legislator Kim Ikgi (金翼基) each came up with a further amendment. Ultimately, Kim Ikgi's amendment was accepted by the National Assembly. According to his proposal, TKM practitioners were titled 'Hanuisa (漢醫師)', and the medical office name of TKM practitioners became 'Hanuiwon (漢醫院)'. Conclusions: The National Medical Services Law passed in 1951 was the beginning of the unique dual national medical license system of Korea. It recognized Western medicine and TKM practitioners equally under the national license system.

중의학 교육의 전문화에 대한 연구 (Study of the Professionalization of Education for Traditional Chinese Medicine)

  • Nowadays most of scholarship is based on the western model. Traditional Chinese Medical education system also follows the western medical education. In the views of medical sociology, it shows very interesting phenomenon that the modernization of traditional area follows the western model of modernization. Moreover, it provides a good chance to discuss whether modernization and westernization of tradition is real development or not. Traditional Chinese Medicine had been the only institutional medicine in China for a long time. But the status of Traditional Chinese Medicine has been changed very rapidly since modern era. Shanghai Traditional Chinese Medical School was established in 1916. But National Party government tried to abolish Traditional Chinese Medicine and it met a crisis of maintenance. But the situation has been dramatically changed when Communist Party got the power in 1949. The Communist Government needed a chief medical service. And Traditional Chinese Medicine could meet the condition. Traditional Chinese Medicine could provide also the ideology of national superiority. Therefore, Traditional Chinese Medicine has been protected and developed by the assistance of the Communist Party. In the process, Traditional Chinese Medical education has been professionalized.

오장(五臟)의 구조에 대한 통합의학적 고찰 (Integrative Study on the Structure of Five Viscera)

  • Allopathic medicine has had a lot of limits though it has maintained the status of mainstream medicine, therefore these days there has been a growing interests in traditional medicine of East Asia. According to this trend, the integration between allopathic medicine and traditional medicine has become a hot issue and the need of mutual understanding on their physiology and pathology increases. However, understanding of traditional medical theory based on ying-yang, five phases, viscera and bowels is still difficult to be accepted to allopathic medical fields. Therefore, this research tried to investigate the similarity and differences of the structures between five viscera and organs. As a result, this research found that the investigation and understanding on viscera in traditional medicine or organs in allopathic medicine has in common in some degree. However, the integration and comparison between two medicines should be done cautiously because there still are the basal different points of understanding of human structure and function.

기존치료법과 침도침 시술을 병행한 요추관협착증의 증례보고 및 분석 (Study and three Cases Report for Lumbar Spinal Stenosis Treatment Using a Combination of Acupotomy and Existing Treatments)

  • This study reports the effect of acupotomy for patients diagnosed with lumbar spinal stenosis(LSS). We treated three patients diagnosed with LSS using acupotomy. We measured the Numerical Rating Scale(NRS), Oswestry Disability Index(ODI) scores and Odom's criteria before and after treatment to assess improvements in symptoms and functions. In all patients, NRS, ODI scores and Odom's criteria were improved. We found a beneficial effect with acupotomy for three patients diagnosed with LSS. Thus, these data suggest LSS may be treated successfully with acupotomy.
